1 COMMUNITY AND FAMILY MEDICINE JOB POSTING Sept 3, 2014 To apply for any of these positions, please submit your resume to our employment website at HEALTH EDUCATION SPECIALIST Division of Community Health Engage network assigned patients to provide health education, coordinate access to health and social services and follow-up on recommendations from care management assessments. Answers questions and supplies detailed information both in person and on the phone to clients, case managers, and other service agencies. Receives and processes a variety of requests for information, registrations, actions, forms, orders, notices, and/or other related information. May conduct initial interviews and evaluate customers' eligibility and needs. Follows through on client needs through coordination of services. HEALTH EDUCATION SPECIALIST Division of OEM- Live for Life Work as the Health Education lead in the coordination of the Duke Farmers Market, Mobile Market, and Wellness Expo events. Assist in the coordination of the chair massage program, implementation of tobacco cessation outreach events, distribution of marketing materials, scheduling of group presentations, evaluation of health education programs and activities, and other special projects & initiatives as needed. Oversee the scheduling and tracking of referrals into various LIVE FOR LIFE databases. Develop health education marketing materials and messages in line with the business plan and assist with program marketing by regularly updating website. Develop, analyze, and create reports from online surveys. Uphold LIVE FOR LIFE business agreements with outside vendors. Assist in supervision of Health Education Intern. COUNSELOR Division of Community Health The person in this position will perform a variety of research and data processing activities for studies related to childhood obesity. The job involves: in-person, phone, and written contact in English with study participants to coordinate study visits and administer questionnaires; management of participant files; counseling and coaching participants, leading health behavior workshops, research participant case management, data collection, data entry, knowledge of research ethics and clinical research guidelines, ability to follow study protocols with a high degree of fidelity, general organizational skills for conducting research activities; and general support of the research team. The person in this position should have excellent communication skills, be comfortable working with diverse individuals, and have some experience or familiarity with recruitment and data entry. This position requires some evening/weekend time and access to a car; all mileage will be reimbursed. The person in this position will be expected to
2 travel to participant homes and other research sites in the greater Triangle area to collect study materials according to a regular schedule to include some early evening hours. Work REQUIRES a Bachelor s degree in Psychology, Sociology, Education or a related field with one or two years of experience in Social work, employee assistance counseling or related setting and clinical licensure OR an equivalent combination of relevant education and or experience. COUNSELOR, Occupational Medicine -EAP program Counselor is involved in a variety of Employee Assistance Program (EAP) services designed to enhance health and productivity of Duke work force. Counselor is responsible for making independent decisions regarding the clinical assessment and management of cases, consultation to management, and addressing occupational mental health issues in the workplace. Provide direct clinical services, including comprehensive mental health assessment, short-term counseling, and referral services to Duke Faculty, staff, and eligible members of their immediate family. Consult with management about occupational mental health and productivity issues. Assess and provide organizational consultation and interventions for events that disrupt the emotional stability and functioning of the work unit. Provide specialized services to various client groups within the University and Health System, notably employees with work injury or illness, substance abuse, suspected impairment, or behaviors of concern. Serve as a mental health professional on Duke#s Critical Incident Response Program, utilizing Psychological First Aid and crisis response. Design and conduct training sessions for management about employee performance problems, personal problems, mental health, and resources available through PAS. Candidate should have a minimum of a Master s Degree in Social Work, Counseling, OR related Mental Health field and a current clinical licensure in North Carolina AND 3 or more years of experience in Social Work, Employee Assistance Counseling or related setting. Nursing Program Coordinator Division of Community Health The complex care nurse-clinician will develop and manage the Duke Children s Complex Care Service designed to support Duke University Health System (DUHS) and Community Care of North Carolina (CCNC) in their efforts to achieve the triple aim of better health and better care at lower cost for the children with medical complexity (CMC) patient population. S/he will provide direct care management for a panel of eligible CMC longitudinally at inpatient and outpatient points of care, and will help to build and refine the infrastructure of this new service line Family Medicine Physician - Clinician/Educator Duke Family Medicine is seeking a full-time, board-certified family physician (MD or DO) to join a thriving and highly innovative practice which is testing new models of care and innovations in education. This position organizes, promotes and carries out all externally-focused events and communications, online lecture series, conferences hosted by the Practical Playbook. MEDICAL TECHNICIAN Division of Occupational Health The desired candidate will administer routine vaccinations; manage the fit testing program which includes maintenance and troubleshooting of equipment, accurate fit testing for N95 masks for tuberculosis. Will also maintain detailed records of tests performed and report laboratory test results to appropriate personnel. Will be responsible for collecting blood specimens from patients using venipuncture or finger stick and perform drug screenings. Candidate will maintain clinical supply of patient care supply and perform other related duties incidental to the work described herein. 20 hour/week position. OCCUPATIONAL HEALTH NURSE Division of OEM We are seeking an experienced nurse who is passionate about the health and safety of employees in Duke EOHW. EOHW services the 30,000 + faculty and staff of the University, Medical Center and Health System. Requires minimum of 1 year adult nursing or medical office experience. Monday- Friday. Alternates approx. every 5-6 weeks with other nursing staff as early (7:30am) clinic nurse and may be needed to staff EOHW at Duke Regional hospital on a rotational basis. Occasional weekend work may be required for special projects. One full-time position.
6 NURSING COORDINATOR Division of Community Health Maintain effective communication with health care team members related to discharge planning and utilization management; Provide educational information in conjunction with direct care providers related to treatments, procedures, medications, and continuing care requirements; Ensure patient understanding and agreement regarding care objectives; Identify progress toward desired care outcomes; Identify potential barriers to the implementation and follow-through of the expected plan of care; Coordinate and facilitate timely implementation of discharge plans for assigned patients; coordinate follow-up care and services as appropriate with receiving care management program (RN, SW, RD, etc); Verify discharge medication data and coordinate with Network pharmacy program to begin post-discharge medication management process; Act as an educational resource and provide consultation to hospital medical personnel regarding discharge planning process and applicable federal, state and local regulations; identify benefits, implications and limitations of home care as appropriate. Conduct transitional care follow-up home visit within 72 hours of discharge with care manager and follow transitional care follow-up protocol; Conduct any necessary follow-up coordination with care management program Document findings, discharge arrangements and actions taken according to departmental guidelines; prepare reports and maintain records as required;
