Cedar Community has 99-year lease of the office condo space next to the hospital IV. Therapy Services A. Cedar Home Health provides (medical) licensed and Medicare-/Medicaid-certified to provide care throughout six southeast Wisconsin counties C. Cedar Pathfinders provides professional care management provided to individuals in home or other settings VI. Hospice A. Hospice for the general community - Licensed in Waukesha, Washington, Ozaukee, Dodge, Fond du Lac and Sheboygan counties B. In-house Hospice for Cedar Lake Health and Rehabilitation Center and all Cedar Communityowned campuses. VII. On-site Pharmacy A. Full-service pharmacy accepting all insurances and delivery to Ridge and Village Homes (Cedar Lake Pharmacy) B. Durable medical equipment assists individuals with devices such as lift chairs, wheelchairs, walkers, etc. (Independence Store) VIII. Retreat and Education A. 40 overnight guest rooms; three conference/classrooms (Cedar Valley Center and Spa) B. Clubhouse located on Cedar Lake Campus with large classroom and kitchen (Learning Center/Village Clubhouse) C. One-room schoolhouse outreach program for school-age children (Ye Olde School) D.
