Prior to drafting any contract for goods or services, the responsible party or the decedent s survivor who is handling the funeral arrangements is entitled to receive a copy of any preneed agreement in the possession of the funeral establishment that has been signed and paid for, in full or in part, by or on behalf of the decedent. Adult General Price List These prices are effective as of June 5, 2015 and are subject to change without notice. The goods and services shown are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the goods and services you selected. This list does not include prices for certain items that you may ask us to buy for you such as: cemetery or crematory services, flowers and newspaper notices. The prices for those items will be shown on your bill or statement describing the funeral goods and service you have selected. Services of Funeral Director and Staff Basic Services of Funeral Director and Staff $ 1995 This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. The fee is already included in our charge for direct cremations, immediate burials and forwarding or receiving of remains. Personnel available 24 hours a day, 365 days a year to respond to initial call. Arrangement interview and coordination of service plans. Accounting and general clerical administration. Securing and recording the death certificate and obtaining the permit for disposition. Coordination of service plans with cemetery, crematory and/or other parties involved in the disposition. Basic coverage of overhead expenses such as facility and equipment maintenance, equipment and inventory costs, insurance and governmental compliance. Transportation Transfer of deceased to Funeral Home (30 mile radius) $ 450 Additional attendant (when needed or requested) $ 125 Funeral Coach (Hearse) $ 375 Limousine (8 passenger 4 hours) $ 450 Utility Vehicle or Flower Vehicle $ 175 Transfer to or from common carrier (Airport) $ 375 Transfer to or from crematory/cemetery $ 375 Transportation provided in excess of 60 total miles round-trip (per additional mile) $ 3

Embalming may be necessary, however, if you select certain funeral arrangements such as a funeral service with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as Direct Cremation, or Immediate Burial. Alternative containers encase the body and can be made of materials like fiberboard or composite materials (with or without any interior dressings). The alternative containers we provide are constructed of wood composite. The crematory used by our funeral home does not permit the use of alternative containers made of non-rigid materials. The only warranties, expressed or implied, granted in connection with goods sold with this funeral service are the written warranties, if any, extended by the manufacturer thereof. The seller extends no other warranties and specifically, no warranties of merchantability and fitness for a particular purpose. The survivor of the deceased who is handling the funeral arrangement, or the responsible party, is entitled to receive, prior to the drafting of any contract, a copy of any preneed agreement that has been signed and paid for, in full or in part, by or on behalf of the deceased and is in the possession of the funeral establishment.
