1 WKU Wellness Portal Frequently Asked Questions Wellness Portal/Zone What is the wellness portal? What is the wellness zone? The wellness web portal is your homepage and link to the WKU Employee Wellness program. Use the wellness portal to take your health assessment, find out about coaching programs, participate in health challenges, connect with others and take advantage of other wellness resources. The wellness zone is the area on the portal page where you can add and view widgets to help you log and keep track of your activities. (Note: not every portal page has a wellness zone.) Wellness Bank What is Your Wellness Bank? Your Wellness Bank is a tool to track activity completion and points earned toward the wellness incentive. This page also lists all the eligible activities and programs that qualify for incentive points. A green check mark in the box next to the activity indicates that activity has been successfully completed. How do I get started? Step 1 - Earn 60 points by completing the below two gateway activities by March 31*. Both gateway activities must be completed to be eligible for the incentive. 1. Online health assessment (HA) - 30 points 2. Biometric health screening - 30 points *Note: New hires must complete the two gateway activities within 90 days from their benefits start date or by November 15, whichever occurs first. Step 2 - Earn 40 points by completing one or more of the following activities by November Lifestyle management coaching (by phone or online) 40 points 2. Condition management coaching (by phone only) 40 points 3. Health challenge (spring, summer or fall) 20 points each 4. Aerobic activity (self-reported) 40 points 5. Weight Watchers (self-reported) 40 points 6. Cooper-Clayton smoking cessation program 40 points 7. Employee Wellness sponsored colloquium / seminar 20 points each
2 How long does it take for my points to show up? Points for the health assessment (HA) and self-reported activities show up immediately after you complete/report the activity. For other activities, points will typically show up in the month following completion of the activity. If you do not see a green check mark next to an activity you ve completed after one month, please contact customer service at How and when will I receive my incentive? Go to Your Wellness Bank to select your incentive option. You will receive the full reward in the second payroll cycle in month following the quarter in which you earned 100 total points and made your incentive selection. See table below for full rewards and payout schedule. Jan 2015 Yes. All fulltime, benefits eligible employees are eligible for the incentive. Depending on your benefit elections and employment type some incentive options may not be applicable. What if I am a new employee? Am I still eligible for the incentive? Yes. If you are a fulltime, benefits eligible employee, you can still earn the incentive. Complete the online health assessment and a biometric health screening within 90 days of your start date. You may have your doctor fill out and submit the physician form if you already completed your annual preventive exam after November 1, All activities must be completed by November 15, 2014 in order to qualify for the incentive.
3 Will I receive the incentive if my employment at WKU terminates? Employees who terminate employment before the scheduled incentive payout date will forfeit the incentive. Active employees only will receive the incentive. Will I be penalized if I do not participate in this program? No, there is no penalty for choosing not to participate in the program. Participation is voluntary. Health Assessment/HA What is the Health Assessment (HA)? This is an online questionnaire that can be filled out to provide you with information on your overall health status. The health assessment is an interactive resource to help you make appropriate health and wellness choices. It is not a biometric health screening service nor is it designed to provide specific medical advice, diagnosis or treatment in any individual case. The health assessment takes about 10 minutes to complete. Biometric Health Screening What is the biometric health screening? A biometric health screening is a measurement of key clinical health markers. It is not meant to provide a diagnosis, but it allows for objective measurement. Coupled with the health assessment, it provides a more comprehensive picture of your health status. When should I complete my biometric health screening? Biometric health screenings must be completed prior to March 31 or within 90 days from your benefits start date if a new hire. What if I have already seen my physician for an annual preventive exam? You can receive credit for the biometric health screening if you have seen your physician for an annual preventive exam and completed lab work on or after November 1, Download the physician form and follow the instructions on the form. Your physician must complete and fax the form within the specified timeframes. What measurements will be taken for the biometric health screening? The biometric health screening includes a blood draw or lab work to measure your glucose and blood lipids. It will also involve a measurement of your vitals, such as height, weight and blood pressure.
