1 Christopher Betz, Pharm.D., BCPS Associate Professor Sullivan University College of Pharmacy Internal Medicine Clinical Pharmacy Specialist Norton Audubon Hospital Louisville, Kentucky Christopher Betz, Pharm.D., BCPS, is Associate Professor in the Department of Clinical and Administrative Sciences at Sullivan University College of Pharmacy (SUCOP) in Louisville, Kentucky. He also is Internal Medicine Clinical Pharmacy Specialist at Norton Audubon Hospital in Louisville, where he serves as a preceptor for pharmacy and medical students and residents in internal medicine. Dr. Betz earned his Doctor of Pharmacy degree at Temple University School of Pharmacy in Philadelphia, Pennsylvania, and he completed a pharmacy practice residency at University of Louisville Health Sciences Center in Louisville, Kentucky. He has practiced clinical pharmacy at Kosair Children s Hospital and Norton Hospital in Louisville and at Ochsner Medical Center in New Orleans, Louisiana. Dr. Betz also taught at Xavier University of Louisiana (XULA) and University of Louisiana Monroe (ULM) Colleges of Pharmacy. Dr. Betz is Chair of the Legislative Affairs Committee for the Kentucky Society of Health System Pharmacists (KSHP) and serves as a member of its Board of Directors. He is Vice-Chair and Chair-Elect of the American Society of Health-System Pharmacists (ASHP) Council on Pharmacy Practice, a Delegate representing Louisiana to the ASHP House of Delegates, and the Network Facilitator in Cardiology for the ASHP Section of Clinical Specialists and Scientists. Dr. Betz is also a reviewer for Annals of Pharmacotherapy and American Journal of Health-System Pharmacy. Dr. Betz has been an item writer for the North American Pharmacist Licensure Examination (NAPLEX) and is a member of the NAPLEX Review Committee. He has served as President of the Southeast Chapter of the Louisiana Society of Health System Pharmacists (SELSHP), Chair of the Legal and Public Affairs Committee for the Louisiana Society of Health System Pharmacists (LSHP), Chair of Recruitment Committee for LSHP, President of LSHP, and member of the Louisiana Medicaid Drug Utilization Review Board. Dr. Betz has been received multiple awards, including Clinical Preceptor of the Year from the XULA Class of 2004, Pharmacist of the Year from SELSHP, ASHP Executive Vice President s Award for Courageous Service at Ochsner Clinic Foundation during Hurricane Katrina, and ASHP President s Award for service to LSHP. He was named Teacher of the Year from the ULM class of 2010 and SUCOP class of 2012.

