SUMMER CAMP PERMISSION - SWIMMING My child has permission to participate in swimming activities with the FSUS Summer Camp. - TRANSPORTATION I understand that all field trips provided during summer camp hours are taken on FSUS buses, another company school bus, or charter buses. My child has permission to ride these buses when I have been informed of the schedule of trips to be taken. - MOVIE I understand PG rated movies will be shown during summer camp. My child has permission to watch PG rated movies. - POLICY ACKNOWLEDGEMENT I have read and fully understand the policies in the Policy Statement of FSUS Summer Camp. - OUTSIDE WAIVERS Some field trip locations require a waiver of their own. In order for your child to participate on the field trip at those locations YOU are responsible for completing the waiver. Field Trips that require the waiver have an asterisk next to them on the schedule, further along in the registration form. The waiver for skyzone can be completed by going to The waiver for rock climbing will be distributed the Monday of that week of camp. Parent Signature: Date: The fees for summer camp are designed to cover the cost of staffing, materials, supplies, field trip entrance fees, bus costs, t-shirts, and use of school facilities. In completing the registration for my child, I understand and agree that: 1. The registration and weekly fees are non-refundable. 2. The registration fee must be paid at the time of registration to reserve a space at camp. 3. I will be responsible for the weekly camp fee on no later than Monday at time of drop off each week that my child is registered. My child will not be able remain at camp if fees for the week are not paid prior to my child starting camp each week. 4. The camp will not offer a daily rate or partial week option. 5. I will be responsible for payment anytime that I am late picking up my child from camp as outlined in the policy statement. 6. I am responsible for the full pay of all the weeks checked on the registration form, I must pay the fee even if my child doesn t attend. 7. I understand that I may only add weeks to this registration if space permits. I have read the above payment contract and agree to all the payment requirements for the summer camp program. 3 Florida State University School SUMMER CAMP Parent Release Form for Media Recording I, the undersigned, do hereby grant or deny permission to Florida State University School (FSUS) to use the image of my child,, as marked by my selection(s) below. Such use includes the display, distribution, publication, transmission, or otherwise use of photographs, images, and/or video taken of my child for use in materials that include, but may not be limited to, printed materials such as brochures and newsletters, videos, and digital images such as those on the FSUS Blackboard and FSUS Web site. 4 Welcome Parents! Welcome to the FSUS EDEP Summer Camp! We are glad that you ve chosen to spend some memorable moments with us this summer. We look forward to providing an exciting summer, filled with wonderful summertime experiences. The scheduled activities are designed to offer opportunities for young people to try new things, learn, grow, and make friendships that last forever. Additionally, we strive to offer a healthy, safe, and fun environment with positive interactions with peers and adults. This handbook has been created as a reference guide for both parents and children. It contains vital information and policies about our summer program. Your child s safety is a top priority and we take this responsibility very seriously. Please take time with your child to read and review the entire handbook. If you have any questions or comments, please call the Coordinator or the Director at We would love to hear from you. Please be sure you have read all of the information contained in this packet. By signing and dating the registration form, it is understood that you and your child have read and are willing to comply with all of the rules and regulations within this manual. General Summer Camp Information Camp Location: 3000 School House Road, Tallahassee We will meet daily in the cafeteria. Camp Hours: 7:00am 6:00pm Camp will open at 7:00 a.m. Please do not arrive prior to 7:00am. Your child does not need to arrive that early unless your work schedule demands it. In general, our daily activities begin by 9:30 a.m., unless otherwise indicated on your calendar. Please pay close attention to field trips by checking the information board and fliers to ensure that your child is on campus in time to catch the bus. We cannot leave teachers on campus for those who arrive late. Camp will end promptly at 6:00 p.m. See late fees for more information. If an emergency situation should arise, and you cannot pick your child up by 6:00 p.m., please call the E.D.E.P. office ( ), and leave a message if there is no answer. Your call will be returned at some time during the day. You can us at prosati@fsu.edu or channa@fsu.edu Communication: Drop Off: Pick Up: On Monday of every week you will receive important information regarding schedules, field trips, special events, etc. Please read all information carefully and save it for future reference. Additionally, each day we will write important information for the upcoming day on the white board by the checkout desk. It is important to read this information so you can prepare accordingly. Each day you should sign your child in at the sign-in desk when you bring them to camp. Should your child be sent in without you, please instruct your child to check in at the desk before they join the group. If your child is arriving later than 9:30am, you MUST walk your child in. You are required personally to come to the cafeteria to sign your child out each day by 6:00pm. Please bring a photo ID with you every day. If you need to have a friend or relative pick your child up, who is not listed on your registration form, be sure to send a signed note with your child that morning in order for us to release your child to that person. They must bring in a photo ID as well. Late Pick Up: We close promptly at 6:00pm. For all children not picked up by the end of the program, $5.00 