University Hospitals Case Medical Center is building upon a legacy of innovation that began nearly 150 years ago and continues today through its Department of Otolaryngology. This spirit of innovation and medical discovery is demonstrated today in a number of programs and initiatives that are helping forge medicine s future: In conjunction with Case Western Reserve University School of Medicine, we are developing a tissue-engineered trachea using cartilage cell growth technology to potentially replace defects in the airway. Our research team, in collaboration with Case Western Reserve University School of Medicine, has identified proteins that help explain deafness in Usher s syndrome and Meniere s disease. The Cochlear Implant Team has defined parameters for restoring hearing using cochlear implants in disorders including far advanced otosclerosis and auto-immune hearing loss. The launch of The Harrington Project for Discovery & Development, a $250 million investment in drug discovery and commercialization that aligns a not-for-profit entity, Harrington Discovery Institute at University Hospitals Case Medical Center, with a for-profit company, BioMotiv. These and other UH Case Medical Center advances are poised to benefit the lives of patients affected by ear, nose and throat disorders for generations to come. Patients of the UH Ear, Nose & Throat Institute at UH Case Medical Center benefit from the outstanding care of its specialists and staff and the pioneering treatment options they employ. UH Case Medical Center is one of 20 sites investigating a novel surgical device treatment for obstructive sleep apnea in patients who cannot tolerate or who struggle with continuous positive airway pressure. The phase 3 STAR trial is designed to test the efficacy of the Inspire Upper Airway Stimulation therapy, in which respiratory effort triggers an implantable hypoglossal nerve stimulator. Implantation of the device takes approximately 90 minutes. Once implanted, patients can control therapy start and stop times using a handheld device. Seven patients were enrolled in the trial through UH Case Medical Center. The trial is being run under the direction of Jonathan Baskin, MD, Chief, Otolaryngology Head and Neck Surgery, Facial Plastic and Reconstructive Drs. Kingman Strohl and Jonathan Baskin Surgery, Louis Stokes Cleveland VA Medical Center; Attending Physician, UH Case Medical Center; and, Case Western Reserve University School of Medicine and Case Western Reserve University School of Engineering, and Kingman Strohl, MD, Acting Chief, Division of Pulmonary, Critical Care and Sleep Medicine, Department of Medicine, UH Case Medical Center; and Professor of Medicine and Professor of Physiology and Biophysics, Case Western Reserve University School of Medicine. The department is now offering a new treatment for cerebrospinal fluid leaks that result from brain hernias that come out of the intracranial cavity into the ear. Treatment of this condition formerly required a middle fossa craniotomy, with a typical three- or four-day hospital stay and a higher risk of complications. With the newer transmastoid extradural-intracranial, or TMEDIC, approach, herniations are repaired using a combination of cartilage, Drs. Pierre Lavertu, Rod Rezaee and Chad Zender fascia and tissue sealant without opening the skull. Under the direction of Maroun Semaan, MD, Co-Director, UH Ear, Nose & Throat Institute, Ear, Hearing & Balance Center and Cochlear Implant Surgery; and, Department of Otolaryngology Head and Neck Surgery, Case Western Reserve University School of Medicine, more than 40 procedures have been performed with positive results, including shorter hospital stays and no risk of brain retraction. The piezoelectric BoneScalpel, an innovative technology that uses low-frequency ultrasonic technology, is being used to more safely harvest bone tissue for head and neck reconstruction. Under the direction of Chad Zender, MD, FACS, Otolaryngology, UH Case Medical Center; and Assistant Professor, Otolaryngology Head and Neck Surgery, Case Western Reserve University School of Medicine, the department has used the scalpel in numerous patients. The department is hiring an additional microvascular surgeon to meet growing demand. The cochlear implant program at UH Case Medical Center and UH Rainbow Babies & Children s Hospital added a second surgeon, Dr. Semaan, from the House Ear Institute in Los Angeles. The UH program, which completed its 700th cochlear implant in 2012, emphasizes comprehensive evaluation and training and ongoing support services for patients and their families. In addition, the department has added cochlear implant services at