2 Mr. Larry Hermann ORIX USA Corporation 1717 Main Street, Suite 1100 Dallas, TX RE: Windermere Memory Care Ocoee, Florida Project Number: Dear Mr. Hermann Partner Engineering and Science (Partner) is pleased to provide you with the requested Weekly Construction Monitoring Report for the Windemere Memory Care facility located in Ocoee, Florida. The findings are detailed in the attached report. The main purpose of this report is to summarize the observations from the project site, comment on the percent of work completed and note any obvious issues onsite. The rendering of any conclusions and/or recommendations in the report reflect Partner s professional opinions and judgments based solely upon our assessment of the information reviewed. On May 13, 2016 Nashae Hall visited the subject property. A site meeting was scheduled between Mr. Hall and the General Contractor and is limited to a review of the weekly progress reports for the project, the status of materials stored on site and a review of schedule compliance. We appreciate the opportunity to provide these services. 3 1.1 Project Description Project Number: Inspection Number: 15 Inspected By: Nashae Hall Date of Inspection: May 13, 2016 Date of Report: Time of Site Visit: 10:00 AM Weather: 85 - Partly cloudy Prepared For: Contractor: ORIX USA Corporation NDC Construction Company Project Description: The project consists of constructing a three-story, 101,495 square foot assisted senior living center facility. The facility will include 124 patient rooms (units) with a total of 150 beds. Administrative offices are located on the first floor. The three-story wing is served by three stairwells and an elevator, and the central lobby and elevator. Common area amenities within the building include dining rooms, a private dining room, activity rooms, living rooms, laundry rooms, salon, pool, lobby, reception, mail room, community kitchen, and restrooms. Exterior amenities include; patios, putting areas, meandering walks, water pond features and pool patio. The site design includes landscaping along the perimeter of the building, parking islands and building courtyards. Adjacent to the site are protected wetlands. Project Location 1061 Tomyn Boulevard, Ocoee, FL Progress Summary / Action Items Progress Summary Site work progress has continued. The landscape and irrigation systems are continuing to be installed throughout the site. Memory Care - Installation of the interior camera system has begun, and areas of ceiling tile are open for the installation. The contractor has also continued to progress with punch work and repairs. Interior paint, doors, and hardware punch work is in progress. Rough clean has been completed; final clean still remains. Assisted Living Facility (ALF) interior construction continues to progress. Interior floor tile continues to progress, including tiling of the main lobby area were elevator equipment was stored. Interior painting, trim, and base board installation continue to make progress and elevator interiors were being prepared for wall panel installations. The exterior painting, stucco, and stone veneer façade were observed to be substantially complete.

4 Action Items The Main Distribution Panel (MDP) has been installed and energized and the 2 nd and 3 rd floor sub panels have been installed. The contractor has continued to make progress with the installation of electrical feeds and wiring. The building is energized and the 2 nd floor sub panels should be powered up next week.

5 2.0 SITE OBSERVATIONS/REMARKS 2.1 Permit Status Permit No. Date Issued Description Expiration Date /31/2014 Grading 8/31/ /18/2014 Foundation 8/18/ /18/2014 Water Main 8/18/ /2/2014 New Construction 8/2/ Manpower/Trades on Site The tradesmen on site during the inspection were site workers, landscapers, floor tile crew, drywall crew, painters, elevator crew, plumbing crew, HVAC, electrical, low voltage, and phone crews. 2.3 Materials Stored On Site and/or Off Site Per the General Contractor, the material stored on site includes tile, cabinets, kitchen equipment, elevator equipment, light fixtures, plumbing fixtures, and mailboxes. 2.4 Site Condition The site appears clean and orderly. 2.5 Quality of Construction The observed work on site appears to be in conformance with industry standards and common practices. 2.6 Schedule GC indicated the project is on track for the revised completion date of July 15, Punch List Items A punch list has not been provided for review.
