3 Information Athlete and Parent/Guardian Statements Applicants and their parent/guardians are required to include a brief declaration in their application package outlining their reasons for application on the forms provided. Program Costs Total Program Cost for CSI Golf is $1325 The charge to a student will be related to the direct costs incurred that are in addition to the costs of providing a standard educational program. Budget: Honorarium for Coaches and Instructors who are not Claremont staff members Supplies, clothing, and equipment Athlete Transportation Program Management Facility Costs Green Fees at Cordova Bay This a program of choice therefore full payment is required. A deposit of $ will be required once the student has been accepted in the CSI and is required by May 31, Of the $ deposit, $ will be non-refundable. Once the program begins in September no refund will be available. The balance of the CSI program payment is due by September 30, 2015 in full or by credit card. Please ensure you stay informed on the new changes and the impacts on student athletes, especially prior to entering their grade 8 year(1st year of eligibility). (Darren Reisig at dreisig@sd63.bc.ca) Admission and Registration The Claremont Sports Institute Teachers and an Administrator will review all applications. If there are more applicants than space permits, admission will be based on the strength of the application. Coach and/or administrator references may be contacted and applicants may be interviewed.

4 Program Delivery Models: Grade 9 & 10 students will be at the golf course three times a week during the year (except for 6 weeks during December and January). On the other days of the week and during the months of December and January, the students will be participating in fitness classes, mini sports units and leadership opportunities in which they will earn 4 additional credits. Some of the at school PE sessions may focus on the following topics: Mini sports units Fitness classes Leadership assignment Active health classes First Aid Sports Psychology Anatomy Physiology Nutrition Relevant Guest Speakers Training Principles Golf Fitness Post Secondary and Scholarship applications * Grade 11 & 12 students receive 4 credits towards graduation for their Monday Wednesday sessions at the Cordova Bay facilities. When we are aff the course (Thursday/Fridays and months of December and January) They will have a spare block during this time where they can further their golf practice and play, take an online course or use this as a study block. Currently the program runs during block 5 and the students are transported to the golf course on a school district bus. It is the student's/parent's responsibility to arrange for their own way home from the course. (The program delivery for CSI Golf has fluctuated in the past based on program registration). Golf Team Although CSI Golf is not directly linked to the school golf team all Grade 10 CSI Golf members will be required to try out for the school golf team. Only CSI golf members who are not earning 5 or more athletic related credits in a school year will actually be eligible to play on the golf team. Regardless of credit status all players will gain experience from the process of competing for a spot on the school golf team. Field Trip to Island Golf Courses Opportunities to play at different local golf courses may be arranged during the year, for Grade students. However, cost associated with these trips may have to be covered directly by each participant. Program fees do not cover these costs.
