This is demonstrated by the strong delivery of key outcomes, resulting in significantly reduced lengths of stay and the successful transition back into the community. Bromley Road offers individualised person-centred programmes of care for patients from both general and forensic backgrounds. A key part of recovery is the next step on a pathway. From admission, the key focus is on supporting a further transition to more independent living environments. Choice Lifestyles is also able to offer a step down from Bromley Road to Albion House, a nearby residential service with therapeutic input based in Lewisham.

OUR SERVICE Bromley Road offers individualised person-centered programmes of care for men and women from both general and forensic backgrounds that have mental health problems, complex needs and meeting the needs of service users with learning disabilities. We have achieved an enviable reputation for delivering high quality, evidenced based care. This is demonstrated by the strong delivery of key outcomes, resulting in significantly reduced lengths of stay and the successful transition of clients back into the community. The addition of step-down facility of Albion House, enhances this unique pathway and supports a smooth transition back to a life of the client s choice. Bromley Road is a 24-bedded rehabilitation hospital for men and women over the age of 18, situated in the heart of the community in Catford, SE London. Evans Boasiako - Hospital Director Service users admitted to Bromley Road may be informal or detained under the Mental Health Act with a primary diagnosis of Mental Health illness with complex needs. Service users may present with Schizophrenia and other mental health disorders Personality Disorders Substance misuse History of complex trauma or abuse Eating disorders Lisa Murdock - Hospital Administrator Forensic background Self harm and/or suicide risk Violence or aggression Mild learning or developmental disabilities

ADMISSION CRITERIA Primary diagnosis of mental health issues, but may also present with: Treatment resistant disorders/difficult to treat illness Personality Disorders A forensic background Challenging behaviours such as aggression, risk of self-harm and risk of suicide History of emotional, physical and sexual abuse Mild learning difficulties and other causes of cognitive impairments Co morbid, eating disorders, substance misuse REFERRAL PROCESS Each patient will be assessed by two members of the clinical team, if it is agreed that the patient would benefit from the service, a pre-admission report, initial care plan and social inclusion report will be generated and sent to the referring professional/care co-ordinator. ASSESSMENT Once admitted the client will undergo an initial assessment where they are supported to engage in their individual care package taking into account related issues and risks. The philosophy of the service is driven by the principles of rehabilitation and recovery, underpinned by social inclusion and robust risk management. Bromley Road treats all our patients as individuals and uses an eclectic treatment approach. There is a wide range of treatments on offer, including: Specialist assessment and therapy Assessment of personality and risk Indivualised formulations Individual work drawing from CBT, DBT Skills Group. CBT group programmes that including anxiety management, assertiveness training, anger management, self esteem group. Medication education, self medication and monitoring Social inclusion plans Complimentary therapies Education involvement Occupational Therapy assessment such as community risk assessment and assessments of activities and daily living. FAMILY & FRIENDS The team at Bromley Road recognises the importance of family and friends and the role they play in recovery and encourage their involvement in their care, with the agreement of the client.

CLINICALLY LED TEAM The service is led by a strong clinical multi-disciplinary team who work corroboratively with the patient and community team to achieve the best outcome for the patient. The care follows the social inclusion recovery-focussed and psycho-social models of care. Outcomes measures such as the Recovery Star and HoNOs are used. Bromley Road takes into account specific needs around gender, ethnicity, sexuality and religion. A dedicated team will support the client to achieve their individual goals and targets by encouraging structured daily activities. PSYCHOLOGY Psychological work focuses on the service users thoughts and feelings and how these manifest in behaviour. Psychological therapy may include: 1:1 sessions to discuss management of specific difficulties associated with symptoms of the patient s diagnosis. Cognitive behavioural therapy Self-management strategies Approaches to facilitate communication between staff and service users Support following specific incidents. Development of alternative strategies for individuals who self harm Substance misuse work. Gillian Bailey - Clinical Psychologist OUR MDT APPROACH We provide bespoke packages of care to individual service users ensuring they have regular, meaningful activities on a weekly basis. Bromley Road is keen to ensure all service users engage in at least 25 hours of therapy or meaningful activities per week. Service users are given an individualised activity planner and are encouraged to attend specific, targeted activities to aid their recovery. Each service user at Bromley Road will be offered approximately 40 hours of meaningful activities and therapies per week. We offer a wide range of Psychological Therapies, Occupational Therapy Interventions and Social Inclusion Activities. OT Occupational Therapy focuses on supporting service users to engage in the activities and life roles that are important and meaningful to them. Occupational Therapy works with service users to develop the skills they need to live independently and successfully move on to supported accommodation or living in the community. Examples of OT interventions may include: Independent Living Skills Assessment (ILSA Rehab) Unique assessment tool developed by Choice Lifestyles staff. Ward based exercise and physical activity groups Gardening Vocational skills development Working towards obtaining recognised ASDAN qualification. Bromley Road Hospital has recently registered with the ASDAN organisation as a provider of recognised qualifications. This enables services users to work towards achieving individualised qualifications that are recognised by colleges, universities and employers during their stay at Bromley Road. The individually tailored qualifications are suitable for a wide range of service users including those with mild-moderate learning disabilities. Educational and vocational input is recognised as a key aspect of recovery in many healthcare models (particularly Occupational Therapy theory and the Recovery Star) and Bromley Road is pleased to offer this as part of our well rounded therapeutic services. SOCIAL INCLUSION Social Inclusion is based on the principle of the service user being part of their local community, making productive use of leave and re-establishing a balanced occupational routine. Examples of social inclusion activities include: Cinema Breakfast clubs and coffee mornings Bowling Engaging in college course or other educational activities. Support to attend Church, Mosque or meet any other religious/spiritual needs. Visiting areas/events of interest. Gym Swimming Snooker Walking club Many other activities are also offered based on the individual wishes and needs of the service users. PATHWAYS OF CARE A key part of the clients recovery is the next step on their pathway. From admission, the key focus is on supporting a recovery pathway to a less supportive environment and community living. Bromley Road has the added advantage of being able to offer Albion House as a step down, 24 hour residential service, which shares the same clinical team ethos and receives input from the same therapy staff which enables rehabilitation and independent living skill development to continue.