will be assessed per child for every 5 minutes after 6:00. The late fee is in place to compensate staff members for their time. If you know you will be late, please attempt to make alternate pick-up arrangements. Please notify us should you foresee a late arrival, as it is often stressful for the child when a parent is late. Lunch and Snacks: Field Trips: Storage: Camp T-shirt: Lost & Found: Lunch will be eaten anywhere between 11:30 and 12:30 depending upon the schedule and level of fun they are having with the pre-lunch activity. Your child must bring a labeled lunch and drink each day, unless otherwise stated on memos. Lunch boxes or lunch coolers are preferred due to the fact that paper bags often rip during travel or when wet from drink boxes. We will be offering slices of pizza on Fridays of each week as a lunch option for $1.00 per slice. You may send as much money as you think necessary for your child to enjoy a filling lunch. Your child must still bring a drink on those days. You will also need to send in 2 snacks per day, one for the morning and one for the afternoon. Please let us know if your child has any food allergies. We do not have the resources to heat food. We go on field trips nearly daily. Please be aware of what field trips are scheduled each week so you can prepare your child and pack accordingly. All field trips are subject to change. Please have your child bring a crate with his/her name on it for the duration of camp. This will be used to store belongings. PLEASE LABEL EVERYTHING YOUR CHILD BRINGS TO CAMP! The children are responsible for their own belongings. Each camper will receive one camp t-shirt for the entire summer. You may purchase additional shirts. It is highly recommended that you label all items (clothing, towels, lunch box, water bottles, etc.) with your child s name. While we make every effort to keep all campers belongings in their crate, we will not be held responsible for lost or stolen items. Each day the lost and found items will be displayed at a table in the back of the cafeteria. At the end of each week, we will take any remaining items to Goodwill. Please make a quick check of your child s crate at the end of the week/day before leaving. Lost items are much easier to recover the same day they are lost.

5 Dress: Please remember to dress your child appropriately for camp. Camp can be messy so we encourage you to dress your child in clothing that you do not mind getting dirty or stained with art materials in the course of your child engaging in a fun activity. Here are some reminders of clothes for camp: T-shirts Shorts (No skirts) Socks Appropriate Clothing for Camp ONLY (No halter or tube tops, No short shorts, etc!) Tennis Shoes (No sandals or flips flops unless it is a water day) LABEL EVERYTHING! Enrollment, Withdrawal & Payment Policies Registration: Cost: The registration fee this year is $50.00 per camper which is non-refundable. The fee must accompany the registration form to ensure that your child has a space reserved for him/her. The cost of camp is $ per week per child, whether your child comes one day or five. We do offer 10% off for each additional sibling. Checks or money orders may be made out to FSUS Summer Camp. All of the weekly field trips will be covered by your fees. On some field trips you will have the option of providing your child with spending money at gift shops or for additional snacks. Days that allow extra spending money will be noted on the weekly agenda that will be distributed on Mondays. We ask that unless it is specified that extra spending money is permitted you do not send your child with any cash. Camp fees are due no later than Monday morning of the week your child will be attending. If payment is not received on Monday morning, your child will not be permitted to stay at camp. You must pay for the weeks you register for, even if you do not send your child to camp. Spots are limited and registering your child for a week means that another child may miss out on that spot. Refunds: No refunds are permitted after the first Monday of each week, except for prolonged illness or family relocation. Behavioral Expectations Conduct: Behavioral Guidelines: Field Trips Transportation: Bus safety: To ensure the safety and comfort for all, we ask individuals to act appropriately while they are in our facility or participating in our programs. We expect all to behave in a mature and responsible way and to respect the rights and dignity of others. Please refrain from using language or actions that can hurt or frighten another person or that falls below a generally accepted standard of conduct. Specifically this includes: Angry or vulgar language including swearing, name calling, and shouting; Physical contact with another person in an angry or threatening way; Any demonstration of sexual activity or sexual contact with another person; Harassment or intimidation with words, gestures, body language, or other menacing behavior; Behavior which intends to or results in the theft or destruction of property; Carrying or concealing any weapons or devices that may be used as weapons. In order to promote your child s physical, intellectual, emotional, and social well-being and growth, staff shall interact with the child and one another to provide needed help, comfort, and support. Behavioral guidance will be constructive in nature, age, and stage appropriate, and will be intended to redirect children to appropriate behavior and resolve conflicts. Parents will be notified when persistent behavioral problems are identified and will include any disciplinary steps taken in response. Whenever we are taking a field trip, we will be traveling on either FSUS school buses or charter buses. Please have your child at FSUS in time to board the bus. We cannot give refunds for no-shows. In order to ensure the safety of all of our campers the following rules are enforced while on the bus: Remain seated while the bus is in motion. Hands, heads, arms, trash, etc. must not go out the window. Quiet inside voices must be used at all times. Legs must be clear of the aisles. Follow the bus driver s instructions. Sit facing front at all times.

