Back then, our healthcare cost trend was on an unsustainable course. BP s healthcare costs were rising 10.5% per year. By comparison, our competitors healthcare costs were going up just 6% per year, and the overall rise in the U.S. was about 7%. On top of that, BP employees rate of receiving key wellness screenings (for example, mammograms and prostate exams) was lower than the national average, meaning that our people were missing important opportunities to catch, treat or prevent curable illnesses. better today. healthier today. because I run. Rick Dorazil Today, we ve made tremendous progress. The number of employees receiving wellness screenings has skyrocketed. And over 37,000 employees and pre-65 retirees and their covered spouses/domestic partners participated in the BP wellness programs in 2011 an indication that our population is more engaged in their health and healthcare. Because of your efforts, we re becoming a healthier BP, and we re starting to see our wellness efforts pay off! BP families are more engaged in their health than ever before. All of these efforts tie directly to the great improvement in claims experience that we ve seen, which means lower increases in our health care premiums. BP benefits because healthy employees are safer and more productive. Supporting better health is not only a smart business decision, but also the right thing to do. That s why the company offers the BP wellness program. 3

5 Who s eligible? The BP wellness program is available to employees and non-medicare-eligible retirees as well as their spouses/domestic partners.* You do not need to be enrolled in BP s HealthPlus Plan to participate in the BP wellness program. Why participate: better health and HealthPlus The BP wellness program is designed to help you improve, manage and maintain your health. And, when you participate, you are rewarded by becoming eligible for the HealthPlus Option in the BP Medical Plan. HealthPlus offers the following advantages over the Standard Option: Lower plan-year deductible and out-of-pocket maximum Lower prescription drug copays and no separate deductible Copays instead of coinsurance for doctor visits Check out our new programs! For 2012, we ve added several new programs that you won t want to miss, including: Biometric Screenings 250 points See page 9 for details. Pilot Program for Weight Management 500 points (Note: This program is for participants with moderate to high health risks. Since this is a pilot program, it will be limited to the first 1,000 participants.) See page 10 for details. Financial Health 125 points per seminar, up to 250 points. See page 10 for details. Note: If you and your spouse/domestic partner were required to earn 1,000 points during the 2011/2012 plan year but did not do so, only the Standard Option will be available to you for the 2012/2013 plan year. For a full comparison of the HealthPlus and Standard PPO Options, please visit bp.com/lifebenefits. However, the HealthPlus Option and the Standard Option have the same premiums, and all of the options in the BP Medical Plan including our HMOs cover preventive care at 100%. So rest assured that you ll have comprehensive coverage, whichever option you choose. If you want to enroll in the HealthPlus Option for the 2012/2013 plan year, both you and your covered spouse/domestic partner (if applicable) must complete the Health Questionnaire between February 6 and March 2, 2012 (by 11:59 p.m. Central time).** Additionally, if you participated in HealthPlus in the 2011/2012 plan year, both you and your covered spouse/domestic partner (if applicable) must each have earned 1,000 points in the BP wellness program by December 31, You and your covered spouse/domestic partner must each earn 1,000 points during the 2012/2013 plan year to continue to be eligible for the HealthPlus PPO/OOA options in 2013/2014. *AirBP Puerto Rico employees are not eligible to participate in the BP wellness programs in 2012/2013. **Employees hired on or after January 1, 2012, as well as employees on international assignment, are encouraged but not required to take the Health Questionnaire and earn 1,000 points in

