This general price list reflects Hadley-Marcom Funeral Chapel s commitment to provide consumer information to each family we are privileged to serve. We know it is important you understand alternatives available to you as well as the specific costs for each of those services. We are confident you will find the information you need within this price list. However, should you have any questions, we welcome your inquiries. Our personal service to you includes assistance with insurance claims, Veteran s benefits, Social Security assistance and letters of verification to employers and airlines. We can assist in setting appointments for finalizing cemetery arrangements, procuring certified copies of the death certificate, obituary notification to both local and out of area newspapers, as well as other special services. Our family service staff and funeral directors provide personal service after the funeral at no additional cost to the family. We are honored by the confidence you have placed in us and are committed to service and satisfaction. PAYMENT OPTIONS Hadley-Marcom Funeral Chapel has established a uniform payment policy in order to serve all families fairly. The payment policy requires that all financial arrangements be completed one full day prior to the service or use of any merchandise. The following are acceptable payment options: 1. Pre-paid funeral plan through Hadley-Marcom Funeral Chapel or other valid, assignable insurance policy upon verification by a member of the funeral home staff. 2. Cash, personal check, cashier s check or money order made payable to Hadley-Marcom Funeral Chapel. Third party checks are not acceptable. (Personal checks will require driver s license and Social Security numbers) 3. Credit Card payment using VISA, MasterCard, Discover, or American Express. While you are free to arrange services on an itemized basis, the service plans below are designed to simplify the funeral arrangement process and are offered on a cost saving basis. Since these packages represent a substantial savings, there are no credits for deletions. These packages are available only to families who purchase all merchandise from Hadley-Marcom Funeral Chapel. These packages represent charges for services and do not include merchandise, sales tax, cash advances, Saturday/Holiday, or evening service charges unless stated in the package. There will be additional charges. PLAN A TRADITIONAL WITH EVENING SERVICE This plan includes Basic Services of Funeral Director and Staff, transfer of deceased from place of death to funeral home, embalming or refrigeration, other preparation of the body, procuring and filing the death certificate and other necessary state and local documents, use of facilities for viewing (4 Hours), use of facilities and staff for evening service, use of facilities for service or staff and equipment for offsite service, funeral coach, service vehicle/flower vehicle. Page 5 of 8 The goods and services shown below are those we can provide our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic service and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. This list does not include prices for certain items you may ask us to buy for you, such as musicians, cemetery or crematory services, flowers or newspaper notices. The prices for these items will be shown on you bill or the statement describing the funeral goods and services you selected. In some cases we may charge you for our services in buying these items or arranging these services. The survivor of the deceased who is handling the funeral arrangements, or the responsible party, is entitled to receive, prior to the drafting of any contract, a copy of any agreement that has been signed or paid for, in full or in part, by or on behalf of the deceased that is in possession of the funeral establishment. Basic Services of Funeral Director, Staff and Overhead $1,995.00 Availability of funeral director and staff to respond 24 hours a day; administrative services including preparation of death certificate, securing doctor s signature, and filing with proper authorities; securing burial or cremation authorization and/or transit permit locally; clerical administration and preparation of various forms of including Social Security and Veterans Administration; planning, supervision and follow-up on all details of arrangement conference with cemetery, crematory, clergy, or other third parties; and shelter of remains. In addition, this fee included a proportionate share of our basic overhead costs. This fee for our services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremation, immediate burial and forwarding and receiving remains.) Embalming $525.00 Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements such as a funeral with viewing. If you do not want embalming, you have the right to choose an arrangement that does not require you to pay for embalming such as direct cremation or immediate burial. (California state law requires embalming or refrigeration of deceased if held for more than 24 hours after time of death.) Forwarding of Remains to another Funeral Home $2,350.00 This charge includes the basic services of funeral director and staff, a proportionate share of overhead costs, removal from place of death to funeral home, embalming, other preparation of remains. This charge does not include the container used for shipping remains, viewing, ceremony or any merchandise or cash advance items. Receiving Remains from another Funeral Home $1,695.00 This charge includes the basic service of funeral director and staff, a proportionate share of overhead costs, temporary shelter of remains, transportation from airport (35 miles) and transportation to local cemetery or crematory. This charge does not include viewing, ceremony, merchandise or cash advance items.

Page 7 of 8 DIRECT CREMATION If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are alternative containers and other cremation containers, including containers made of wood, containers made of fiberboard and containers made of other composite materials. We offer a selection of cremation oriented caskets. Our charges for direct cremation (without ceremony) include minimum services of funeral director and administrative staff, transportation of deceased from place of death to funeral home, transportation to the crematory, crematory fee and refrigeration.
