Mental Health Supporting mental health service users in both the community and in hospital is dependent on giving care workers and clinicians access to the most up-to-date information. High quality care is also dependent on the ability to create person-centred assessments and care plans which are underpinned by evidence and best practice. Belfast Health & Social Care Trust Civica is working with Belfast Health & Social Care Trust, one of the UK s largest Trusts, to enhance the provision of community health and social care built upon the foundation of an Electronic Patient Record (EPR) and Case Management system designed to improve services and efficiency through integrated working. The Belfast Trust delivers health and social care services to 340,000 people across the city as well as specialist services to all of Northern Ireland. When fully deployed the Paris EPR system will support over 5,000 clinical staff and will be accessed from over 120 locations to support the joined-up delivery of safe, timely and high quality care. The system underpins the Trust s focus on patient-centred services which can be delivered from central locations or in the community as appropriate, including the seamless sharing of information to support Belfast s modernisation agenda and to enhance decision making, risk management and cost savings Paris EPR and Case Management 03
3 Community Health Adult Social Care Community healthcare is becoming more important due to a number of factors including earlier discharge from hospital and of course, the population requiring ever increasing, age related care. Paris enables community staff to work within one area of Paris, namely staff diary. Within this application, the healthcare provider can view upcoming contacts, book new contacts and record their entries into the clinical record. Assessments, reviews, care plans and progress notes can be recorded, updated and viewed from one place allowing quick and easy access to the care record. ParisMobile enables peripatetic workers access to the person s clinical record at any time during their working day. > Supports multiple referral intake and care processes models > Fully supports the community information data set > Personal diary and group diaries > Planned visits easily booked into staff diaries > Supports outcome measures > Toolkit that allows the trust to design and implement their clinical documents on the system > Integrated letters and reporting > Support for e-prescribing > Robust and flexible security model > Clinic booking > Spell checker with medical dictionary. The landscape for adult social care continues to change rapidly as organisations strive to find creative ways of making their budget go further. Personalisation, Self-Directed Support (SDS) and Personal Budgets are enablers for this but their implementation poses a variety of challenges. New business processes and roles are evolving and the systems that support these must respond to this change flexibly and quickly. Paris Child Health has proved an efficient and effective tool for the planning of vaccination and immunisation services within Cheshire. The flexibility of reporting from the system has meant that we can produce detailed data within a short time period and respond to the changing demands of modern health care provision. 4 Prescribing Other Capabilities Research has shown that the application of electronic prescribing and associated on-line decision support can reduce serious adverse drug events by 55% (Bates et al, JAMA 1988). ParisPrescribing provides electronic prescribing with integrated decision support, provided by First Databank. Civica s solution allows a trust to assess the cost of prescribed drugs against their effectiveness for individual patients, by combining prescription data with patient demographics and clinical parameters such as diagnoses, admissions, length of stay and patient outcomes. > e-prescribing and administration removes paper medication records, improving patient safety > Safer and more accurate prescribing, resulting in fewer prescribing errors > Control over which drugs can be prescribed and their cost implications > Full audit trail of prescribed drugs, supporting compliance with local governance standards > Integrated decision support, reducing prescribing errors and facilitating evidence-based best practice with respect to medicines. Hosting and Managed Services > Civica provides a comprehensive hosting and managed service based on secure, N3 compliant data centres with full disaster recovery capabilities, to support Cloud-based delivery. Interoperability Mobile Working ParisConnect introduces the ability for Paris to interoperate with other systems by providing access to the Paris data layer, managed through ParisConnect web services. ParisConnect enables internal or external event-driven processes to be automated, thus removing the need for manual data entry. This results in higher data quality levels and reduced costs which ultimately improve the level of care offered to patients. Working directly with the electronic care record whilst out in the community is proven to save time and reduce risk. Many Civica customers already benefit from using Paris in their communities but, to-date, this has been constrained by the availability of a mobile network signal. Now, ParisMobile offers a complementary solution that is available regardless of network availability. Patient/client records are securely synchronised and stored on a mobile device. This gives staff the confidence to know that, wherever their visit takes them, they ll have an up-to-date care record available and the ability to perform their key tasks electronically. Nurses now have consistently up-to-date information for each patient and spend less time and money on travel. The initiative has freed up an extra 1,300 working hours per month across the department to spend giving frontline care. In addition, patients say they feel more involved in their own care and satisfied with the service.
