1 1 XAVIER UNIVERSITY OF LOUISIANA THE INSTITUTE FOR BLACK CATHOLIC STUDIES CHECK LIST THE FOLLOWING LIST WAS PREPARED FOR YOU, TO KEEP TRACK OF EVERYTHING YOU NEED TO DO THROUGHOUT THE PROGRAM BEFORE ARRIVAL AT XAVIER UNIVERSITY OF LOUISIANA A. PREPARATION FOR SUMMER PROGRAM (Use the following checklist to check off your completed action) Read this publication, paying close attention to the particulars of your program Degree or the Continuing Education (C&E) Program. By the end of April, please make sure all those who are preparing a letter of recommendation for you have received the Letter of Recommendation Form. If you need to prove proficiency in English, please make sure you have taken the TOEFL Test. B. COMPLETE APPLICATION HOUSING AND OTHER FORMS Application/Pre-Registration Forms for Admission. Statement of Intention (written by NEW/TRANSIENT DEGREE and NEW C&E STUDENTS APPLICANTS ONLY). Have official transcripts sent from your university to XULA Institute for Black Catholic Studies (for NEW/TRANSIENT DEGREE and NEW C&E STUDENTS ONLY). Application fee (check or money order to be sent with application package). Letters of Recommendation (pay particular attention to each program specifications). Housing Application Form Scholarship Application Form and all accompanying information (if applicable). Health Forms: Please send the health forms directly to Xavier University Health Services with a copy to the IBCS office This is due as early as possible. Make a Portfolio for yourself with copies of all materials sent to the Institute. C. MAIL ALL APPLICATION FORMS (Send Application package and Health Forms before the deadline dates) JANUARY 21 APRIL 30, 2014 Early Application (and) Scholarship Eligibility Students will be eligible to apply for a scholarship ONLY during the early application process. MAY 1 JUNE 27, 2014 Regular Application (and) NO Scholarship Eligibility Students will NOT be eligible to apply for scholarships during the regular application process. JUNE 28 JULY 18, 2014 Late Application / Late Fee Assessed Students will NOT be eligible to apply for scholarships during the late application process. (NOTE: Applications received AFTER June 1 st will be charged a non-refundable late fee of $100.00)
2 2 D. PAY FEES All other fees (including tuition, room and board, activity fee, technology fee, laundry fee, etc.) should be paid by May 1, (See Fee schedule at the end of this form). E. PURCHASE BOOKS AND READ IN ADVANCE Check for booklist on the website and be prepared to purchase them at the University Center s Barnes and Noble Bookstore, or in advance at Amazon.com, or other online service. Begin reading texts, and Xavier University rules and regulations on webpage at ( in preparation for summer classes. F. PACK USING THE WHAT TO BRING LIST Double check and pack items from the on-line What To Bring List at the following link: UPON ARRIVAL AT XAVIER UNIVERSITY OF LOUISIANA A. CHECK IN Check in at St. Martin De Porres Residence Hall (Building #44) Unpack your things and get settled in. B. LATE REGISTRATION JUNE 28 JULY 18, 2014 All students must complete FINAL REGISTRATION and pay out ending balance of fees due to the University by this date regardless of the Program or week the students will be attending! C. HEALTH COURSE APPROVAL REGISTRATION AND FISCAL CLEARANCES Go to the University Health Services, located in St. Joseph Student Center, 3 rd floor for Health clearance. For those who have been health cleared, return to Xavier South (Building #40) to the IBCS Office, Room #560, to meet with the Associate Directors of the Degree and C&E Programs, who will be your immediate Advisors. Have your schedule approved by your program advisors Proceed to the UC, rooms 205 and 205C for final registration, and to Student Accounts to pay tuition and all other fees that are due. D. IDENTIFICATION CARDS To get Xavier ID cards, go to the Card Services Office in the University Center, room 122, across from the Post Office.
3 3 E. BOOK PURCHASE Book purchases are at Xavier University Center s Barnes and Noble Bookstore 2 nd floor of the UC. F. PARKING PASSES Stop by the IBCS Office in the Xavier South Building Room 560 (fifth floor). G. CHECK OUT IMPORTANT!!! Before leaving Xavier campus, please make sure that your housing key is turned in to avoid being charged and billed $25.00 for a replacement key. Also, please make sure that all of your financial obligations are settled. (Thank You). H. FEES (PLEASE SEE FEES ON NEXT PAGE) Please do not hesitate to call the IBCS office if you have questions or concerns.
4 4 Institute for Black Catholic Studies 2014 FEES SUMMER FEE SCHEDULE FOR DEGREE AND C&E PROGRAMS APPLICATION FEES (Required of all students) These fees are non-refundable and due at the time application is submitted. $ Application Fee $ Housing Reservation Fee (On campus residents only) $ Late Fee (late registrants only) REGISTRATION FEES 1. COURSE / PROGRAM REGISTRATION $10 Student ID $135 Activity Fee (covers general program & Community Life costs) 2. TUITION: (Rates are subject to change) A. MASTERS DEGREE PROGRAM: $200 Per semester hours (i.e. 3 semester hours = $600 per course) 3. TECHNOLOGY FEES (Master Degree Students Only) 1-4 semester hours = $50 (i.e. Once course = 3 semester hours) 5-8 semester hours = $ semester hours = $ semester hours = $200 B. CERTIFICATE AND ENRICHMENT PROGRAMS: (Rates are subject to change) $200 - Tuition per week (Eldership Retreat) $285 - Tuition per week (Catechists, Leadership and Youth Ministry) 4. HOUSING (Rates are subject to change) $230 per person Room and Board for double occupancy (2 weeks=$460) (3 weeks=$690) $340 per person Room and Board for single occupancy (2 weeks= $680) (3 weeks - $1020) $ 15 Laundry fee (Required of ALL on-campus residents) FYI: Ordinarily, non-local students are encouraged to stay on campus and to share housing, (double occupancy) to facilitate community and to encourage collaborative study. NOTE: Requested by some seminarian formation directors in compliance with Episcopal guidelines as well as some older adult students who have special needs.
5 5 PLEASE BE AWARE OF THE FOLLOWING: 1. Registration, Tuition, and Technology fees, (Master Degree Students ONLY), are due April 1, 2014, for early and regular applications. Late application fees are due no later than June 15, Non-receipt of payment at these times may result in automatic deregistration. Returned checks will result in immediate deregistration and a charge of $35.00 for NSF. 2. If the student s tuition and required fees are included in a check issued by a joint sponsoring agent or groups, be sure to supply the accompanying documentation that clearly identifies the name of the student, his/her program of study, and any itemized monetary disbursements. 3. Please do not include money for the student s textbooks, class materials, supplies, copying, etc, in the tuition and fee check. These items are paid for separately. 4. Please bring a file folder as your documentation file containing copies of all application information with you, (correspondence, application, health clearance, checks and related data, etc).
