This position involves providing Assistant services to the Home Based Rehabilitation program to clients in their homes under the direction and support of Clinicians. Some involvement in centre based rehabilitation programs may also be required. Organisation Overview: West Gippsland Healthcare Group is a sub-regional public hospital providing 77 Hospital and 110 Aged Care beds. We provide acute medical and surgical, paediatric, obstetrics & gynaecology, emergency, aged care and community services, primarily in the Shire of Baw Baw, approximately 100 kms east of Melbourne. Qualifications/Experience: Mandatory: Recognised Assistant Certificate IV or equivalent qualification. Experience in provision of therapy support services Experience working as part of a team Experience with computer skills including word processing programs Experience in general administration /clerical duties Key Selection Criteria: Key Competencies: Excellent communication (both verbal & written) skills. Ability to work effectively with minimum supervision to implement therapeutic and related activities in a variety of environments, including home. Ability to complete allocated tasks promptly and competently. Ability to apply theoretical knowledge base to clients. Clear understanding of scope of practice for this role. Ability to identify circumstances or changes in patient status requiring additional input from an Clinician. Computer skills including use of Microsoft Word, Outlook and Excel. Good organisational skills. Current Victorian Driver s Licence. Experience in independently delivering therapy programs. Ability to monitor changes in patient condition and suggest appropriate interventions based on these changes. Understanding of the therapy role in the Community Rehabilitation / Home Based Rehabilitation setting. Sound knowledge base in areas particular to the provision of Assistant services. Knowledge of computer programs including Microsoft Word, Outlook and Excel. Personal Attributes: Ability to relate well to clients from diverse backgrounds. Excellent interpersonal skills. Key Responsibilities: Support Services: Provide therapy support services as part of a client-centred, multi-disciplinary team in the Community Rehabilitation Centre Home based Rehabilitation Service and
centre based services (as required) in accordance with relevant policies and procedures (both program and organisation and facility wide). Provision of therapeutic activities /services devised and delivered with the support of Community Rehabilitation Centre / Home Based Rehabilitation Service Allied Health Clinicians. Communicate desired outcomes to patients, family members /caregivers and relevant staff as appropriate. Prepares resources and environment and returns same to previous state in a timely manner. Ensures services and information are delivered in a professional and timely manner with a strong customer focus Encourages patient involvement in their care. Teamwork: Participate in planning, implementation and evaluation of therapy sessions Attend team/staff meetings and participate in transfer and discharge planning. Liaise with relevant members of the Healthcare team. Resource Management: Ensure equipment and supplies are used efficiently and according to procedures / guidelines. Reporting and recording of client / patient information: Document therapy delivery as per required standards Documentation of daily statistics as per required standards. Records daily details of client contact via statistics as required by the Community Rehabilitation Centre. Report on client / patient progress appropriately. Confidentiality: Ensure that any personal, private or sensitive information obtained regarding a staff member or client remains confidential. Ensure that effective lines of communication are maintained with key stakeholders. To participate in an annual performance appraisal every twelve (12) months. Actively seek feedback from Stakeholders (Health Professionals, Consumers, Carers & the Community).
