Dr. Connie Ha received her pharmacy doctorate degree from the University of Southern California (USC) and completed a Pharmacy Practice residency at Kaiser Permanente Medical Center Santa Clara. Dr. Ha worked at Kaiser Santa Clara as an ambulatory care/drug use management pharmacist for several years before joining the pharmaceutical industry. Dr. Ha is passionate about the pharmacy profession and enjoys networking and learning from pharmacy leaders. In her free time, Dr. Ha enjoys trotting the globe, reading a good novel, and trying various foods at farmers markets. Connie Ha, Pharm.D. President Lawrence Troxell, Pharm.D. I pursued a PGY1 residency at UCSF and worked at the VASF, St. Mary s Hospital in San Francisco before working for Kaiser Santa Clara. I to listen to a variety of music, play the violin, was a competitive swimmer and still swim but don t compete. Colleen DeLizza is a clinical pharmacist at First Databank, South San Francisco in the clinical decision support area. Previous to this position, she spent many years and served many roles at El Camino Hospital, starting as an on call pharmacist, then moving on to assist in developing several clinical services, involving both inpatient and outpatient services, and then manager of operations. She continues to serve as a member on the El Camino Hospital IRB. Some of her interests include cycling, hiking, cooking, spending time with her children and friends, and as a keen enthusiast for life-long learning, attending lectures on varied subjects. My primary areas of practice are general medicine and emergency medicine. I have been actively involved in Quatra for three years since my arrival to California. I served as the communications chair (2010), secretary (2011), and CSHP delegate (2012) Melissa attended University of California San Francisco for pharmacy school and completed her pediatric residency at Lucile Packard Children's Hospital. After residency, she stayed on at Lucile Packard and has been working as a clinical pediatric pharmacist for nearly four years. Melissa is currently working with the inpatient Acute Care units and outpatient clinic providers to provide comprehensive care to patients. Shabnam Gaskari, Pharm.D. CSHP Delegate Shabnam Gaskari received her doctorate in pharmacy in 2009 from the University of California San Francisco (UCSF) and subsequently completed a Pharmacy Practice Residency at Lucile Packard Children s Hospital at Stanford (LPCH). Shabnam is currently a NICU/OB clinical pharmacist at LPCH and holds a faculty appointment at UCSF; she also serves as a Stanford IRB Panel Member. Shabnam enjoys traveling, skiing and playing piano. Betty Lee, Pharm.D. CSHP Delegate Betty graduated from UCSF School of Pharmacy and has been working as a clinical pharmacist at Lucile Packard Children s Hospital at Stanford for 15 years. She precepts pharmacy residents and students in pediatric infectious diseases and holds a faculty position as an associate clinical professor of School of Pharmacy at University of California, San Francisco. Betty has been serving QUATRA county chapter as Chapter Delegate in 2011 & 2012, and she was the Continuing Education Programs Chair in 2010. She enjoys singing, playing the piano and the cello, snowboarding and traveling. May is a Bay Area native, who grew up in Fremont, CA. She earned her Bachelors Degrees in Molecular and Cell Biology and Public Health at UC Berkeley. Continuing Education Chair Hello, my name is Sandra J. Weissmiller, Pharm.D. I am a Northern California native who has lived in the North Eastern and Southern states. I am a new graduate from the University of Tennessee Health Science Center, c/o 2011. While I was in pharmacy school, I was very active in student pharmacist organizations, and became very passionate about pharmacy practice. Drug information, pharmacy education, and clinical pharmacy are aspects of pharmacy that inspire me most. Currently, I am practicing in the community setting with the hopes of transitioning into more clinical roles. Rock climbing, pilaties, and yoga and my favorite regular fitness hobbies. Membership Chair Freda received her bachelor s of science degree from the University of Illinois Urbana-Champaign and went on to receive her doctor of pharmacy from Midwestern University Chicago College of Pharmacy. After graduation Freda moved to the Bay Area and has been practicing in an anti-coagulation clinic, in the emergency room as a medication reconciliation pharmacist and as a clinical pharmacist. She has been involved in Quatra since her move to California in 2010. In her free time Freda enjoys crafting, cooking, and exploring new destinations. Lily received doctorate in pharmacy from the Universiy of Pacific, Thomas J Long School of Pharmacy (UOP). Performed Pharmacy Practice Residency (PGY1) at VA Long Beach Medical Center, and Pharmacy Administration Residency (PGY2) at Cedars-Sinai Medical Center, and is a Board Certified Pharmacotherapy Specialist (BCPS).
