4 4 CEMENTATION A new prdigm: Cementtion procedures for Dentl implnts Prt II: Permnent & Retrievble Options for Cementtion The success of implnt restortions depends not only on the sound osseointegrtion of the implnt fixtures, but lso on the integrity of the connection of the prosthetic superstructure to these fixtures. In screw-retined designs, loosening or brekge of the screws my led to filure or replcement of the whole implnt. For cement-retined designs, dissolution of cement my led to loss of the bond between the finl prosthesis nd implnt butment, thus requiring recementtion. 1 Although the use of cemented superstructures limits the retrievbility of the prosthesis, it hs been successful in simplifying the restortive phse of tretment. An dvntge of cemented superstructures is tht conventionl prosthodontic procedures my be used during the clinicl nd lbortory phses of restortive tretment. Moreover, the cement-metl interfce llows for smll discrepncies which re not cceptble in screw-retined restortion, nd my even ct s shock bsorber. 2 One more dvntge of cemented superstructure is tht minor divergence of implnts cn be esily corrected during butment design which mkes it even esier to ccomplish esthetic restortions, s the butment bulk cn be more fvorbly mnged. 3 On the other hnd, permnent cementtion of implnt-supported restortions should only be considered when future retrievbility of the prosthesis for repir or modifiction is not desired or when surgicl correction of peri-implnt tissue problems is not nticipted. 4 Permnent cementtion my become gret disdvntge in cses of loosening of the butment screw, porcelin frcture or even if frmework frcture occurred. 5, 6 Sometimes, conventionl cst fixed prostheses re recommended to be provisionlly cemented to butment teeth in order to evlute occlusion nd tissue rections nd hs been dvocted for up to two months. 7 This lso pplies to implnt-supported nd implnt-tooth-supported prostheses which re sometimes provisionlly cemented to evlute occlusion nd occlusl loding of the implnt fixture. Provisionl cements my lso be used s finl cements in cses of implnt supported restortions. 2 The type of provisionl cements selected for given clinicl sitution controls the mount of retention obtined. 2, 8, 9 However, provisionl cementtion is unpredictble to some extent, due to the mny different fctors tht influence the retentiveness of restortion 10, 11 nd cn result in difficult retrievl or premture loosening. For implnt systems in which the butments re cemented to the fixtures, the provisionl cement must be strong enough to resist functionl forces, but wek enough to llow esy removl of the superstructure when necessry 2, 12, 13 without hrm to the butment nd implnt fixture. Implnt supported fixed restortions my be cemented to butments with provisionl luting gent to enble removl of the restortions. The tensile bond strength of the cement must be gret enough to resist lterl nd verticl forces during function. The tper, surfce re nd texture of preprtions ffect the retention of the csting. 14 With regrd to potentil retrievbility of cement-retined implnt restortions, the retentive strength of the luting gents is criticl. Nejtidnesh 11 evluted the retention vlues of implnt-supported metl copings using different luting gents. He concluded tht the dul cure resin modified glss ionomer; zinc phosphte, zinc polycrboxylte, nd Pnvi F hd sttisticlly the sme retentive qulity nd re recommended for finl, definitive cementtion of single implnt-supported restortions. The provisionl cements nd glss ionomer my llow retrievbility of these restortions. In n in vivo study, Singer nd Serfty 1 reported success when implnt-supported cst restortions were retined with provisionl luting gents. Retrievbility of the cst restortion is desirble if intervention therpy is required nd for the mintennce of the supporting tissues nd implnts. The use of either provisionl luting gent or screw should llow retrievbility; however, provisionl cement should eliminte problems noted with screwretined prostheses. When cemented superstructure is used, conventionl prosthodontic procedures my be completed in both the lbortory nd clinicl phses of restortive tretment. Tensile bond strength of permnent dentl cements, such s zinc phosphte nd glss ionomer, hs been reported with both nturl butments nd implnt butments. However, published bond strength dt for provisionl luting gents used with cst restortions cemented to implnt butments is limited. The tensile strength of the luting gent should llow retrievbility when required, yet be sufficient to retin the prostheses during function. The results of the study showed tht thermo-cycling cused significnt reduction in the retentive vlues of ll provisionl luting gents investigted. This is in greement with the results obtined by Michlkis 16 nd Kokubo. 17 Michlkis 16 concluded tht therml cycling hd detrimentl effect on the retentive properties of ll cements investigted. Also Kokubo 17 stted tht the retentive force of TempBond NE decresed significntly fter therml cycling. They concluded tht TempBond NE my not be pproprite for the retention of single-tooth zirconi butments nd coping restortions. This is in greement with our results s TempBond nd TempBond NE hd the lowest retentive lod. Also Rmp 18 found tht Temp Bond nd Provilink temporry cement exhibited the lowest men tensile bond strengths, while Neo Temp cement exhibited tensile bond strength more thn three times tht of Temp Bond. Nejtidnesh 21 evluted the retention vlue of implnt-supported zirconium oxide cermic copings using different temporry cements nd concluded tht the retention of zirconi cermic restortions over ITI solid butments my be influenced by the type of cement. Michlkis 22 evluted the filure lods of four provisionl luting gents used for the cementtion of FPDs supported by 2 implnts or 4 implnts. Nogenol luting gent exhibited the lowest retentive vlues in both types of FPD while ImProv (polyurethne) ws the most retentive of ll cements tested, possibly relted to its higher compressive strength.
