3 YOUR 2012 BENEFITS STATE HEALTH BENEFIT PLAN MEMBERS New State Health Benefit Plan Wellness Options UnitedHealthcare is happy to announce new enhancements to the SHBP HRA, High Deductible Health Plan (HDHP) and Health Maintenance Organization (HMO) Plan Options for These new WELLNESS plans are designed to help you and your spouse to become more actively engaged in developing healthy behaviors as a way of life. In return, you will be rewarded for your active participation in taking steps that may improve your health and well-being. The power to engage is yours! If you are currently covered by SHBP, you may elect coverage in one of the three WELLNESS Plan Options. By making a WELLNESS Promise to have you and your covered spouse complete your Health Assessment and required biometric screenings by June 30, 2012, you are able to take advantage of lower premiums, copayments, coinsurances, deductibles and out-of-pocket maximums. However, for those of you who do not wish to make the WELLNESS Promise, you may elect to enroll in one of the Standard Plan options which will have higher premiums and higher out-of-pocket expenses.
