General Price List These prices are in effect October 1, 2014, but thereafter subject to change without notice. ITEMIZED SELECTIONS The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. PROFESSIONAL SERVICES: Basic Services of Funeral Director and Staff...$695.00 Our basic services charge includes but is not limited to 24-hour availability of funeral director and/or staff to respond to initial request for services, arrangement conference with family or responsible party, arrangement of service, tribute or memorialization. Staff to advice, assist and recommend appropriate personalization and individualization of the services to commemorate the unique life of your loved one. Obtaining and recording vital statistics and biographical information. Death certificate preparation, filing and obtaining necessary permits, authorizations, consents and assignments. Coordination with other parties such as hospital, doctor s office and staff, cemetery, crematory, clergy, and others involved in the services and disposition. Sheltering and care of decedent. Also included in this charge are overhead expenses relative to our facilities such as professional licensing, administrative services, insurance, maintenance, building and utility expenses, taxes, legal and accounting fees, recording keeping, equipment and inventory expenses. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, forwarding or receiving remains, and other packaged funeral offerings as allowed by law)

PREPARATION: Embalming... $395.00 Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. Other Preparation of the Deceased: A. Dressing and Placement of Deceased in Casket... $175 B. Sanitation and cleaning of unembalmed remains... $175 C. Additional Procedures for Organ/Bone Donors or Autopsied Remains... $225 D. Restoration of remains per hour (when necessary)...$ 95 E. Refrigeration of unembalmed remains (It is state law to require refrigeration of an unembalmed body after 24 hours)... $50 F. Dressing of unembalmed remains...$175 G. Pacemaker removal and disposal...$45 H. Hairdresser or providing Lockets of Hair...$25 & up USE OF EQUIPMENT AND STAFF: A. Visitation at your Church or Other Facility (One Day)... $195 Our services include visitation setup, display of floral arrangements, supervision and attendance during the visitation, viewing or wake. B. Funeral Service at your Church or Other Facility... $375 Our services include coordinating the funeral arrangement, supervision of funeral, Staff coordination of the funeral ceremony, setup and use of equipment for the funeral ceremony. C. Memorial Service (Remains not present) at your Church of Other Facility... $250 Our services include coordinating the memorial service, supervision of ceremony, staff coordination of the ceremony, setup for the memorial service. D. Graveside Service... $275 Our services include coordination the funeral arrangements at graveside, supervision of the graveside service, equipment and staff necessary to conduct the graveside service. E. Additional charge of staff for evening service...$250 F. Additional charge of staff on holidays...$500 G. Additional charge of staff for Weekend service...$250 H. Private viewing of unembalmed remains at our holding facility (15 min)...$275 Monday through Friday from 10:00 a.m. 3:00 p.m. $50.00 additional on weekend)

All Other Vehicles... $1.95 per mile ADDITIONAL SERVICES: Mailing of Cremains within USA by certified mail... $95 and up Local Delivery of Cremains to Cemetery/Mausoleum......$75 Delivery of Cremains to Sacramento Valley VA National Cemetery........$195 Consulate Documentation and Translation Fee....... $500 Delivery of Cremains to Sacramento Airport and booking flight..$225 Placement of obituary, per newspaper (out of town funeral home)..$25 Insurance Assignment processing fee..$125 Use of prep room for Autopsy or tissue recovery.. $500 SPECIAL CHARGES: Forwarding Remains to another Funeral Home... $895 This charge includes: transfer of remains into our care, basic services of staff, embalming, necessary authorizations, and local transportation to Sacramento International Airport. (This charge does not include any other services, casket, and air tray or airline fee) Receiving Remains from another Funeral Home... $695 This charge includes temporary shelter of remains, basic services of staff, transportation of remains from Sacramento International Airport and delivery to cemetery or crematory. This charge does not include the use of staff visitation, rites and ceremonies, or any other service or merchandise not specifically listed.

Immediate Burial....$6,990 to $595 This charge includes the transfer of remains in to our care, basic services of staff, local transportation to the cemetery, and necessary permits and authorizations. This charge does not include cemetery or government fees, the use of staff and facilities for visitation, rites and ceremonies, or any other service or merchandise not specifically listed. A. Immediate burial with casket provided by purchaser... $595 (All such containers must meet any cemetery applicable state or requirement) B. Immediate burial with minimum casket selected from our funeral home... $1090 C. Immediate burial with casket selected from our funeral home... $595 (In addition to the cost of the casket) Direct Cremation $2,685 to $465 This charge includes the transfer of the decedent into our care, basic services of staff, refrigeration, local transportation to the crematory, and necessary permits and authorizations. This charge does not include the crematory or government fees, the use of staff and facilities for visitation, rites and ceremonies, or any other service or merchandise not specifically listed. If you want to arrange a direct cremation, you can use an Alternative Container. Alternative Containers encase the body and can be made of materials like fiberboard and composition materials (with or without outside covering). The containers we provide are made of fiberboard, wood and other composite materials. See descriptions below. We also offer a selection of cremation oriented caskets. 1.
