1 19 - Health Education and Wellness 19.1 Southern Nevada Health Education Program Offerings The Health Education and Wellness (HEW) Division believes that education is an integral part of preventative health. We provide programs dedicated to improve the lifelong wellness of your patients. We recognize that wellness is a never-ending process involving physical, emotional, intellectual and spiritual health. Our goal is to prevent illness and to help your patients manage existing health concerns through knowledge and provision of tools they need to make self-care decisions to improve their quality of life. Programs offered include, but are not limited to the following: Asthma (adult and child) Cancer Nutrition Chronic Obstructive Pulmonary Disease Diabetes Exercise Grocery Shopping Tours Healthier Living Heart Failure Heart Health (includes Cholesterol, Hypertension and Triglycerides) Lactation Lifestyle Management Nutrition and Fitness Pregnancy Preventive Healthcare Senior Health Stress Management Support Group Tobacco Cessation Program Weight Management (Adult [age 18 and over], Teen [ages 14-17], Child [ages 7-13] and Toddler [ages 2-6]) Weight Management Support Group Women s Health Medical Nutrition (available as Registered Dietitian consultations only) The Tobacco Cessation Program (TCP) is a collaborative partnership between HEW, HPN, and SMA. The program involves behavior modification one-on-one sessions and support group sessions. Program overview: Program orientation provided for all potential participants. One-on-one assessment and treatment plan developed for each participant. Treatment plan includes both behavior modification and possible medication therapy. 3-month treatment program for medication includes Chantix, Zyban and Nicotine Replacement Therapies (NRT) of the patch, gum, and lozenge (brand provided by THC or chosen by patient). Zyban and Chantix prescriptions are authorized only for participants in the Tobacco Cessation Program. Participants are required to attend at least 10 behavior modification sessions, in the form of group and one-on-one sessions, but patients may attend as many sessions as needed. 2 There is no cost to Health Plan of Nevada, Sierra Health & Life, Senior Dimensions, Smart Choice or Nevada Check-Up members for the program. Non-members will be required to pay a $5 orientation fee and $55 program fee. Health plan members, with the exception of Senior Dimensions, are eligible to receive medication offered free of charge pending insurance eligibility and compliance within the program. Non-members are not eligible for the medication component of this program. Senior Dimensions members will pay the generic co-pay for Zyban or the brand name co-pay for Chantix.The patient s provider and the health educator with the credential of licensed alcohol and drug counselor are involved in the TCP protocols. Consultations are available for any health education program listed above and for all dietary/nutrition needs. Staffed with a Certified Health Education Specialist (CHES), Registered Dietitians (RD), Certified Diabetes Educators (CDE), Licensed Alcohol and Drug Counselors (LADC), and a team of individuals with a Master s Degree in Exercise Physiology and in Health Promotion, the Health Education and Wellness Division is designed to focus on both preventive care and the management of existing health concerns. 4 One-on-one Health Education Consultations with a health educator are available onsite, by phone, and over a secure video connection using Telehealth, at the Senior Dimensions office and are recommended for patients who are high risk and/or those who may be at risk, having special health issues. Consultations are available for any health education program listed above. All health education programs/one-one-on consultations are available at the following locations: Senior Dimensions Office, 5370 Kietzke Lane, Suite 100, Reno, NV Our health education team is comprised of professionals dedicated to teaching people the skills needed for lifelong health. This referral system involves patients participation in health education programs that tie directly to their physician s referral for a more complete care package. Providers receive information on their patients progress through chart noting outlining patient participation, concerns, and whether or not the patient completed the program. Access to the above service may be obtained by phone or through a patient referral (paper or electronic). We will assist you and your staff by coordinating, scheduling and providing health education programs or one-on-one appointments with a health educator for your patients. This product offers a computer-based, step-bystep program to help members better manage their health concerns. Each module includes health information, as well as interactive tools and webisodes for further learning. HPN 2014 Section 19 Health Education and Wellness 4

