2 Northfield Villa & Vista We Offer You Life Care... Life Care can protect you and your hard -earned assets from the ever-increasing cost of Long Term Care. You can live independently, but have the assurance of knowing Assisted Living services are on the premises should you need them. When you need nursing care, our new Care Center is ready to serve you at The Residency. The one-time entrance fee is similar to paying a one-time Long Term Care insurance policy premium, with no elimination period. Your only financial obligations are the entrance and monthly fees. When using the care center or assisted living the only additional costs are for meals and supplies. Also a portion of your entrance and monthly fees are considered medical expenses for your taxes. Independent Living Located on the west of side of Gering, NE the Villa/Vista has a great view of Scotts Bluff National Monument. There are a total of 100 Independent apartments, 55 in the Villa building and 45 in the Vista building. Both buildings have undergone recent remodeling. Apartments range in size from 470 to 1450 square feet, and have a variety of floor plans. The apartment is yours to decorate, flooring is furnished, and kitchen appliances are provided. Utilities, cable TV, housekeeping, one meal a day, maintenance, grounds care, scheduled transportation, Emergency Call Alert and much more are included in your monthly service fee. Guest rooms are available at an additional fee.. You are encouraged to live an active lifestyle, and the only thing that needs to change is your address. Many social and recreational activities are offered including gardening, exercise, music, Bible studies, Vespers, cards, and games. There are also a variety of community activities with area churches, theatres, civic organizations and museums, including North Platte Valley Museum and the Farm & Ranch Museum. There are always opportunities for volunteerism; a number of residents volunteer at the museums and Regional West Medical Center. 4 Northfield Retirement Communities QUESTIONS OUR NEW RESIDENTS MOST FREQUENTLY ASK How often is my apartment cleaned? Every other week. Do I bring my own sheets and towels? Please bring your own sheets for at least four bed changes. NRC provides clean bath and kitchen towel service, as well as biweekly laundry services for your bedding. May I have my bedding changed for me? Only if requested, and for a small fee. Where do I put my dirty towels and my trash? Separate dirty towel and trash containers may be found in each of the resident laundry rooms. How is meal service handled? Noon meals are served in the Villa and The Residency dining rooms. You may eat your evening meals in the Villa, the Vista or The Residency dining room. The same menu is served at both the noon and evening meals but with multiple choices. May guests dine with me? Yes. We encourage you to invite guests whenever possible. Please notify the Concierge/ Front Office in advance when you intend to entertain guests. If you plan to entertain more than two guests for a meal, please notify the office at least 24 hours in advance. Charges for guest meals will be added to your monthly statement. May I get assistance to hang pictures, curtains, or move things in my apartment? Our Maintenance staff is available to assist you with many of these types of services. The cost is $20 per hour with a minimum of 1/2 hour. What kinds of social activities are available? We have weekly activity nights (cards, bingo, movies etc.). There are various small card groups as well. Many other activities are scheduled throughout the year. Please watch the bulletin boards, resident newsletters, and note the daily announcements made at the dinner hour for up-to-date information on current resident activities. 5 Is transportation provided to different locations in town? Yes. Transportation is provided to all medical offices in Scotts Bluff County free of charge. A shuttle schedule is provided for residents to enjoy the most popular stops in the Scottsbluff/ Gering area. Periodically, transportation is provided to various special activities in the community. Please watch the bulletin boards for times and locations. How and where do I pay my monthly bill? You may pay your monthly bill at the business office. If you desire, electronic funds transfer may be arranged with your bank to make direct payment of your bill. How often will my monthly fee change? Our fiscal year is from May 1 to April 30. This is typically when a change may happen. However, the Board of Directors of Northfield Retirement Communities reserves the right to change or increase the service fee if economic conditions make it necessary to do so. Written notification of a change in rates will be made at least 30 days prior to the effective date of the change. Are there extra storage areas and/or garages available? Storage space is located in the basements of the Villa and The Residency. At the Vista, storage compartments are located in the laundry rooms. Garages may not be immediately available. Please contact the office for more details about the garage waiting list. When do I get my keys? When you sign your Occupancy Agreement. What if I need nursing care? If your health needs change and nursing care is requested, you may be assured to know that our Health Care Center is close by at our Scottsbluff campus. After a period of restorative or recuperative care, many residents return to their apartments. Assisted Living is available at our Gering campus. What if I have a medical emergency? You may use the Care Alert pendant that you are issued to notify staff of your emergency. How are medications handled? Our nurses are available to create an individualized plan of service for you. What other health and wellness services are available? We have monthly health care days and an annual mini health review. Please watch the bulletin boards, monthly newsletters, and note the daily announcements for information regarding other health, fitness, and wellness programs provided. Where may I get my hair done? Right here. Please feel free to call or visit either the Villa or The Residency beauty shops for a shampoo, haircut, or styling. You may ask the stylists about their charges. What Utilities are Included? As part of your monthly fee or rent, local telephone, expanded basic cable, high-speed internet, heating/cooling, repairs, etc are included. All your utilities are included and you will never have a repair bill!

Lang, CPA February 11, 2014 Gretchen Traw Chief Financial Officer Northfield Retirement Communities 2100 Circle Drive Scottsbluff, NE Dear Gretchen: As requested, we have reviewed your calculation of the medical expense percentage of monthly fees paid to Northfield Retirement Communities relating to a tax-deductible medical expense for calendar year Based on our analysis, your calculation of the medical expense portion of fees paid of 33.06% percent appears reasonable and can be used by the residents for As you know, the facts and rulings presented in Revenue Rulings , and , and related Letter Ruling allow the portion of residents payments attributable to providing medical care as deductible to them personally on schedule A of their tax returns as an itemized deduction. Therefore, the estimated percentage you have calculated for 2013 of 33.06% is a proper amount attributable to the cost of providing medical care to residents. This percentage would apply to payments actually paid in 2013, whether for an entrance endowment or for monthly fees. Legislation relating to long-term care insurance sets forth a guideline that caring for individuals with two or more ADL (activities of daily living) limitations qualifies as a medical expense. Therefore, to the extent that residents in assisted living exceed that level of disability, monthly fees for assisted living would be a qualified medical expense.
