4 In your neighborhood The Plaza Assisted Living began with one man s love for his aging parent. Faced with limited options, it became his mission to build a quality community for seniors in Hawaii as well as their families who are experiencing the same situation. Specialized programs and services to suit the individual needs of our residents: 4 Independent Living 4 Assisted Living 4 Memory Care 4 Respite (short-term stay) INDEPENDENT LIVING The Plaza s Independent Living program allows residents to continue their daily routines. ASSISTED LIVING The Plaza s Assisted Living program has been designed to foster and promote each resident s independence. A caring staff is available to provide assistance with activities of daily living, such as medication management, bathing, incontinence management and grooming, to name a few. MEMORY CARE Caring for a family member who suffers from Alzheimer s disease or dementia can be challenging. The Plaza Assisted Living offers a Memory Care Program in a comfortable and supportive environment, specifically designed to care for residents afflicted by memory loss, including Alzheimer s diseases and other related dementias. The designated Memory Care floor houses a state-of-the-art secured alarm system that enables residents to stroll freely within the Memory Care floor. For those residing on the Memory Care floor, a specialized recreation program has been developed to offer residents a daily routine. 6 LEVELS OF CARE

7 Quality Life. RESPITE (SHORT-TERM STAY) The Plaza s Respite (short-term stay) program provides the same services that we offer to our permanent residents. We provide independent and assisted living, as well as memory care services. Residents who participate in The Plaza s Respite Stay Program are offered a partially furnished apartment. Who could benefit from the Respite Program? 4 Individuals needing assistance prior to returning home after a hospitalization, surgery or illness 4 Family or at-home caregiver is going on vacation or in need of some relief time 4 Living arrangement is being renovated 4 A trial stay prior to a permanent move to The Plaza Personal Care Services Personalized care plans are tailored to individual s needs, based on an assessment by The Plaza s nursing staff. Services may include: 4 Bathing 4 Dressing 4 Redirection & Reminding 4 Grooming 4 Medication Management 4 Hygiene 4 Personal Laundry Service 4 Incontinence Care LEVELS OF CARE 7

All menus are nutritionally balanced and are regularly reviewed by a Registered Dietician. 9 ACTIVITIES The Plaza Assisted Living provides its residents with a variety of recreational options. Whether they are in the mood for engaging in the exercise program, participating in arts and crafts, or are in pursuit of a slower pace, The Plaza s activity programs are offered daily. Residents can participate in a variety of activities* such as Tai Chi, chair Yoga, Wii games, pet therapy, tea social, dining out, Zumba and much more. The activity program is continually evolving according to the interests and needs of our current residents. 10 PERSONAL CARE SERVICES AND WELLNESS The Plaza s Assisted Living program has been designed to foster and promote each resident s independence. Residents will be able to continue their lifestyle while also having access to care. A nurturing staff will provide assistance with activities of daily living. Each apartment is equipped with an emergency call-system and access to Plaza staff is available 24 hours a day.
