In building a worksite wellness program with Tufts Health Plan Momentum+ you can energize your workforce and reward employee engagement in wellness activities. Over time, small steps toward lifelong healthy behaviors could improve productivity, decrease absenteeism, and strengthen your bottom line. Our best-practice wellness solutions are easy to implement. Our goal is to meet the needs of companies with 51 or more subscribers. All employees and their dependents 18 years of age and older who are enrolled in Tufts Health Plan are eligible to participate in Momentum+.

Our activity-based programs motivate employees to earn credits at their own pace by completing wellness activities they choose you can engage your workforce and encourage success by providing incentive rewards such as group premium reductions, HRA/HSA funding, extra vacation time, and gift cards. See how on the next page Whether participants want to stay healthy, get healthier, or boost their well-being, Momentum+ offers more choice for better results: } More than 50 health and wellness topics } Choice of program activity participation } New interactive tools } Monthly start dates Legal considerations when delivering wellness programs include compliance with: Health Insurance Portability and Accountability Act (HIPAA), Americans with Disabilities Act (ADA), Patient Protection and Affordable Care Act (ACA), Genetic Information Non-Discrimination Act (GINA), Equal Employment Opportunity Commission (EEOC rules and regulations). General guidelines for wellness programs: Participation in wellness programs must be voluntary, Medical information obtained must be kept confidential, Should not be used for making employment decisions, Must be offered to all similarly situated individuals, Reasonable alternative standards must be available for certain program. It s always a good idea to consult with your legal counsel when designing wellness programs.

5 Optional Programs for Qualified Employers Momentum+ Partnership Premium Reduction Option For qualified fully insured employers Momentum+ Partnership option provides support for an effective worksite wellness program for fully insured employers who: } Exclusively offer Tufts Health Plan coverage with 100 or more enrolled subscribers } Offer a Your Choice plan or an Advantage plan with a minimum $1,000 deductible and at least one-third of subscribers enrolled (if it is offered with other Tufts Health Plan plans) Premium Reduction Employee Participation PHA Biometrics 25 Credits 1% 35% % 1.5% 35% % 2% 70% 2.5% 70% How to Earn Up to 2.5% Premium Reduction } Incent employees to complete the online PHA (10 credits), enter their 5 biometric values/ measures in the PHA (5 credits), and complete various health and wellness activities which result in receiving 10 additional credits for a total of 25 credits. } Employees are able to complete a variety of health and wellness activities for credits including online seminars, monthly challenges, online coaching modules, condition management, and worksite activities. Momentum+ Outcome-based Option For qualified self-funded employers Momentum+ Outcome-based option is available to self-funded employers ready to advance employee engagement and promote personal responsibility by providing incentive rewards for achieving, maintaining, or improving at least one of the following health outcomes: } Meeting acceptable health measurements (glucose, height/weight for BMI, total cholesterol, LDL, blood pressure, and tobacco-free status) } Demonstrating health measurement improvement between the first and second years of the program } Adhering to therapeutic recommendations for certain conditions Requirements to Qualify } Self-funded employer offering employees Tufts Health Plan s Condition Management and Health Coaching programs } Necessitates a two-year commitment including annual PHA campaigns and biometric data VIRTUAL COACHING } Feel less stress } Reach a healthy weight } Improve your diet } Be more active } Live tobacco free HEALTH CONVERSATIONS } Asthma Control Medication } Low Back Pain } Heart Attack Prevention } Sleeping Well } Healthy People Get Active MONTHLY SEMINARS } Less Stress for Better Living } High Blood Pressure Silent Killer } Going Organic } Fat or Fit } Understanding Arthritis Employees can find a full list of topics in the wellness section at mytuftshealthplan.com.
