5 ADULT BEHAVIORAL HEALTH UNIT SOCIAL SERVICE WORKER Salary: $2, $2,838.00/mo. Opened: 10/30/15 Posting#: Position# 125 Perform complex social service work. This position will be assigned to Level of Care 4. This position will be trained to conduct clinical screenings, assessment, and service eligibility based on consumer need. The individual will be responsible to coordinate, link and authorize appropriate service, and to monitor the progress of those individuals in meeting treatment goals. Deliver interventions such as case management, rehabilitation skills, group\individual sessions, crisis resolution, and discharge planning base on the need of the particular unit. The worker will ensure that all cases have an updated treatment plan, ANSA, authorizations, financials, diagnostic assessments, and that all service records are submitted within 24 hours. This individual will also be serve on the crisis on-call rotation on a weekly basis. Work will adhere to the facility productivity standards depending on the program assigned to at each month. Opened: 10/15/15 Posting#: Position# 746 The TCS will perform moderately local authority service coordination\service delivery work based o program grant, or funding guidelines. This position will be stationed in Webb will require traveling to Starr, Jim Hogg and Zapata Co. The TCS duties (but not limited to) traveling to consumer homes and other places in the community on an as needed basis to assess needs, develop cessation plans and deliver tobacco cessation services as part of the goals on a Person Centered Plan(PCP). The SSW will assist consumers in accessing medical, social, educational and other services that will help the consumer achieve quality of life and community participation acceptable to the consumers, will also provide the execution of recovery\rehabilitation plans, in the area of skills streaming, medication training\support and family/child education. The Case Manager will carry a caseload with a minimum of 45 and a maximum of 70 for a routine case manager and a minimum of 20 with a maximum of 30 for an intensive case manager. The SSW ensures that his\her caseload have updated recovery plan, ANSA\CANS authorizations, financials, diagnostic assessments, demographics and submit all progress notes within same day of the delivery of service.
