1 Kingsbrook Jewish Medical Center The Kingsbrook Jewish Medical Center/David Minkin Rehabilitation Center (KJMC/DMRI) is an 784 bed teaching, medical and pharmacy university affiliated, non profit, private community institution. KJMC is comprised of a 318 bed acute care hospital and The Rutland Nursing Home, a 466 bed facility offering skilled nursing, rehabilitation, and post acute care services. KJMC has a medical/surgical and cardiac intensive care unit, traumatic brain injury and coma recovery unit, geriatric psychiatry unit, ventilator unit, step down cardiac care telemetry unit, cardiac suite, memory disorders program, pain management and treatment center, wound care and hyperbaric medicine center, renal dialysis unit, vascular laboratory, and a 911 receiving emergency department. The Department of Medicine maintains an independent medical residency program of over 50 physicians and many board certified faculty. 2 Pharmacy Residency Programs at The Kingsbrook Jewish Medical Center The Kingsbrook Jewish Medical Center and the Arnold & Marie Schwartz College of Pharmacy and Health Sciences, Long Island University, offer the following one year ASHP accredited pharmacy residency programs: A state of the art USP 797 intravenous admixture service dispenses more than 1,000 doses daily. PGY 1 Pharmacy Residency PGY 2 Critical Care Pharmacy Residency PGY 2 Geriatrics Pharmacy Residency PGY 2 Internal Medicine Pharmacy Residency The Pharmacotherapy Team employs highly skilled Clinical Pharmacists, who have completed either post graduate Pharm.D. degree with a pharmacy specialty residency, a Pharm.D. degree with 2 years of pharmacy residency training, or extensive equivalent experience. Clinical Pharmacists specialize in all of the following settings: Critical Care, Emergency Medicine, Internal Medicine, Psychiatry, Geriatrics, Long Term Care, Wound Care, Infectious Diseases, and Ambulatory Care. 19

3 Pharmacy Services at The Kingsbrook Jewish Medical Center The Department of Pharmacy is fully computerized, provides 24 hour pharmacy services, and utilizes a robotic dispensing system. The Pharmacy Department employs over 40 professional and support personnel. The Pharmacy Department dispenses over 10,000 unit dose medications daily. The hospital has a closed loop drug distribution and administration system. Prescribers utilize the Computerized Physician Order Entry (CPOE) system, where medication orders simultaneously interface into the Pharmacy Computer System, pharmacists validate these orders, the Pharmacy Robot dispenses medications, and nurses administer medications using barcode scanners. Computer terminals are located throughout the hospital and on medication carts. Profiled automated dispensing cabinets are located in every acute care unit and the medication is not accessible for dispensing until a pharmacist verifies the medication order. An Emergency Medicine Satellite Pharmacy and several profiled automated dispensing cabinets supply medications and an Emergency Medicine Clinical Pharmacist provides medication management services to the Emergency Department. Dr. Cohen is the Editor in Chief of The Journal of Pharmacy Practice. He has published over 50 text book chapters and manuscripts. Dr. Cohen has published and presented over 50 poster abstracts at national meetings. He has presented over 200 invited lectures on pharmacotherapeutic topics throughout the United States. Dr. Cohen is a frequent presenter at the American Society of Health System Pharmacists and Society of Critical Care Medicine national meetings. Dr. Cohen serves as a reviewer for the journals Critical Care Medicine, Clinical Therapeutics, The American Journal of Health System Pharmacists, and The Journal of Clinical Pharmacology. Dr. Cohen is the Past President of the New York State Council of Health System Pharmacists (2012). He is a member of the national PharMerica Pharmacy & Therapeutics Committee. Dr. Cohen s areas of specialty and interest include critical care medicine, geriatrics, neuropsychiatry, adverse drug events, leadership, and pharmacy technology/medication safety. Assistant Director of Pharmacy Operations MICHAEL BIGLOW, B.S., Pharm.D., BCPS, BCPP Psychiatry Assistant Director of Pharmacy Operations Dr. Niyazov s professional background includes experience as a pediatric oncology staff pharmacist and a consultant in health economics and outcomes research. He manages dayto day operations, scheduling, and personnel management of the pharmacy. Dr. Niyazov precepts candidates and Pharmacy Residents for the Pharmacy Administration rotation at KJMC. Dr. Niyazov is the chapter president of the National Community Pharmacists Association and Rho Chi National Pharmacy Honor Society. He is a member of the Academy of Managed Care Pharmacy, American Society of Consultant Pharmacists, American Pharmacists Association, and International Society for Pharmacoeconomic and Outcomes Research.
