JOB DESCRIPTION POST: COMMUNITY LEARNING DISABILITY NURSE GRADE: REGISTERED NURSE BAND 6 REPORTS TO: ACCOUNTABLE TO: COMMUNITY TEAM LEADER SERVICE MANAGER, COMMUNITY TREATMENT AND SUPPORT SERVICES JOB SUMMARY/MAIN PURPOSE The post holder will work as a member of a multi-disciplinary team to provide a person centred nursing service to people with learning disabilities. The post holder will be responsible for the assessment, planning, implementation and evaluation of nursing care. Whilst it is envisaged that this job will normally require 9-5, Monday to Friday, working hours, future service development may require the post holder to work extended hours and/or on call as part of a 7 day a week service. The post holder may therefore be required to work shift patterns in line with Trust policy, European Working Time Directive and Agenda for Change terms and conditions for on call and shift work. Main Responsibilities/ Functions 1. Assesses, plans, implements and evaluates nursing care. 2. Ensures that programmes of nursing care are of a high standard. 3. Carries out nursing procedures as delegated. 4. Carries out assessment and interventions relating to challenging behaviour. 5. Promotes positive mental and physical health for clients, in partnership with all others involved. 6. Acts as an Investigating Officer for adult safeguarding procedures as required. 7. Participates in case conferences and other meetings as required

8. Maintains high standards of record keeping.. 9. Works collaboratively with other parts of the community learning disability services. 10. Works collaboratively with hospital services. 11. Works collaboratively with private, voluntary and independent providers. 12. Participates in the care management process as required. 13. Reports to Professional Nurse Advisor on clinical aspects of the service and works within all NMC policies and standards. 18. Ensures the economic use of all supplies and the maintenance of equipment in a satisfactory working condition. 19. Provides statistical, clinical and other information as required by the team leader. 20. Carries out other duties as delegated by the team leader. Records Management Used as part of their business with the Belfast Health and Social Care Trust, including patient/client, corporate and administrative records whether paper based or electronic and also including e-mails. All such records are public records and are accessible to the general public, with limited exceptions, under the Freedom of Information Act 2000, the Environment Regulation 2004 and Data Protection Act 1998. Employees are required to be conversant with the Belfast Health and Social Care Trust policy and procedure on records management and to seek advice if in doubt. Environmental Cleaning Strategy The Trusts Environmental Cleaning Strategy recognizes the key principle that Cleanliness matters is everyone s responsibility, not just the cleaners Whilst there are staff employed who are responsible for cleaning services, all Trust staff have a responsibility to ensure a clean, comfortable, safe environment for patients, clients, residents, visitors, staff and members of the general public. Infection Prevention and Control The Belfast Trust is committed to reducing Healthcare associated infections (HCAIs) and all staff have a part to play in making this happen. Staff must comply with all policies in relation to Infection Prevention and Control and with ongoing reduction strategies. Standard Infection Prevention and Control Precautions must be used at all times to ensure the safety of patients and staff. This includes:- Cleaning hands either with soap and water or a hand sanitiser at the appropriate times (WHO 5 moments ); Using the correct 7 step hand hygiene technique; Being bare below the elbows when in a clinical environment; Following Trust policies and the Regional Infection Control Manual (found on intranet); Wearing the correct Personal Protective Equipment (PPE);

Ensuring correct handling and disposal of waste (including sharps) and laundry; Ensuring all medical devices (equipment) are decontaminated appropriately ie cleaned, disinfected and/or sterilised; Ensuring compliance with High Impact Interventions. Learning Disability You are applying for a post working with people with a learning disability. You should be aware that given the nature of the disability of some of the people with whom you will be working you may witness or experience challenging behaviours. These behaviours are likely to include verbal and/or physical abuse displayed by people towards themselves, other clients or staff. Given this you should consider your application carefully if you think that working in such an environment would present you with particular problems. P E R S O N N E L S P E C I F I C A T I O N COMMUNITY LEARNING DISABILITY NURSE (BAND 6) ESSENTIAL 1. Must be a current employee of the Belfast Trust or must be currently working in the Belfast Trust as an Agency Worker 2. Registered Learning Disability Nurse on Part 1 of the NMC Register 3. Minimum of 3 years post-qualification experience at Band 5 in Learning Disability Nursing, or equivalent 4. Hold a current UK Driving Licence and has access to a car- this criteria will be waived in the case of an applicant whose disability prohibits driving, but who is able to organise alternative arrangements. NOTE: Where educational/professional qualifications form part of the criteria you will be required, if shortlisted for interview, to produce original certificates, and one photocopy of same, issued by the appropriate authority. Only those certificates relevant to the shortlisting criteria should be produced. If educational certificates are not available an original letter and photocopy of same detailing examination results from your school or college will be accepted as an alternative. If successful you will be required to produce documentary evidence that you are legally entitled to live and work in the United Kingdom. This documentation can be a P45, payslip, National Insurance Card or a birth certificate confirming birth in the United Kingdom or the Republic of Ireland. Failure to produce evidence will result in a non appointment.