8 BP wellness program pointers Want some pointers for meeting your personal health and points goals in 2012? Just follow the steps below and you ll see results in no time! Step 1 Take the Health Questionnaire between February 6 and March 2, 2012 The Health Questionnaire is your gateway to better health. Answer the questions in this 20 to 45-minute survey and you ll receive a personalized report that gives you a snapshot of your health and helps you identify risk factors and areas you should focus on to improve or maintain your health. Individual responses to the Health Questionnaire are completely confidential. Results are compiled by StayWell our third-party partner in wellness BP will see only aggregated data. Access the Health Questionnaire online (preferred) Beginning February 6, 2012, you can access the Health Questionnaire online 24 hours a day, 7 days a week, from any computer with access to the internet. Everyone is invited to complete the Health Questionnaire for However, you and your spouse/domestic partner (if applicable) are required to complete the Health Questionnaire if you want to enroll in the HealthPlus Option in the BP Medical Plan for the 2012/2013 plan year. Online preferred method (February 6 through 11:59 p.m. on March 2, 2012): Both employees and spouses/domestic partners should go to bp.com/lifebenefits and click the employee or spouse button, as applicable. From there, you will be taken to StayWell Online via a secure link through the BP Benefits Center. Note: Spouses/domestic partners can now access StayWell via a secure link through BP Benefits Center online. The first time spouses/domestic partners access the BP Benefits Center online, they will be required to re-register and create a user ID and password. Request a printed copy. If you don t have access to a computer, you can request a printed copy by calling the StayWell HelpLine at Representatives are available Monday through Thursday from 8 a.m. 8 p.m. Central time, Friday from 8 a.m. 6 p.m. Central time, and Saturday from 8 a.m. noon Central time. Note: To be eligible for HealthPlus for the 2012/2013 plan year, your completed paper Health Questionnaire must be postmarked by March 2, Complete the Health Questionnaire each year to keep your health information up to date. You can even update your answers anytime throughout the year if your health changes. Pointer 1 Know your numbers Completing the Health Questionnaire is easy it takes about 20 to 45 minutes and doesn t require an extensive health history. Knowing the following numbers, which you can get from an annual physical exam, will help strengthen your results, but you can still complete the Health Questionnaire even if you do not have all the information: Measurements: height, weight (BMI) Blood pressure Cholesterol: HDL, LDL, Triglycerides 7

9 Step 2 Step 3 Call a Health Advisor and create a personalized health action plan Participate in the wellness programs and earn 1,000 points by December 31, 2012 Once you ve reviewed your Health Questionnaire personalized report, call a StayWell Health Advisor to review your results and answer any questions you may have. Plus, the Health Advisor can suggest which wellness programs might fit your interests and help you meet your personal health goals. Call StayWell at Health Advisors are available Monday through Thursday from 8 a.m. 8 p.m. Central time, Friday from 8 a.m. 6 p.m. Central time, and Saturday from 8 a.m. noon Central time. Beginning January 1, 2012, you ll earn points for each wellness program you complete. Points range from 125 to 500 points. Points do not roll over from year to year. There really is something for everyone. And you can participate online, in-person, by phone or by mail whatever works best for you. A complete list of programs and the registration directions start on page 11. Pointer 2 Pointer 3 Earn 250 points in 10 minutes It takes just 5 to 10 minutes to call a Health Advisor and review your Health Questionnaire results. If you call a Health Advisor by June 30, 2012, you ll earn 250 points. Go to My Points to track your progress At any time throughout the year, you can track your points online through My Points on the StayWell Online website. My Points shows your point totals and lists the programs available to you so you can meet your goals. Log on to StayWell Online via bp.com/lifebenefits. 13 Asthma BP s wellness programs on asthma just might help you breathe easier. Program Name Points What to expect Telephonic Condition Management 250 This is the BP Condition Management program. In this confidential program, participants work with a StayWell coach to get information about managing certain chronic conditions, including asthma. Points may be earned by those participants eligible for the StayWell condition management benefit. While this activity may involve multiple telephone calls, points are given at the completion of the third call with a StayWell coach and are awarded in the year that the third call is completed. Contact StayWell at to find out if you re eligible for the Telephonic Condition Management program. Online Health & Family Centers: Asthma Informational While you don t earn any points, you have access to these online modules to help you learn more about asthma and what you can do to manage your asthma and get more out of life. In addition to our adult programs, StayWell offers a pediatric asthma program for your dependents under 18. The pediatric asthma program is similar to the adult programs in that StayWell will contact you based on medical claims they have received that identify your dependent as having asthma. You, as the adult guardian of the child under 18, would receive a call from an enrollment specialist regarding your dependent and his or her asthma. If you choose to enroll on behalf of your dependent, a coach will work with you the guardian and your family to manage your dependent s overall health, especially in regard to the challenges of asthma. 12