Bhatt, Pharm.D., BCPS, is Associate Professor of Pharmacy Practice at the Massachusetts College of Pharmacy and Health Sciences. He teaches in the areas of cardiology, anticoagulation, clinical pharmacokinetics, and adult internal medicine. Dr. Bhatt s areas of scholarly interests include inpatient anticoagulation with parenteral and oral anticoagulants, anticoagulation reversal therapies, investigational anticoagulants for venous thromboembolism, and the role of the pharmacists in venous thromboembolism treatment and prophylaxis. Dr. Bhatt earned a Doctor of Pharmacy degree at Northeastern University in Boston, Massachusetts, and completed a pharmacy practice residency at the University of Wisconsin Hospital and Clinics in Madison, Wisconsin. He also completed the American College of Clinical Pharmacy s (ACCP) Cardiology Practice and Research Network (PRN) mini sabbatical training in inpatient anticoagulation at the University of California Davis Medical Center. Dr. Bhatt is also a board certified pharmacotherapy specialist. As a member of the American Society of Health-System Pharmacists (ASHP), Dr. Bhatt has served as the network facilitator in anticoagulation for the ASHP Section of Clinical Specialists and Scientists. He is also a member of ACCP, the American Association of Colleges of Pharmacy, the American Heart Association, the American College of Cardiology, the International Society on Thrombosis and Haemostasis, and the Massachusetts Society of Health-System Pharmacists. He is also a clinical pharmacy specialist with Denver Health Medical Center in an internal medicine clinic, where he focuses on cardiovascular and metabolic pharmacotherapy. Dr. Marrs earned his Doctor of Pharmacy degree from Butler University in Indianapolis, Indiana. He completed a postgraduate year one pharmacy practice residency at Mount Carmel Medical Center in Columbus, Ohio, and a postgraduate year two family medicine specialty residency at the University of Colorado in Denver. He is a board-certified pharmacotherapy specialist with added qualifications in cardiology and a clinical lipid specialist recognized by the Accreditation Council for Clinical Lipidology. His ongoing research is in the area of optimizing pharmacotherapy for reducing cardiovascular risk and improving goal attainment rates, as well as optimizing transition of care from the inpatient to outpatient setting. Dr. Marrs is an active member of the American College of Clinical Pharmacy, American Society of Health-System Pharmacists, American Association of Colleges of Pharmacy, and National Lipid Association. Currently he serves on the ASHP Council on Therapeutics. Helen M. Marshall, Pharm.D., BCPS, BCOP Clinical Pharmacist, Hematology/Oncology Seattle Cancer Care Alliance University of Washington Medical Center Seattle, Washington Helen M. Marshall, Pharm.D., BCPS, BCOP, is Clinical Pharmacist in Hematology/Oncology at the Seattle Cancer Care Alliance and University of Washington Medical Center in Seattle, Washington, where she cares for patients with solid tumors. She also is Clinical Instructor for the University of Washington School of Pharmacy in Seattle, Washington. Dr. Marshall is a graduate of the University of Washington School of Pharmacy. She completed a postgraduate year one residency at University of North Carolina Hospitals in Chapel Hill and a postgraduate year two residency in hematology/oncology at Duke University Hospital in Durham, North Carolina. Scheetz, Pharm.D., M.S., BCPS (AQ-ID), is Assistant Professor at Midwestern University Chicago College of Pharmacy in Downers Grove, Illinois, where he serves as an instructor in pharmacokinetics. He practices clinically as Infectious Diseases Pharmacist at Northwestern Memorial Hospital in downtown Chicago and serves as director and site coordinator for two postdoctoral specialty pharmacy programs in infectious diseases pharmacotherapy. Dr. Scheetz earned a Doctorate of Pharmacy degree at Butler University in Indianapolis, Indiana, and a Master of Science in Clinical Investigation degree at Northwestern University in Chicago. He completed a pharmacy practice residency and an infectious diseases fellowship at Northwestern Memorial Hospital, and he is a boardcertified pharmacotherapy specialist with added qualifications in infectious diseases. Dr. Scheetz has authored a number of original research manuscripts and review articles in the area of anti-infective pharmacokinetics and pharmacodynamics, and he currently serves as a reviewer for several infectious diseases and pharmacy journals. His primary research interest is in the ecology of antimicrobial resistance as it pertains to the pharmacokinetic and pharmacodynamic interactions among antimicrobials, pathogens, and hosts. He is particularly interested in assessing modifiable pharmacologic variables in the treatment of infectious diseases.

5 Douglas Slain, Pharm.D., BCPS, FCCP, FASHP Associate Professor and Infectious Diseases Clinical Specialist West Virginia University School of Pharmacy Morgantown, West Virginia Douglas Slain, Pharm.D., BCPS (AQ-ID), FCCP, is Associate Professor at West Virginia University (WVU) School of Pharmacy in Morgantown, West Virginia. He also is an infectious diseases clinical specialist for the Infectious Diseases Consult Service and outpatient infectious diseases clinic at WVU Hospitals. At WVU Hospitals, Dr. Slain serves on the antibiotic committee of the pharmacy and therapeutics committee and is a key member in the institution s antibiotic stewardship program. In addition, he is the program director and principal mentor for the infectious diseases pharmacotherapy specialized residency. Dr. Slain earned his Doctor of Pharmacy degree from Duquesne University in Pittsburgh, Pennsylvania, and he completed a residency and fellowship in infectious diseases pharmacotherapy at Medical College of Virginia (Virginia Commonwealth University) in Richmond, Virginia. He is a board-certified pharmacotherapy specialist with added qualifications in infectious diseases. Dr. Slain was named Clinician of the Year by the Society of Infectious Diseases Pharmacists in Amie Taggart Blaszczyk, Pharm.D., CGP, BCPS, FASCP Assistant Professor and Division Head Geriatrics Texas Tech University HSC School of Pharmacy Advanced Practice Pharmacist VA North Texas Health Care System Dallas, Texas Amie Taggart Blaszczyk, Pharm.D., CGP, BCPS, FASCP, is Assistant Professor of Geriatric Pharmacy Practice and Division Head of Geriatrics at Texas Tech University Health Sciences Center (TTUHSC) School of Pharmacy on the Dallas/Fort Worth campus. She also directs the postgraduate year two geriatric pharmacotherapy residency accredited by the American Society of Health-System Pharmacists (ASHP) at TTUSHC. Dr. Taggart Blaszczyk earned her Doctor of Pharmacy degree from Duquesne University in Pittsburgh, Pennsylvania. She then went on to complete a pharmacy practice residency at Clement J. Zablocki VA Medical Center in Milwaukee, Wisconsin, and a specialty residency in geriatric pharmacotherapy at University of Maryland School of Pharmacy in Baltimore. She is a certified geriatric pharmacist and

