Welcome! Niagara is one of 45 communities across Ontario taking part in The Healthy Kids Community Challenge. What is The Challenge? To promote the health of children aged 12 years and under and accomplish the Ontario Government s Healthy Kids Strategy to reduce childhood obesity rates. From June to September 2016 the City of Port Colborne is challenging youth to PORTicipate. Every Day! Through partnership with the Port Colborne YMCA, Community Living Port Colborne -Wainfleet, local schools and various community groups, the City is offering youth their own Passport to the City. How to Use Your Passport This Passport is your ticket to PORTicipate in recreation programming, explore parks, beaches and trails, and enjoy community and city events. By participating in eligible activities and events outlined in this Passport you can earn stamps to track your activity and get a chance to win great prizes. The goal of Healthy Kids Community Challenge is to promote and enable healthy behaviours in Port Colborne families because we believe that healthy and active families lead to healthy and active kids! Rules & Regulations 1. Only completed Passports can be eligible for random draw of prizes. (minimum number of stamps and completed questionnaire). 2. Only one Passport to be submitted per person. 3. Eligible participants must be enrolled in Grades 1-6 at the start of the program. 4. Completed Passports to be returned to the following Community Services Office: (a) Roselawn Centre, 296 Fielden Avenue, Port Colborne, ON L3K 4T6 (b) Vale Health & Wellness Centre, 550 Elizabeth Street, Port Colborne, ON L3K 2C3 5. Passports to be received by a Community Services Office no later than Friday, September 2, 2016 at 4:00 p.m. 6. Winners to be announced Friday, September 9, 2016. 6