the Louis Stokes Cleveland VA Medical Center on the Wade Park Campus, making this the only place in the region where veterans can receive cochlear implants through the VA. Department Highlights: Department of Otolaryngology Head and Neck Cancer The Case Western Reserve University School of Medicine Department of Otolaryngology s residency program offers three key advantages: 1 exposure to the surgical and medical care of patients in four different clinical settings with diverse patient populations UH Case Medical Center, a large, tertiary care teaching hospital; UH Rainbow Babies & Children s Hospital; the Wade Park Campus of the Louis Stokes Cleveland VA Medical Center; and MetroHealth Hospital, a county hospital; 2 a tradition of hands-on, in-depth surgical experience with access to all subspecialty areas; and 3 a success rate that includes more than two-thirds of recent graduates accepted into their top choices for a prestigious fellowship program. The residency program is highly competitive, with more than 320 applications submitted for three positions. Approximately 80 percent of students who choose otolaryngology residencies apply to the school s program. Dr. Brian McDermott Medical students, residents and fellows benefit from hands-on instruction from these leading medical experts and have access to a broad spectrum of programs from which to pursue. This multidimensional approach and dedication to education by UH Case Medical Center and Case Western Reserve University School of Medicine play a major role in further enhancing our rich tradition of developing medicine s future leaders. Every fall, Cliff A. Megerian, MD, FACS, hosts the Temporal Bone Surgical Course. Doctors from throughout the country attend the event at the UH Temporal Bone Lab, where delicate head and neck surgeries are performed on specimens. In addition, knowledge of temporal bone anatomy and surgical skills are developed in the Temporal Bone Surgical Anatomy Laboratory. Each resident is provided with a set of basic instruments and is encouraged to spend time drilling regularly throughout the year. An intensive course is given each year for 10 weeks, during which each resident dissects several temporal bones with careful supervision. In conjunction with the temporal bone anatomy course, a review of the endoscopic anatomy of the paranasal sinuses is given by the full-time faculty. This intensive review using hands-on cadaver dissections is an essential element in learning the popular and successful technique of endoscopic sinus surgery. The Department of Otolaryngology Head and Neck Surgery at Case Western Reserve University School of Medicine in conjunction with UH Case Medical Center and the UH Seidman Cancer Center, is proud to announce the establishment of a one-year Head & Neck Surgery and Reconstruction fellowship beginning July This fellowship offers advanced training in more than 120 microvascular free tissue transfers per year, endoscopic and open skull base surgery, minimally invasive head and neck surgery, transoral laser and transoral robotic surgery, and sentinel node mapping for head and neck melanoma. Because of increased patient demand and institutional support for expansion, the Department of Otolaryngology Head and Neck Surgery is seeking to add the following full-time academic faculty positions: Rhinologist-Allergy Surgeon (fellowship trained), Otologist/ Neurotologist (fellowship trained), Pediatric Otolaryngologist (fellowship trained), Head and Neck Surgeon Scientist with a focus on squamous cell carcinoma research, General Otolaryngologist with an interest or additional training in sleep medicine, and Laryngologist trained in tracheotomy care, voice surgery and cartilage research (fellowship trained). 12 University Hospitals Case Medical Center UHhospitals.org/ent Department of Otolaryngology Head & Neck Surgery 13

Research Dr. Kumar Alagramam UH Case Medical Center s physician-scientists are leaders in their respective fields, and their ongoing clinical research programs are at the leading edge of medical progress. A strong emphasis on translational, or bench-to-bedside, research means that new and innovative treatments and technologies transfer more rapidly from the research laboratory to actual patient care. James Dennis, PhD,, Case Western Reserve University School of Medicine, Harvey Tucker, MD, Professor at the School of Medicine, and Mark Weidenbecher, MD, Research Associate at the School of Medicine, are involved in research of tissue engineered auricular cartilage for laryngotracheal reconstruction, which has received $1.9 million of funding from the National Institutes of Health. Kumar Alagramam, PhD, is