6 board-certified pharmacotherapy specialist. She also completed a traineeship in Parkinson s disease sponsored by the American Society of Consultant Pharmacists (ASCP). Dr. Taggart Blaszczyk s practice and research interests include neurodegenerative diseases affecting older individuals, as well as the impact of senior education and community outreach on outcomes. Dr. Taggart Blaszczyk serves on the editorial board of The Consultant Pharmacist and is an active member of ASCP, ASHP, American College of Clinical Pharmacy (ACCP), and Dallas/Fort Worth Chapter of ACCP. She also sits on the medical advisory board of the Northeast Texas chapter of the American Parkinson Disease Association. Philip J. Trapskin, Pharm.D., BCPS Manager, Patient Care Services University of Wisconsin Hospital and Clinics Madison, Wisconsin Philip J. Trapskin, Pharm.D., BCPS, is Manager of Patient Care Services and Practice Advancement at University of Wisconsin Hospital and Clinics in Madison, Wisconsin. He also is Program Director for the postgraduate year one pharmacy residency program. Dr. Trapskin earned a Doctor of Pharmacy degree from the University of Wisconsin-Madison School of Pharmacy, and he completed a postgraduate year one pharmacy residency and postgraduate year two pharmacy practice management residency at University of Kentucky Chandler Medical Center in Lexington. He is a board-certified pharmacotherapy specialist. Dr. Trapskin was part of the multidisciplinary team from UW Health that won the ASHP Foundation s 2011 Award for Excellence in Medication- Use Safety for its anticoagulation stewardship program. He is a member of the Pharmacy Society of Wisconsin (PSW) Board of Directors, and he was named Pharmacist of the Year by PSW in 2011.

7 Linda Tyler, Pharm.D., FASHP Administrative Director, Pharmacy Services University of Utah Hospitals and Clinics Salt Lake City, Utah Linda Tyler, Pharm.D., FASHP, is Administrative Director of Pharmacy Services at University of Utah Hospitals and Clinics in Salt Lake City. She is responsible for comprehensive pharmacy services at four hospitals, three infusion services, and 14 retail pharmacies, plus she oversees ambulatory clinical services and the University of Utah Drug Information Service (DIS). She also serves as residency program director for the two-year combined Master of Science in Pharmacotherapy degree and residency program, which includes a postgraduate year one residency in pharmacy practice and postgraduate year two specialty residency in health-system pharmacy administration. In addition, Dr. Tyler serves as Associate Dean for Pharmacy Practice at the University of Utah College of Pharmacy in Salt Lake City. Dr. Tyler is a graduate of the University of Utah, where she earned her Bachelor of Science and Doctor of Pharmacy degrees. She completed a pharmacy residency at the University of Nebraska Medical Center in Omaha. Dr. Tyler served as Director of the University of Utah DIS at a time when it achieved national prominence. Her responsibilities included coordinating the services of the DIS and developing drug policy for the organization. The DIS currently provides contract services for many national organizations, including monitoring of drug shortages on a national basis and provision of content for the American Society of Health-System Pharmacists (ASHP) Drug Shortages Resource Center. Dr. Tyler s areas of interest include new drugs, literature evaluation, medication safety, drug shortages, evidence-based decision making, and drug policy development. She is co-editor of Chemotherapy Manual, which is published by Facts and Comparisons. Dr. Tyler is recognized as a Fellow of ASHP.

Worthington, Pharm.D., BCPS Associate Professor of Pharmacy Practice McWhorter School of Pharmacy Samford University Clinical Specialist General Pediatrics Children s of Alabama Birmingham, Alabama Mary A. Worthington, Pharm.D., BCPS, is Associate Professor of Pharmacy Practice at the McWhorter School of Pharmacy at Samford University in Birmingham, Alabama. She holds a joint appointment with Children s of Alabama as a pediatric clinical pharmacy specialist. Dr. Worthington is a graduate of The Ohio State University, earning both her Bachelor of Science in Pharmacy and Doctor of Pharmacy degrees there. She completed a residency in hospital pharmacy at Columbus Children s Hospital and a residency in pediatric pharmacotherapy and home health care at The University of Tennessee in Memphis. Dr. Worthington is a board-certified pharmacotherapy specialist and has been actively involved as a pediatric pharmacy practitioner for 25 years. Her current patient care activities are primarily in the area of general pediatrics.