4 If medically possible, please refrain from food or drink, other than black coffee or water, 9 to 12 hours prior to your biometric health screening appointment for better accuracy in glucose and cholesterol readings. Where can I complete my biometric health screening? You may complete your biometric health screening with WKU Health Services or through you primary care doctor s office. WKU Health Services: An online scheduler is available before and during targeted biometric screening periods (e.g., during J-term and spring break) to schedule an appointment. Call WKUHS at (select option #1 for the appointment line) to schedule an appointment outside the targeted screening periods. Primary Care Doctor s Office: Download and print the Primary Care Physician Form and schedule an appointment with your in-network primary care provider. Note: You must complete the entire biometric screening process at WKUHS or through your primary care doctor s office. Results from different offices cannot be combined to make complete. What if I do not complete the health assessment or the biometric health screening? Can I still earn 100 points and receive my incentive? You may earn the points, but you will not receive the incentive. The online health assessment and the biometric health screening are required activities for the incentive. You may complete as many additional activities as you would like, but you will not be eligible for the incentive if you do not complete the health assessment and biometric health screening. How will I receive the results from my biometric health screening? 1. From the Employee Wellness program. You will receive an indicating Your Individual Health Assessment Report is Ready once the biometric screening data has been married with your health assessment. Depending on how soon after the appointment the provider submits the data to LiveHealthier, it may take two to three weeks for you to receive this system generated From WKU Health Services Patient Portal If you have an active WKU Health Services Patient Portal account your lab results should be posted to it five business days after your screening visit. You will receive an from WKUHS notifying
5 you that the results have been posted. Go to to check your account status. Go to wku.edu/healthservices to set up a new account. Health Coaching What types of coaching programs do you offer? WKU Employee Wellness offers online and phone lifestyle management coaching as well as phone condition management coaching. Lifestyle management coaching topics include physical activity, nutrition, weight management, stress management, tobacco cessation, general health, high blood pressure, and high cholesterol. Condition management coaching topics include coronary artery disease (CAD), congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), high-risk asthma, diabetes, depression, low back pain, high-risk pregnancy maternity management and healthy pregnancy. Health Challenges What are health challenges? Health challenges are fun activities designed to encourage individuals to engage in healthy behaviors. Be on the lookout for more information on health challenges during each academic term. Widgets What is a widget or online tracker? A widget is a quick and easy tool used to track your physical activity and nutrition and diet information as well as your glucose and hemoglobin levels. It takes less time to log items on the widget than through the Physical Activity or Diet and Nutrition logs. Though the widgets do not show as much information as that which is provided in the logs, all information entered in the widget is entered in the appropriate log and you can view all of your information by looking in the log. Microsoft Silverlight must be installed on your computer in order for you to view the widgets. You may download it for free here: Also, an internet security setting of medium is recommended when using Silverlight and widgets. To raise or lower your internet security level, go to Tools > Internet Options on your browser. If the security level is too high, Silverlight won t work properly. If you are using a shared computer, ask the computer administrator for help installing Silverlight. How do I add a widget to my wellness zone? Go to the Your Health Trackers page and click on "Add Widgets to Health Dock." Select a new widget from the list by clicking on the "Add Widget to Health Dock" button on the right. Your
6 new widget will appear on the health dock on your wellness toolkit page. Then choose the widget that you want to add to your wellness zone. Click on the downward arrow at the top of the widget and select "Assign to Wellness Zone." How many widgets can I have in my wellness zone? You can add up to two widgets on your wellness zone. Widgets My Nutrition Tracker How do I use the Nutrition Tracker? To log your diet using the Nutrition Tracker widget, select the date you wish to log the food, beverage or meal (note: the widget defaults to today's date). Next, type in the item that you wish to log. A list of matches will appear and you can pick the one that most closely matches yours. Then enter the number of servings, the serving unit and the meal type. Hit the "Submit" button to complete. How do I view what I documented on the Nutrition Tracker? Information entered on the Nutrition Tracker widget can be viewed by going to the Health Tools tab and then selecting Diet and Nutrition Log. How do I remove or update an item on the Nutrition Tracker? To remove or update a food, beverage or meal that you logged on the Nutrition Tracker widget, go to the Diet and Nutrition log from the Health Tools tab. Make sure that you are viewing the day that you logged the item you wish to update or remove. You can change the days with the tabs at the top of the planner. If you want to remove the item, click Remove on the right. If you want to make an update, click on the item, edit with the notepad/pen icon and then click Update. Adding information on the log will not be reflected on the widget. Widgets My Activity Tracker How do I use the Activity Tracker? To log your activity using the Activity Tracker widget, select the date you wish to log an activity (note: the widget defaults to today's date). Then type in the activity that you wish to log. A list of matches will appear and you can pick an activity that is the closest match. Next, enter the number of minutes that you performed the activity and the activity type, and hit the "Submit" button to complete. How do I view what I documented on the Activity Tracker? Information entered on the Activity Tracker widget can be viewed by going to the Health Tools tab and then selecting Physical Activity Log.
