Field Trip Permission Slip - Summer Camp 2016 Please sign and return this permission slip to your child s teacher or to the school office prior to your child s first field trip. This gives Pilgrim Lutheran School permission to take your child on any of the field trips listed on the bottom of this permission slip. Your child will be transported to and from each field trip by a chartered bus with a professional driver. The first field trip is Wednesday, June 8, 2016. Please review the attached field trip packet for specific details on each field trip. Please return this form prior to your child s first field trip. Child s Name (Please Print Clearly): has my permission to attend the field trips selected below during Pilgrim Lutheran School s 2016 Summer Camp. Field Trips begin June 8, 2016 and end on August 11, 2016. Parent s Signature Date Please put an X next to the field trips you think your child may attend even if plans change later. This will help us have an idea of how many are going on each trip and to let the places we are visiting know as well. Your child will NOT be allowed to attend any field trips until this permission slip is completed, signed and turned in. We will be visiting the Bellaire Family Aquatic Center on the afternoons listed below. There are lifeguards on duty as well as swimming areas with shallow water. Can your child swim? Hello K-5 th Summer Camp Parent! Please see below for specific field trip information for the rest of the summer. Please keep this sheet as your reference so you will know what time we will depart each day, when we will return and what to bring if anything. Please have your child here at least 30 minutes prior to departure so that we have adequate time to get prepared and leave on time. All field trips are at no cost to you with the exception of snow cones at the pool or snacks at the movie. If you would like your child to have snacks at the movie, please give the teacher snack money in an envelope marked with your child s name and labeled, Movie Snack Money. Snow cones at the pool are $4.00 each. If you want to send snow cone money, please put it in an envelope marked with your child s name and labeled, Snow Cone Money. Wednesday, June 8th Houston Museum of Natural Science: Block Party Please have your child at Pilgrim no later than 8:30am for a 9am departure. We have a scheduled group time for the Block Party (Interactive play designed to inspire imaginations of all ages) at 9:45am. At 11am the children will see National Parks Adventure in 3D. Lunch will be eaten at McDonalds in the museum after the movie. An order form will come home prior to the field trip. Thursday, June 9th Miller Outdoor Theater - James & The Giant Peach Please have your child here no later than 9:45am for a 10:15am departure. We will arrive back at the school by 12:20pm to eat lunch. Please pack a extra water bottles or a refillable water bottle for the program. It is outdoors and will be hot. Wednesday, June 15th Main Street Theater Shrek the Musical Please have your child at Pilgrim no later than 8:15am for a 8:45am departure. The performance starts at 9:30am and we are required to be there early for parking and seating. We will return to the school for lunch before going to the pool. Please send a cold lunch if you did not order lunch through the lunch program. Thursday, June 16th Sugar Land Skeeters Baseball Club Please have your child here by 9am for a 9:30am departure. We will arrive at the ballpark for a tour, then have a hotdog lunch and time for play on the field. They will have water activities on the field, so please have your child wear their bathing suit and bring a bag with a towel, extra clothes to change into as well as sunscreen. We will return to the school by 1pm.

Wednesday, June 22nd Sky Zone Trampoline Park Please have your child here no later than 9:15am for a 9:45am departure. All children must have a signed waiver (2nd page behind the permission slip) in order to jump. If we do not have a waiver, your child will not be allowed to jump. If your child has a pair of Sky Zone socks, please bring them. If not, we will purchase socks for everyone and the children will be able to keep them for their next visit to Sky Zone! We will return to Pilgrim by 12:30 for lunch. Thursday, June 23rd Wednesday, June 29th Thursday, June 30th Houston Downtown Aquarium Please have your child here no later than 8:55am for a 9:15am departure. We are scheduled to be at the Aquarium at 10am. The children will have lots of time to explore the Aquarium Adventure Exhibit before eating lunch at the Aquarium. Lunch is included in this field trip. The Aquarium lunch includes turkey sandwich, chips, cookie, fruit and a drink. After lunch we will continue to explore and then go on the Shark Voyage Train Ride and other attractions. We will arrive back at the school by 2:30pm. Part of the day will be spent outside and we learned last year that they do not have outside water fountains. Please pack extra water bottles or a refillable water bottle for your child. Houston Museum of Natural Science: Dinosaur 3D & Butterfly Exhibit Please have your child at Pilgrim no later than 8:30am for a 9am departure. The Dinosaur 3D movie begins at 10am followed by the Butterfly Exhibit at 11am. After the butterfly exhibit, the children will eat lunch at McDonalds in the museum. An order form will come home prior to the trip. NRG Stadium Tour Please have your child here no later than 10:55am for an 11:25 departure. Our tour is scheduled at 12pm. We plan to return to Pilgrim by 1:30pm. We will eat lunch early due to the required departure time. Please send a cold lunch today since hot lunch is served at 11:30am and we will depart prior to that. Wednesday, July 6th Houston Zoo Please have your child at Pilgrim no later than 8:30am for an 9am departure. We will eat lunch at the zoo around 11am (lunch choices will be sent home prior to the field trip) and then continue to explore until it is time to leave. We plan to return to Pilgrim by 2:00pm Thursday, July 7th Children s Museum of Houston Please have your child at Pilgrim no later than 8:45am. We will depart Pilgrim no later than 9:15am. We are scheduled to be at the museum at 10am but extra time is needed for parking, etc. The students will tour the main exhibit halls and then have lunch at the museum café. Lunch consists of a hot dog, chips, drink, fresh fruit and a cookie. Lunch is included at no cost to you. We plan to return to the school by 1:30pm.

