This gathering celebrates the many Green House projects growing homes across the country. Mark your calendar for this special gathering you won t want to miss! This year s theme, Sweet Homes Alabama gives a nod to The Green House Cottages at St. Martin s in the Pines and all of the organizations working to create sweet homes throughout the country, where elders and persons living with disabilities can live lives of meaning and power. We honor the commitments you have made and the hard work you are doing to make this vision a reality in your communities. We hope this experience provides you with the opportunities for the learning, growth, replenishment and fun that you so richly deserve! Visits to St. Martin s homes will be scheduled before, during and after the Conference with an exclusive evening reception for Shahbazim at the homes on Thursday evening. Dr. Bill Thomas will join with the elders and staff of many Green House homes to network, share successful practices and spend time building relationships and creating intentional community among Green House adopters. We are offering Continuing Education Credits for Nursing Home Administrators and Licensed Nurses. 6 Pre-Conference Programs and Special Events (1) Coaching Approach to Accountability, Real Accountability Wednesday, September 7, :30 am 4:30 pm Prerequisite: This program is open to only Green House Guides who have completed Coaching Supervision: The Green House Guide Education Program. Are you interested in honing your coaching skills further in an environment of collaboration and support with your fellow Green House Guides from projects across the country? Join us for this special one-day pre-conference session designed just for graduates of Coaching Supervision: The Green House Guide Education Program. In this session you will further your exploration of core elements and common challenges of a coach s role in supporting team problem-solving and conflict resolution for team development and build skills to more effectively: Balance support and accountability in your role as a Guide in team problemsolving; Manage reactivity in your role as an agent of change; Mediate conflict from a coaching perspective; This session will be facilitated by Kate Waldo, CTRS, Organizational Change Specialist with PHI National. PHI National has been a critical partner to The Green House Project in the development of The Green House coaching curriculum. (2) Exploring the Role of Directors of Nursing in The Green House model September 7 8:30 am 4:30 pm The role of Director of Nursing is key in developing and sustaining nurses and Shahbazim to practice in new ways with effectiveness. For nurses the DON is a coaching leader as she role models person-directed care, empowerment and a coaching approach to supervision. For the Shahbazim she is a coach, mentor, teacher and partner for clinical and caring excellence. The DON inspires a shared vision, models the way and encourages the heart as staff engage in a profoundly different model of care and living. In this session Nursing Leadership will: Discuss the role of nursing and nursing leadership in The Green House Model. Explore the DON role and responsibilities as a coaching leader and partner Brainstorm a position description and competencies for the DON role within this model The session will be facilitated by Susan Frazier, RN, MS and Anna Ortigara, RN, MS, FAAN with participation from Nurse Leaders from Green House Projects. 6

7 (3) Site Visits to The Cottages at St. Martin s in the Pines The Cottages at St. Marin s in the Pines are nestled on the retirement home campus in a residential neighborhood of Birmingham. These Green House homes are the first multistory homes, with beautiful three story buildings holding one Green House home per floor. In each home reside 10 elders living in intentional communities. Hear their story and visit in the homes with staff and elders. Learn about their financial results and quality outcomes. Wednesday, September 7: 9:00 am 12:00 pm 1:00 pm 4:00 pm Friday, September 9: 9:00 am 11:30 am Space is limited to the first 20 people registered for each visit. Additional Fee: $25 Includes round-trip bus transportation to be provided from The Wynfrey Hotel (4) **Site Visit and Evening Reception: Shahbazim and Future Shahbazim Only. Thursday, September 8 5:00 pm 8:30 pm Shahbazim and Future Shahbazim are invited to a reception at The Cottages at St.Martin s Thursday evening. There will be no charge for this event, but please register so that we may determine attendance. The hotel rate is $109 per night for a single or double room. Continuing Education This Conference will be submitted for NHA and RN Continuing Education Credits. The National Continuing Education Review Service (NCERS) of the National Association of Long Term Care Administrator Boards (NAB). Rush University College of Nursing is an approved provider of continuing nursing education by the Illinois Nurses Association, an accredited approver by the American Nurses Credentialing Center s Commission on Accreditation.
