The CDP is a graduate program. To be considered for admission to the program, applicants must complete three applications. The first is at CSUmentor.edu to the University s Graduate Admission application, the second is to the School s Master of Science degree program in Nutritional Science and, third to the Coordinated Dietetics Program. Each application has its own requirements and must be submitted independently. The CDP program is a 3-4 year program, depending on the graduate coursework and program. The student must complete the CDP program and earn a Master of Science in Nutritional Science degree prior to taking the Registration Exam for Dietitians. Students with a degree in Nutritional Science or Dietetics may complete the program in less than 3 years based upon evaluation of their transcripts and acceptance into the program. The complete CDP application is due and must be postmarked by Friday, January 29, Please send to the School of Kinesiology and Nutritional Science, Physical Education Building, PE 206, and mark 2016 CDP. Only complete applications will be accepted, which include the 1) CDP informational sheet, 2) Statement Letter, 3) Resume, 4) Two Recommendation forms and 5) original transcripts. Late applications will not be accepted nor any changes or amendments after the due date. Accepted applicants are notified in Spring Also, note that you need to show how you will complete all CDP prerequisites prior to Fall 2016 on the application form. In addition, when calculating overall GPA, only undergraduate coursework may be counted. 4 1. Submit Statement Letter. This is an opportunity for you to introduce yourself to the CDP Selection and Review Committee. Letter must be typed and limited to two (2) pages, double spaced. It should include a statement explaining: a. Your reasons for entering the field of dietetics. b. Your feelings about the field of dietetics. c. Your short and long range professional goals. d. Your reasons for applying to the CSULA Coordinated Dietetics Program. e. Other pertinent information you may feel would be beneficial to the committee in making a decision on your admission to the program. This may include your experience as a volunteer, having a relative in the profession, or long standing interest in nutrition. 2. Resume, one page typed. Include your volunteer experiences in nutrition, food service and any related activities. 3. Include a minimum of two recommendation forms. At least one recommendation must be from an educator (not Cal State LA Nutritional Science or Food Science & Technology faculty). Ask each evaluator to fill out recommendation form, place in a sealed envelope and sign across the bottom. Include completed recommendation envelopes with the application packet. 4. The interview schedule will be communicated to the applicants. NOTE: All applications must be complete at the time of submission, including the information sheet, statement, resume, recommendations, and official transcripts. Incomplete applications will not be accepted. Failure to submit a complete application will result in the immediate disqualification of the application. *If you have been a matriculated student at CSULA for at least one (1) quarter, official transcripts from CSULA are not needed. Please include unofficial GET transcripts. However, if you are a graduate of another university, please include official transcripts from that university, showing awarding of your degree and successful completion of prerequisite courses.