14 Back Care (Lifestyle Management) Back aches are a real pain. Take care of your back and participate in BP s back care programs. Program Name Points What to expect Telephonic Lifestyle Management: Back Care 250 If you want one-on-one attention, this program provides a personal Health Coach to help you with back care. These professional Health Coaches will call you at a place and time convenient for you. They will help you set goals, deal effectively with barriers, evaluate helpful resources and keep you on track for success! You must start this Telephonic Lifestyle Management program no later than September in order to have time to complete it by December 31 and earn the 250 points. Back Care Wellness Class (Formerly Every Move You Make ) 125 (up to 500 points) Mail-based Lifestyle Management: Back Care 125 This class focuses on back flexibility, strength and moving safely at work and home. Participants will learn flexibility and strengthening exercises while becoming more aware of each move their body makes. If you prefer to work independently, you may benefit from this mail-based program. This six-month program provides you with a series of useful self-help educational materials, tools and activities, and additional resources that help you meet your personal health goals and become a healthier you. Online Health & Family Centers: Back Care Informational You must start this Mail-based Lifestyle Management program no later than May in order to have time to complete it by December 31 and earn the 125 points. A healthy back supports your body. While you don t earn any points, you have access to these online modules to help you learn methods to keep your back healthy and strong. 13

15 Blood Pressure (Lifestyle Management) If your Health Questionnaire identified your blood pressure as a potential health risk, consider BP s blood pressure program. The good news is that by making simple lifestyle changes, individuals can significantly reduce their risk. The Healthy Heart Program is designed to help members take small steps today that can lead to long-term benefits. 16 Cancer and Complex Case Management Make the most of your health and your healthcare through BP s complex cancer and complex case management programs. Program Name Points What to expect Complex Case Management 250 Complex Case Management is for a small percentage of the BP population who have an acute condition that requires extensive treatment, such as a severe stroke, transplant, severe injury or cancer. Participants in the Complex Case Management program receive support in navigating the healthcare system and optimizing their BP healthcare benefits. Points are given once a care plan has been created and at least one call with a Case Manager is completed. Points are awarded in the year that the call with a Case Manager is completed. Online Healthy Living Program: Cancer Fighting 125 (no limit) Typically employee/retirees or spouses with select severe conditions are identified by the Case Manager for participation. See back page for contact information and call to find out if you re eligible for the Complex Case Management program. With studies linking diet to cancer, it s becoming more evident that individuals can reduce their risk for disease by making simple changes in their lifestyle. The Cancer-Fighting Program helps you eat healthier and increase activity two of the best weapons against disease. 19 Financial Health Program Name Points What to expect Financial Health 125 (up to 250 points) You will have the opportunity to earn wellness points by participating in both web-based and on-site classes on a broad range of personal financial topics. These include retirement planning, debt management, estate planning, real estate, planning for higher education and more. Your financial health is an integral part of your total wellness and so financial education should be part of a comprehensive wellness program. General Health Put the results from your Health Questionnaire to work. The following programs can help you take steps today to ensure better health tomorrow. Program Name Points What to expect Annual Physical Biometric Screening 500* OR 250* We recommend that you complete a biometric screening before having your annual physical so you can go over your screening results with your doctor. However, if you do not have a biometric screening, your annual routine physical** must be completed with a licensed physician and should include a blood test to measure your cholesterol (including HDL, LDL and Triglycerides), a check of your blood pressure and updated BMI. Not all screenings are considered routine and may not be covered by the BP Medical plan. Go to the wellness tab on the LifeBenefits website at for a list of the types of screenings and lab tests that are typically covered. New Reporting Option: You can now use the Annual Physical Self Report Option within your My Points Account to immediately receive points credit. BP may audit self-reported responses. Note for women: While your annual well-woman exam qualifies for the 500 points, we highly encourage you to also complete an annual physical exam with your primary care physician. Note for Group Leaders: Participating in the executive physical program will allow you to qualify for the 500 points. * You can only earn points for either the annual physical OR the biometric screening. If you complete both, you will receive 500 points. ** Annual routine physicals performed in network will be covered at 100% with no copay or deductible. However, your doctor may order additional lab tests or procedures that are not considered routine. If they are not considered medically necessary, you will be responsible for the full cost so it is important to always check with your insurance provider before agreeing to any additional testing. 18

20 Program Name Points What to expect Health Advisor Call 250 (If completed by June 30, 2012) Once you complete your Health Questionnaire, call a StayWell Health Advisor. During the call you can learn more about the results of your questionnaire and find out which wellness activities can help you meet your individual health goals. Call a StayWell Health Advisor to: Review the results of your Health Questionnaire Learn more about BP wellness activities Receive answers to your health and wellness questions The StayWell Health Advisor call is completely confidential and should last about 5 to 10 minutes.
