2 Memory Care Unit Victoria Mews offers a specially designed, intimate family-oriented unit with 16 private suites for residents with Alzheimer s and other related forms of dementia. In Connections, you will find individualized care in a secure setting, focusing on the unique needs of residents progressing through various stages of dementia. High nursing ratios per resident plus a specially trained activities staff ensure that every individual is getting excellent quality of care. There is a variety of custom-designed activities within the unit, as well as events, trips, and entertainment integrated within the larger Community. Connections is a vibrant part of Victoria Mews. The outdoor enclosed area with a spacious patio and a vegetable garden provides some of the most relaxing moments for our residents. Our tranquil surroundings also include a large koi pond, many species of birds, white tail deer and wild turkeys, all helping residents to stay connected to nature. The professionally designed living, dining and recreation space in Connections consists of an elegant dining room, state-of-the-art kitchen, spacious and bright activity area, plus a cozy living room. Colors and fabrics are picked specifically to enhance the positive atmosphere of the unit. All doors are secured and alarmed, and each suite is equipped with an emergency call bell system. Family and friends are encouraged to visit anytime. The Nursing staff closely monitors the health and safety of the residents. Complete monthly Wellness Assessments are performed to guide the development of individualized care plans. A Physician and a Nurse Practitioner are available for consultations. The services of a Registered Dietician and Registered Pharmacist are utilized routinely to ensure that all clinical goals are met most efficiently and expeditiously. On-site Physical, Occupational and Speech Therapists are available to assist with safety and mobility. Connections is a special unit in every sense. We visited many assisted living homes...but, the minute we walked into Victoria Mews we knew that this was different! The awesome staff makes a tremendous difference in Joan s life (and ours). The doctors, nurses and aides respond quickly to our questions and concerns. We very much appreciate their compassionate care and dedication...and... the food is great, too! 4 Moderate Income Program New Jersey Housing and Mortgage Finance Agency Victoria Mews has an agreement with the NJHMFA to set aside 20 of its units for residents who qualify under their Moderate Income Program. The program reduces the overall cost of assisted living for those residents who are income eligible and are accepted into the NJHMFA program. A representative at the NJHMFA will determine if a resident is qualified after reviewing the application and required documentation. Our staff will assist you with the paperwork. MIP Monthly Rate $4, per month A resident may require care in addition to the basic services. The initial nursing assessment and discussions about medication management will be conducted by our registered nurses and will help to define the services a resident will need when moving to Victoria Mews. Additional assessments will be conducted at a minimum of every six months to monitor the resident s needs. Medication Management and Levels of Care, when needed, will be covered in the MIP rate. 7 Service Level Overview The New Jersey Department of Health and Senior Services requires that as a resident s needs change, services must be able to be added or adjusted to meet the needs of the resident. Service Levels is the mechanism we use to adjust to a resident s ever-changing needs. The daily apartment rates include the following Basic Services: Resident suite, routine maintenance/repair, use of all common areas, living rooms, library, exercise room, movie theater, chapel, dining room and grounds Participation in activities programs and entertainment Three nutritious and well-balanced meals, snacks and special holiday meals Daily bed making and trash removal from suite Weekly vacuum/dusting, cleaning bathroom/kitchenette and laundry service Monthly wellness assessment conducted by a registered nurse (or more frequently as needed) Transportation for scheduled community events/outings Utilities: heat, hot water, air conditioning and electric 24 hour staffing Emergency call responsive system in each suite A resident may require care in addition to the basic service listed above. The initial wellness assessment and discussions about medication management will be conducted by our registered nurse, and will help to define the services a resident will need when moving to Victoria Mews. Additional assessments will be conducted a minimum of every six months to monitor the resident s needs and to determine if service level adjustments are needed. A written copy of each new assessment and service level will be sent to the resident and/or designated family member. Medication Management New Jersey state regulations require that medications administered by staff members in an assisted living community must be packaged in blister pack type cards. Our provider is Discover Rx LTC Pharmacy. Discover Rx also profiles and monitors medications and their possible interactions. The Victoria Mews nursing staff administers routine medications as well as PRN medications between the hours of 7:00 am and 11:00 pm. We also provide monitoring of the parameters as needed, i.e.: apical rate for digoxin, blood pressure for certain antihypertensives and blood sugar monitoring for diabetics as ordered. The nursing staff communicates with the primary physician regarding lab tests and results, dosage changes, the recording of medication, and they communicate with the family and physician about the effectiveness of medications.