Wednesday, July 13th Movie at Edward s Theater The Secret Life of Pets First Showing (times have not been released but are generally at 11 or 11:30am. Departure times will be updated when times are released. Lunch plans will be finalized when the movie time is confirmed and parents will be updated prior to the day of the movie. Movie snacks will be at the parent s expense. Please see page 2 of this packet for movie snack money instructions. Wednesday, July 13th Bowling, Games & Pizza at Main Event Please have your children here by 8:45am for a 9:15am departure. We will arrive early to get settled and fitted with bowling shoes. We are scheduled to bowl from 10am-11am, eat lunch from 11am-11:45am and then each child will get a $10 Fun Card to play games until we leave. We will depart at 12:30 and head straight to the Bellaire Pool. Please be sure to pack extra sunscreen for your child, a towel and a change of clothes. Wednesday, July 20th Planet WOW! Indoor Bounce and Games Please have your child at Pilgrim no later than 8:50am for a 9:20am departure. This gives us time to get there, unload the bus and be ready to play by 10am. We will play until 11:30am and then head back to the school for lunch before going to the pool. Thursday, July 21st Mad Potter Please have your child here no later than 9:15am for a 9:45am departure. The children will be able to pick their own piece to paint in the $10 range. When they are done, we will head back to Pilgrim for lunch before heading to the pool. Please send a cold lunch or order through the lunch program. The children will not come home with their ceramics today. The ceramics will stay at Mad Potter to be fired and then we will pick them up when they are ready. It typically takes 2 days. We will send them home when they are ready. Please be sure to pack extra sunscreen for your child, a towel and a change of clothes. Wednesday, August 3rd Movie at Edward s Theater Ice Age: Collision Course First Showing (times have not been released but are generally at 11 or 11:30am. Departure times will be updated when times are released. Lunch plans will be finalized when the movie time is confirmed and parents will be updated prior to the day of the movie. Movie snacks will be at the parent s expense. Please see page 2 of this packet for movie snack money instructions.

Thursday, August 4th Ice Skating at the Galleria Please have your child here no later than 8:30am for a 9:00am departure. We are scheduled to skate at 10am, but it will take time park, rent skates and put them on, etc. We will skate until 12pm. Lunch will be eaten at McDonalds by the ice rink at 12:30pm. An order form will come home prior to the field trip. Please return it to the teacher by the morning of the field trip with your lunch selection. We will depart the Galleria at 1:30pm and will return to Pilgrim by 2pm. Wednesday, August 10th - To Be Determined... Thursday, August 11th Monkey Bizness Please have your child here no later than 9:20 for a 9:50am departure. We are scheduled to start playing at 10:30am. Campers will play until 12pm and then will eat lunch at Monkey Bizness. Lunch is included. After lunch we will head directly to the pool. Lunch will be provided on the following field trips. Please do NOT order lunch from the hot lunch program on the following days: Wednesday, June 8th - Houston Science Museum Thursday, June 16th - Sugar Land Skeeters Baseball Club Thursday, June 23rd - Downtown Aquarium Wednesday, June 19th - Houston Science Museum Wednesday, July 6th - Houston Zoo Thursday, July 7th - Children s Museum Wednesday, July 13th - Main Event Thursday, August 4th - Ice Skating at the Galleria Thursday, August 11th - Monkey Bizness ***Lunch may be eaten at Edwards Theater on Wednesday, July 13th and Wednesday, August 3rd, depending on the movie times. Notice will go home prior to the field trip dates in plenty of time to order lunch from the lunch program, if necessary. Please fill out the waiver on the following page for the Sky Zone Trampoline field trip on Wednesday, June 22nd. Your child will not be allowed to jump without a signed waiver!