5 The Online Learning Modules include the following topics: The Balancing Act - Diabetes Blood Pressure, Cholesterol, Triglycerides - Heart Healthy Expectations - Pregnancy Foundation for the Future Preventive Healthcare The Puff Stops Here Stop Smoking LEAN on Me Weight Management There is also a personal Health Risk Assessment (HRA) available online. The HRA can be the first step on the road to better health. It is designed to help identify and prioritize health and wellness goals. After completing the HRA, a profile will be generated automatically for the member to print. The profile provides the following information: Personal health risk factors Health and screening recommendations Programs and services that meet the member s needs To access the Online Learning Modules, follow the steps below: Go to If the member already has an account (through Health Plan of Nevada or Sierra Health and Life), the member would type in their username and password and select submit. If the member does not have an account Service, the member would click the Create an account link and follow the instructions listed. The account type will be member and the username is the member s 11-digit member number (no dashes) found on their insurance card. Once the member is logged they can click the MyHEWOnline box to the right of the screen. This will automatically log them into MyHEWOnline and they can access the Health Risk Assessment (MyHRA), online learning modules, and various webisodes and health education tools Worksite Wellness Offerings Worksite Wellness Centers are established at select corporate locations through contractual agreements between the Worksite Wellness section of Health Education and Wellness and those organizations interested in having their own customized program. If you have questions or would like more information about worksite programs, please call (702) The following core components are included at no cost: Needs Assessment o HEW will provide a link for an electronic survey to be sent to employees. o Aggregate results report will be provided for management. Health Communications o Marketing materials such as flyers provided to either and/or display on bulletin boards to communicate information about HEW and MyHEWOnline. o Standard health education materials on popular health education topics (diabetes, heart health, weight management, smoking cessation) provided for employers to distribute (each topic template will include an article, health tips, e- mail blast and recipe). 6 Customized Website Portal - MyHEWOnline Personalized website developed for employees to access the health risk assessment (HRA), online learning modules, resources, tools and webisodes. o o Health Risk Assessment (HRA) The HRA is an online tool used to assess health risks. Results from the HRA flag health risk factors that require additional attention by the employee. Employees receive a personalized profile report. Aggregate result report provided for management. HEW staff will meet to present recommendations to management team. Online Learning Modules Learning modules available on the following topics: diabetes, heart health, pregnancy, preventive health care, stop smoking and weight management. Ability to track module completion Employees can print a certificate of completion. Employee Health Challenges o If you are interested in having an onsite employee health challenge program, the following will be included: o Electronic collateral materials to print and facilitate the 8-week program. o Health educator trains designated employee to become a facilitator to conduct onsite program. Note: it is the employers responsibly to pick designated employee and for the employee to market, program plan and implement. HEW s role is to provide the training only. Employee Health Fairs o Health fairs are available for employers with 500 or more employees. Additional buy-up options are available, including onsite biometric screenings, educational services, incentive programs and coordination of health events. Health Education and Wellness has several additional worksite wellness incentive-based packages and customized bundles to meet your worksite wellness needs. My Way to Wellness was recognized as a 2011 Finalist for the Case In Point Platinum Award in the category of Wellness/Prevention. This is an award program that recognizes the most successful and innovative case management programs working to improve health care across the care continuum. My Way to Wellness, created for the Clark County School District support staff, is a 12-month program that starts with an online Health Risk Assessment. The tool is reviewed with the health educator and areas of focus are identified that can be worked on. A wellness visit is scheduled with a PCP and each month participants receive a mission card that keeps them on target with their task. They are also connected to a wellness consultant to help set realistic goals and address challenges. Those who reach their goals are featured in a success story that goes out HPN 2014 Section 19 Health Education and Wellness 6

7 to all employees. The program was designed to ensure quality, consistency and motivation throughout the year. Health Becomes You, a worksite wellness program created for the Clark County School District employees, was chosen as silver winner for the School Employee Wellness Award Program in 2013 by the Directors of Health Promotion and Education (DHPE). This is awarded annually to select school employee wellness programs for their excellence in planning and implementing school employee wellness programs that promote employee health and lead to improved productivity and lower health care costs. This comprehensive program includes on-site biometric screenings, Health Risk Assessment, 1-on-1 consults with a health educator, intramural events, seminars on various health education topics, online incentive programs and fun events.
