1 SAN MATEO COUNTY BEHAVIORAL HEALTH & RECOVERY SERVICES DOCTORAL LEVEL PRACTICUM PLACEMENTS San Mateo Behavioral Health and Recovery Services (BHRS) is a comprehensive county behavioral health services program dedicated to serving the needs of adults with serious mental illness and/or co-occurring substance abuse concerns, as well as severely emotionally disturbed youth and their families. Services are offered to children, adolescents, adults, and older adults in inpatient, outpatient, and outreach programs by multidisciplinary staff that includes psychologists, psychiatrists, nurse practitioners, nurses, social workers, MFTs, trainees, interns and peer support positions. BHRS provides clinical training opportunities to students pursuing their masters degree, their Alcohol and Drug Certification, or their doctorate. This packet describes practicum opportunities for doctoral level students. We offer 4 main placement sites for Practicum Students: Adult Forensic in Redwood City, Juvenile Forensic in San Mateo, Youth outpatient in Daly City, and Adult Outpatient in San Mateo. All of these sites have licensed psychologists serving as primary supervisors. Training: San Mateo County is proud of its training programs as we provide training opportunities for approximately psychology interns, masters-level trainees, Alcohol and Drug Certificate Program students, and psychiatric residents every year. For psychology practicum students, there are opportunities for individual supervision, group supervision, and case conferences. All students are also required to attend any of the five didactic training seminars throughout the county. There are bi-monthly psychiatric grand rounds at the hospital that are open to all staff and students. Regular in-service trainings and specialized staff trainings are also available for students to attend. Cultural Competency Stipends are available for those who are able to help provide services to underserved populations within our system of Older Adult, Adult, and Youth Services. Each stipend is up to $5,000 for an intern or trainee who works 24 hours in their placement. Stipends will be pro-rated if the student works fewer hours. Please ask for a stipend application if you are interested. Included in this packet is information on each of the placement sites. (training and meetings) Program Description: The Youth Services Center offers an excellent opportunity to develop psychological assessment and report writing skills, while also serving adolescents and their families in multidisciplinary forensic settings. Students must complete coursework in psychological testing prior to writing psychological reports for the Court. Trainees devote at least half of their time to one of 5 direct, clinical assignments alongside their testing and report writing: 1. The Forensic Team provides the following to youth detained in the County of San Mateo s Juvenile Hall: crisis response, assessment, brief and longer-term individual and family therapy, group therapy, and case management. 2. The clinical team at the Margaret J. Kemp Girls Camp engages in gender-responsive, culturally competent, and trauma-informed group, individual, and family therapy to address mental health needs and co-occurring disorders. Demands of this team may limit availability for psychological assessment and report writing. 3. Clinicians at Camp Glenwood collaboratively deliver individual, group, and family therapy, as well as crisis response and case management to adolescent boys who have been committed for several months to a program-intensive forest location near La Honda. 4. Additionally, the Community-Based Family Therapy team works closely with probation officers to serve disenfranchised families within their communities. The team provides linguistically and culturally competent services primarily in families homes with some on-site work and case management. Demands of this team will preclude availability for psychological assessment. 5. Please see also a description for our Assessment Center on the following page. 5 Youth Services Center Assessment Center: Practicum The Assessment Center is the entry point for every youth in our county placed on Probation or diverted from it. The Center s multidisciplinary team decides a recommendation to the Court regarding the youth, or it creates a diversion contract for the youth. When possible, this team also accepts referrals from sources throughout the community, including Human Services Agency, schools, and other agencies to determine initial interventions and appropriate case plans. This could include short-term direct services (e.g. individual and family therapy with case management). At the Assessment Center, students will learn how to: Use the Probation PACT interview (which creates a recidivism risk probability number). Use the MINI KID, Mini International Neuropsychiatric Interview to gather initial evidence suggesting the possibility of DSM IV-TR Axis I and II diagnoses. Collaborate with a Probation Officer who has interviewed the family and gathered other relevant information from, school, CPS, case workers, and prior law enforcement contacts. In cooperation with the Probation Officer present a recommendation to the Court. Assess conjointly with a Probation Officer a youth s eligibility for diversion. Co-create with the Probation Officer a Diversion Contract addressing all relevant risk factors. Make appropriate CPS reports Provide crisis response with on-site supervisor support. Challenges: Determining a provisional diagnosis regarding the youth using the above mentioned tools in a limited amount of time. Communicating your findings with professionals from different disciplines with differing perspectives. Advising the Court whether, based on your diagnostic information, a full Forensic Mental Health Evaluation will be needed to aid the Court in determining an appropriate disposition and treatment plan for the youth. Building a bond of trust between a youth and yourself in a limited amount of time, in a juvenile justice institution. Must be available Thursdays from 8:30 to 1:00. Must be able to work at least one evening/week until 6 or 7 pm. Must have a driver s license and car insurance to be able to drive to school and other sites on behalf of the client. Placement runs from the first week of September to the last week of May. Program/Duties: Work with children 0 18 years of age. Perform clinical assessments; individual, family and group therapy. Perform Special education assessment when required. Collaborate with agencies involved in the client s treatment. Collaborate with the school the client is attending. Work with a diverse ethnic population. The program offers individual, group, family therapy, medication support & clinical case management services within a multi-ethnic, multidisciplinary team environment. Our team is committed to providing integrated treatment to clients with psychiatric problems, substance abuse, developmental and/or medical conditions. The emphasis of treatment is culturally congruent, evidenced based with wellness & recovery focused in order to maintain hope & support the clients goals of healthy interdependence & self-sufficiency with family & peer support. CLIENTELE SERVED: We serve adults over 18 years of age whose symptoms are severe & significantly impact level of functioning. Our clients are low income, disadvantaged & culturally diverse. INTERN ASSIGNMENTS: Students join our multidisciplinary team & provide clinical case management & therapy to a case load of 12 clients. In addition, students co-lead 1-2 weekly groups (e.g. Psycho educational, Skills Groups or Dual Diagnosis). Of the 12 clients, half would be amenable to traditional psychotherapy using Staged Model of Trauma Treatment, CBT and DBT. Students are trained to provide semistructured diagnostic interviews & complete comprehensive biopsychosocial assessments & take on an Officer of the Day shift, with staff support, increasing crisis management skills. INTERN TRAINING AND EDUCATIONAL OPPORTUNITIES: In addition to weekly individual supervision, team meetings, staff meetings & group supervision, interns are provided a weekly Master s Seminar & Grand Rounds lectures. San Mateo County offers multiple trainings & students are required to take on-line training (Avatar, HIPPA, Locus) & attend didactic trainings. Trainees are also encouraged to participate in Health Equity Initiatives that specifically targets cultural groups.