7 How do I remove or update an item on the Activity Tracker? To remove or update physical activity you logged on the Activity Tracker widget, go to the Physical Activity Log from the Health Tools tab. Make sure that you are viewing the day that you logged the item you wish to update or remove. You can change the days with the tabs at the top of the planner. If you want to remove the item, click Remove on the right. If you want to make an update, click on the item, edit with the notepad/pen icon and then click Update. Adding information on the log will not be reflected on the widget. Widgets My Glucose Tracker How do I use the Glucose Tracker? To log your glucose using the Glucose Tracker widget, select the date you wish to log your glucose (note: the widget defaults to today's date) and select the time. Then enter the number under glucose and click Submit to complete. How do I view what I documented on the Glucose Tracker? Information on the Glucose Tracker widget can be viewed by going to Track Your Numbers and selecting Glucose. How do I remove or update an item on the Glucose Tracker? To remove or edit your glucose, go to Track Your Numbers and select View Log. To edit the glucose, go to the appropriate date and time and select the notepad/pen icon. To remove the glucose, find the date and hit the X icon. Widgets My A1C Tracker How do I use the A1C Tracker? To log your hemoglobin level using the A1C Tracker widget, select the date you wish to log your hemoglobin (note: the widget defaults to today's date) and select the time. Then enter the number under % hemoglobin A1C and click Submit to complete. How do I view what I documented on the A1C Tracker? Information on the A1C Tracker widget can be viewed by going to Track Your Numbers and selecting Hemoglobin A1c (HbA1c). How do I remove or update an item on the A1C Tracker? To remove or edit your hemoglobin, go to Track Your Numbers and select View Log. To edit the hemoglobin, go to the appropriate date and time and select the notepad/pen icon. To remove the hemoglobin, find the date and hit the X icon.
8 Healthy Living Tab What is the Healthy Living Tab? The Healthy Living tab houses health education articles relating to general wellness topics. The articles create awareness about health risks and provide actionable strategies to fit healthy behaviors into your lifestyle. All of the health education articles about healthy living are written or approved by LiveHealthier staff or come from other LiveHealthier reviewed and trusted sources. Topics include: back and neck care, diet and nutrition, immunizations, maternal and child health, physical activity, safety, screening recommendations, stress management, tobacco cessation, and traveler s wellness. Health Conditions Tab What is the Health Conditions Tab? The Health Conditions tab houses health education articles relating to prominent health conditions. The articles provide substantial information to help educate about the prevention, symptoms, diagnosis and treatment of several conditions. All of the health education articles are written or approved by LiveHealthier staff or come from other LiveHealthier reviewed and trusted sources. Topics include: brain health, cancer, clinical trials, diabetes, emotional health, headaches and migraines, heart health, obesity, osteoporosis, and pulmonary conditions. Health Tools Tab What is the Health Tools Tab? The Health Tools tab contains information to further assist you in making positive lifestyle changes or maintaining current healthy habits. You have the ability to set goals, log daily routines, create personal plans and track progress toward health behavior change. Health Tools Tab Diet and Nutrition Log What is the Diet and Nutrition log? The Diet and Nutrition log is a health tool located under the Health Tools tab that allows you to log and track your nutrition intake. If it is your first time accessing the Diet and Nutrition log, you will need to set your meal plan, a start date and your calorie level. Documenting information in the diet log is for your own personal use. How do I record information here? On the left-hand side of the page, select the day you plan to consume the food or beverage. Under the Nutrition Browser on the right-hand side of the page, use one of the four options