We reserve the right to change our weekly themes or activities. We will make you aware of any changes as soon as possible. Field trips may change without notice based on weather or other problems that may occur with the field trip location but changes will be kept to a minimum. Medical & Emergency Information Illness or Injury: Accidents: Allergies: Children must be healthy enough to participate in the program s daily routine. We do not have the facilities to care for sick children and therefore do not allow them to attend the program. For the safety and comfort of your child, please keep them home until they feel better and no longer present the danger of passing on their illness. If you are keeping your child home due to illness, please contact the camp by 9:30am and let the staff know of your child's absence. When your child has a fever (fever of 101) or vomiting/diarrhea, please make sure they remain at home 24 hours after their temperature and symptoms return to normal. If your child becomes ill while at camp, you will be asked to pick up your child as soon as possible. All precautions will be taken to prevent serious health risks to all campers. In the event that a minor injury occurs, First Aid will be administered at the camp location by the camp staff and an Accident Report will be written up. In recent years, there has been an increase in the number of children with severe allergies to peanut products and other things in our camps. We try our best to accommodate these campers without inconveniencing other campers. If you re aware that your child is severely allergic to something, it is your responsibility to notify us in advance so we may take proper precautions.

7 Emergency Contact Information: Emergency information is very important for us to provide the safest possible environment for your children. Please notify us right away when there is a new work or home phone number, or if you have moved and have a new address. If your child is sick or injured, it is important for us to be able to contact you right away. Please keep these accurate at all times. Emergencies: In the event of a medical emergency, immediate action will be taken by the staff as per your orders on the camp registration form and policies and waivers. Please be sure to keep these forms updated at all times. If parents or other responsible adults are unable to be reached, the child will be taken to the nearest hospital for any necessary treatment. In general, in the event that a major injury or health problem arises and professional medical care is required, the following steps will be taken: Immediate First Aid will be administered by the camp staff person until professional services arrive. You will be contacted. If you cannot be reached, the emergency contact person will be notified. 911 will be called. A staff person will accompany your child to the hospital and remain until you or your emergency contact person arrives. The incident will be described in writing on an Accident Report Form. Medication: Personal Belongings Our medication policy is primarily established to accommodate the administration of medication(s) commonly prescribed by physicians for the treatment of short-term illnesses. Prescription and "over-the-counter" medications will not be dispensed without written consent from the child s parent. Complete the medication authorization section of the registration form. Keep all medication in the original container with the prescription label / direction label attached. Medication must be labeled with the child s name, the name of medication, the dosage amount, and the time or times to be given. Hand all medication (including inhalers, etc.) to the staff member at the check in desk.. Campers are not allowed to keep medications on their person, in their backpacks or lunch bags. All medications will be locked up and given to your child at the prescribed time. What NOT to bring to Camp: Please do not allow your child(ren) to bring personal belongings to camp. We cannot be responsible for the loss or damage of toys, games, clothes or other personal belongings. Please do not bring the following to camp: Any electronic games / devices (this includes long field trips) Money (unless otherwise specified) Trading Cards Guns, knives or weapons of any sort - fake or real Valuable items Cell Phones- Phone usage will not be permitted during camp hours at all. If a cell phone is in use, it will be confiscated and returned to the parent at pick up. If you need to contact your child or your child needs to contact you, the camp phone will be available. What TO bring to Camp: Please ensure your child brings the appropriate items to camp for each day. Below is a list of things that we recommend: Labeled Crate (to hold all of your child s belongings while on site) Labeled Backpack (to hold all of you child s belongings on long field trips) Lunch (unless otherwise noted on weekly schedule) 2 Snacks Each Day Water Bottle SUNSCREEN- We will not be providing sunscreen and cannot be held responsible for your child getting sunburnt if you do not pack sunscreen with them. We will ensure it is put on the child as long as they have it with them.