the primary investigator in a five-year NIH grant in the continued study of Usher syndrome after finding the gene for one Usher type, and will work closely with the Case Western Reserve University School of Medicine Department of Ophthalmology and Visual Sciences. Brian McDermott, PhD, Assistant Professor, Otolaryngology, Case Western Reserve University School of Medicine, is the primary investigator conducting research using a $1.9 million grant from the NIH for a five-year program to elucidate the molecular mechanisms of hearing and deafness. This clinical research examines the mechanisms of development of the hair bundle, a mechanosensitive organelle that translates sound into an electrical response that is decoded by the brain to permit hearing. Dr. McDermott s research team works in the Zebrafish Laboratory, which was built with a $500,000 startup grant from the School of Medicine. Qing Yin Zheng, MD,, Otolaryngology, Case Western Reserve University School of Medicine, member of the Case Comprehensive Cancer Center, is in the third year of a five-year NIH-funded study that has the potential to uncover new therapeutic approaches to treat hearing loss in Usher syndrome, which accounts for nearly 50 percent of the deaf-blind population. This $1.66 million grant is Dr. Zheng s fourth major NIH grant for research involving Usher syndrome. Nicole Maronian, MD, Director, Voice & Swallowing Center, UH Ear, Nose & Throat Institute; and, Case Western Reserve University School of Medicine, is working closely with epilepsy specialists at UH Case Medical Center and in Northeast Ohio on a program that involves implanting a vagal nerve stimulator into the patient s neck. By stimulating the vagal nerve, epilepsy attacks are reduced. Pierre Lavertu, MD, chief of the treatment of head and neck cancers in coordination with the award-winning UH Seidman Cancer Center, has been performing percutaneous tracheotomies for almost a decade. The procedure is done under bronchoscopic guidance. The advantages of this approach versus the traditional technique are that it can be done rapidly, often within five to 10 minutes, does not require transportation of patients to the operating room, and does not require operating room availability. The literature does not show any increase in complications compared with traditional tracheotomy. All National Institutes of Health (NIH) funding for basic and clinical research is awarded to the School of Medicine at Case Western Reserve University. 10 University Hospitals Case Medical Center One of America s First Academic Medical Centers University Hospitals Case Medical Center has been at the forefront of patient care, physician education and medical research for nearly 150 years. Founded after the Civil War as a place of healing, the institution embraced a broader role as medical science advanced. By the turn of the twentieth century, a modern general hospital was enhanced by specialty facilities for women, children and infants. An affiliation with the medical school of what was then Western Reserve University today Case Western Reserve University formed one of the first true academic medical centers in the United States, as University Hospitals of Cleveland joined Johns Hopkins Hospital and Massachusetts General in combining the training of young physicians and the pursuit of laboratory research with delivery of the most advanced patient care. In 1931, new facilities brought University Hospitals and the School of Medicine onto the same medical campus in University Circle. UH Case Medical Center was selected by a multidisciplinary committee of health care quality and patient safety experts. Awarded annually, the prize is supported by a grant from the McKesson Corp., a national health care services company based in San Francisco. The criteria for the 2012 award include the demonstration of an organizational commitment to and progress in achieving the Institute of Medicine s six quality aims safety, patient-centeredness, effectiveness, efficiency, timeliness and equity. 12 Harrington Discovery Institute at University Hospitals Case Medical Center Harrington Discovery Institute is led by Director Jonathan Stamler, MD. Dr. Stamler holds the Robert S. and Sylvia K. Reitman Family Foundation Distinguished Chair in Cardiovascular Innovation at both University Hospitals and Case Western Reserve University School of Medicine. He also serves as Director for the Institute for Transformative Molecular Medicine, Professor of Medicine and Professor of Biochemistry at Case Western Reserve University School of Medicine. Mukesh K. Jain, MD, is the Chairman of the Executive Committee for Harrington Discovery Institute.
