ELEVATORS: Each of the five elevators is identified on page 4. GENERAL: 1. Cleaning The machine room equipment is maintained in fairly good condition, but some minor cleaning will need to be worked in over the first several inspections: Clean up a small amount of cable lubricant on the machines and floor that was thrown off the suspension ropes. Clean cars top of some cable lubricant which gets tracked onto the tile flooring at the landings when exiting the car tops. Clean a small amount of trash from the elevator pits. The E elevator machine room floor was just painted. I can t conclude whether this was done by the elevator or the Owner s personnel. 2. Replacement parts An adequate supply of replacement parts is provided in the elevator #1 machine room. A set of hand tools is kept on site. 3. Performance Was measured for these elevators in terms of speed and ride quality and was found to be consistent. Elevators #1 4 are running at or slightly above the contract speed of 350 FPM, measured at 350 362 FPM. The ride measured in starting, acceleration, deceleration, and stopping g forces is acceptable with one exception. 4. Signal fixtures Including position indicators, can and hall pushbuttons, car direction indicators, etc. were in satisfactory condition. All elevator telephones functioned properly on normal power. 5. Emergency Systems Emergency operation of signaling devices, lighting, and communications is maintained with battery backup power supplies. The car emergency light and alarm functioned on all five elevators but the elevator telephone emergency power failed on elevators #1 2. This would be cause to fail these elevators at the annual safety inspection by a State licensed elevator inspector.

Page 2 The annual test of elevator operation under emergency power hasn t been performed since 2010. See additional information under Periodic Tests. 6. Suspension Ropes Several elevators have a slight reduction in diameter but meet code requirements for wire breaks and minimum diameter. Counterweight runby meets all applicable requirements. 7. Permit to Operate The current Permit to Operate issued by the State of WI is required to be displayed by the Owner in each machine room or in each elevator car. The Permits found in the machine rooms were two years old, and expired in November of 2011. The current certificates must be displayed by the Owner. 8. Maintenance Control Program A written Maintenance Control Program (MCP) is a requirement of the current ASME A17.1 elevator code, which is applicable to all new and existing elevator equipment. has a standard MCP booklet for traction elevators in each machine room. A Maintenance Task Log corresponds to the MCP and denotes the maintenance tasks that has determined are required either annually or semiannually at a minimum on traction elevators. Of the 45 tasks that has determined are required on traction elevators Only 13 (Elev #1) and 14 (Elev #2) and 16 (Elev #3) of the 45 tasks are marked for 2013. Check marks are used rather than the date performed for the task log for 2010, 2011, 2012, and 2013. However the actual dates are required by the elevator code for examinations, adjustments, repairs, and replacements. 9. Maintenance Records A Maintenance Log is provided in each machine room. The current format is minimal in content for the recording the maintenance tasks compared to the previous format found in the machine room for year 2009. Maintenance records for 2010 and 2012 are missing entirely from the elevator #1 and #2 machine rooms in violation of the maintenance specification. There is no record of servicing the DC machine brake and replacement of the traction machine gearcase oil, both of which are required annually per the maintenance agreement. 10. Firefighters Operation A monthly record of Firefighters Operation testing is required, including date of test and the findings. A check mark is used rather than the date performed for most of the Fire Service records. Monthly tests were apparently missed for some elevators. For example, elevator #3 has no tests marked for January April 2011 and for April December 2010.

Callback Records It is also a requirement of the ASME A17.1 elevator code to document a description and dates of call backs (trouble calls) or reports that are reported to elevator personnel including corrective action taken. State of WI requires that these records be retained in the machine room. Few callbacks are documented for each elevator. I was told that elevator #3 in particular has experienced a high number of callbacks, which isn t documented on site in the machine room. While there were 7 callbacks within a 2 week period at the end of March 2011, only 3 were documented each in 2012 and 2013, which the on site personnel tell me isn t accurate. 12. Periodic Tests Are required at annual (Category 1) and five year (Category 5) intervals. Comments below apply to elevators #1 2 3 4. Emergency Power The Category 1 and Category 5 tests include operation on emergency power. While the Category 5 full load test is documented as being completed in 2010, the subsequent Category 1 (annual) emergency power tests weren t made in 2011 and 2012, and the WI test reports are marked N/A. 2012 The Category 1 tests were made a month late on 12/27/12 but were due by 11/28/12. 2011 Although the test reports are required to be retained in the machine room, no 2011 test report exists in the machine room for elevators 1 2 3 4. The test were conducted on 11/28/11 according to the test tags, which would have been almost 5 months past the due date of 7/1/11. 2010 The State test reports confirm the 2010 tests were made on 7/1/10. Category 1 Test Due Date The Category 1 tests are due in 2 months by 12/27/13 (elevator #1 2 3 4) and by 4/23/14 for elevator E. The test of emergency power operation must be made as part of the Category 1 tests. Category 1 Test Due Date The Category 5 tests are due next in July August 2015, and this requirements applies to traction elevators #1 2 3 4 only. My conclusion is that the records for maintenance, callbacks, Firefighters Operation, and Periodic Tests are insufficient to comply with elevator code requirements shown below, and with the elevator maintenance specification. MAINTENANCE RECORDS (ASME A17.1 2007 requirement 8.6.1.4.1) Maintenance records shall document compliance with 8.6 of the ASME A17.1 2007 Safety Code for Elevators and Escalators and shall include records on the following activities: (a) description of maintenance task performed and dates (b) description and dates of examinations, tests, adjustments, repairs, and replacements (c) description and dates of call backs (trouble calls) or reports that are reported to elevator personnel by any means, including corrective action taken (d) written record of the findings on the firefighter s service operation required by 8.6.11.1

According to the State of WI online elevator database, elevator #3 has State Reg. No. 26039 and elevator #4 is 26040, but the metal data tags on the car tops are actually reversed from what is shown above. Facing elevators #3 4, the facility has these elevators labeled #4 and #3 from left to right. So moving clockwise the elevator numbers are 1 2 4 3. The State data tags will be retained in place. If the present numbering system is to be retained then the State records should be corrected to reflect that State Reg. No. 26039 40 are elevator #4 3, respectively. The other option is to relabel the elevators #3 4 from left to right when facing the elevators. Under this option the elevator # 4 3 signs inside the elevators, at floors 2 3, in the machine room, and the voice message on the elevator telephone must be changed to #3 4. Check for signs at all landings. Advise how elevators #3 4 should be numbered. I can assist with changing of the State records.

The governor cable data tag is mismarked as a 5/8 wire rope. 11. A cover is missing for the leveling unit on the car top. 12. The hoistway door sight guards are loose at floors 4 and 7. 13. The hoistway door at 6 th floor doesn t close automatically from any position 14. A hoistway hangar dust cover for the 4 th floor is missing. 15. Both leading door panel gib runners at 1 are missing. 16. Horizontal clearance between the 1 st floor and entrance frame exceeds 3/8. 17.