9 (Food Items, Favorites, Meal Inspiration or Search) to select the food or beverage item. Then enter the serving size that you plan to eat or drink. From the Add Item to Plan drop-down menu, select the appropriate category. Then click Add to Plan. Once the food or beverage has been consumed, find it on your plan and click Log. Be sure to click Log for the entries to register as logged items rather than planned items. Click on the item on your plan on the left-hand side of the page to edit, remove or add comments. What is a meal plan? A meal plan is a list of food and beverages that you plan to consume during the day based on a set calorie level. Meal plans allow you to track your daily nutrition intake and can assist in reaching nutrition goals. You can create your own meal plan by adding items to your daily plan on the Diet and Nutrition log. Adding items to your Favorites list makes it easier to add to your daily plan. To change your meal plan, click Change next to Meal Plan on the Diet and Nutrition log. To change your calorie level, click Change next to Calorie Level on the Diet and Nutrition log. You can also print your weekly plan and use it to create a shopping list by clicking Print Week s Menu/Shopping List. Health Tools Tab Physical Activity Log What is the Physical Activity log? The Physical Activity log is a health tool located under the Health Tools tab that allows you to log and track your physical activity. If it is your first time accessing the Physical Activity log, you will need to set your BMR (basal metabolic rate) and your lifestyle. In the future, if you would like to change your BMR, go to the Physical Activity log and above Activity Browser, you can click Change BMR. Documenting information in the physical activity log is for your own personal use. How do I record information here? On the left-hand side of the page, select the day you plan to complete the physical activity. Under the Activity Browser on the right-hand side of the page, use one of the four options (Activity List, Favorites, Inspire Me! or Search) to select the activity. Then enter the duration of time in minutes that you plan to perform the activity and the appropriate workout category, and click Add to Plan. Once the activity has been completed, find it on your plan and click Log. Be sure to click Log for the entries to register as logged activities rather than planned activities. Click on the activity on your plan on the left-hand side of the page to edit, remove or add comments. What is a physical activity plan? A physical activity plan is a list of the activities that you plan to perform. Physical activity plans allow you to track your daily activity and can assist you in reaching physical activity goals. You can create your own activity plan by adding activities to your daily plan on the Physical Activity log. Adding activities to your Favorites list makes it easier to add to your daily plan. To change
10 your physical activity plan or duration, click on either Exercise Plan or Duration located above the Activity Browser. To view all the activities that you completed for the week, click on Print Week s Plan on the calendar. Health Tools Tab My Page What is My Page? My Page helps you to stay organized, motivated and empowered in finding ways to live healthier every day. Here you can connect with other colleagues from your region and work together to find new routines and behaviors that will further improve your healthy lifestyle. Health Tools Tab Goals and Plans What is Goals and Plans? Goals and Plans is a health tool which allows you to write down your health goals and create a plan to achieve them. Access Goals and Plans from the Health Tools tab on the top navigation bar. Click on Add New Goal. Add the information in the fields to outline your goal and click Create. You can also fill out smaller steps that will help you reach your goal. Health Tools Tab - Energy Balance What is Energy Balance? Energy Balance is a health tool which allows you to view your energy balance over time. This will show if you are losing, maintaining or gaining weight based on the entries you have logged in the Diet and Nutrition log and the Physical Activity log. Track Your Numbers How do I track numbers such as BMI, glucose, cholesterol, blood pressure and pulse rate? The Track Your Numbers page allows you to track measurements including BMI (body mass index), glucose, cholesterol, blood pressure, pulse rate and hemoglobin. You can enter a measurement manually by clicking New Entry and recording the date, time and health data. Hit the Submit button to complete. To change or remove a measurement, first click on that measurement and then select View Log. To remove an entry, click the X icon on the right. To update an entry, select the notepad/pen icon to edit and then click Submit. Diabetes Center What content is available in the Diabetes Center? The Diabetes Center has articles, handouts, videos and more to help you manage your condition.
11 Privacy Policy and Customer Service Where can I find the terms of use and privacy policy? The terms of use and privacy policy are both located at the bottom of the wellness portal page. How do I contact LiveHealthier Customer Service? Click on the Customer Service link at the bottom of the wellness portal page or [email protected]. Standard business hours are Monday through Friday, 6:00 a.m. to 6:00 p.m. CT. Queries left during non-business hours will be responded to within the next business day. In addition, you may also contact live customer service during our standard business hours via the link at the bottom of the wellness web portal or call
