2 The Center for Prostate Cancer Leaders in Prostate Cancer Treatment and Research The Center for Prostate Cancer at the North Shore-LIJ Health System is the premier referral center for prostate cancer care in the region. There are many ways prostate cancer may be treated and our expert team and advanced services will educate patients on their best options and offer the highest quality care. And because we are part of the North Shore-LIJ Health System, our patients have the support of full-service hospitals to care for the whole patient. The Center for Prostate Cancer provides multidisciplinary care centered on the individual needs of each patient. The Center is one of a select number of specialized facilities in the Northeast, and the only one on Long Island dedicated to prostate cancer patients. Our specialists are preserving function and quality of life for patients and are spearheading pioneering research programs in collaboration with the newly founded Hofstra North Shore-LIJ School of Medicine. Our clinicians and researchers are actively collaborating in an effort to develop personalized treatment options based on the specific characteristics of each patient s tumor. 1
3 The Patient Experience The news of prostate cancer can be devastating. Fortunately, prostate cancer is treatable and when found early, curable. A key challenge of effective treatment is making sure the patient receives a thorough and timely evaluation of his disease, so he can choose the best course of treatment. Each patient is evaluated, counseled, and provided with a definitive course of treatment and supportive care within days of referral to The Center. All new patients are seen by key members of the multidisciplinary treatment team in a comfortable and caring office setting. This initial evaluation serves as the gateway for patients entering The Center, and as the cornerstone for all subsequent care. This initial evaluation transforms the potentially cumbersome evaluation process by multiple providers into an efficient, satisfying experience from the perspective of patients and providers alike. Our mission is to provide: an environment of excellence a reassuring, easy-to-navigate patient experience a programmatic environment which nurtures providers ability to offer the finest care Innovative Approaches Unique approaches to prostate cancer treatment offered here include: Advanced diagnostic techniques to determine which men are candidates for active surveillance and which need aggressive treatment Minimally invasive surgical techniques that may permit treatment to lessen the recovery and hospitalization Latest radiation technology and treatments that provide high doses of radiation safely and precisely Multidisciplinary patient care team will work with you to develop a treatment plan that balances potential side effects and benefits of treatment. 4 Organization and Clinical Care Initial patient evaluation The initial workup, including images from our radiologists and lab work, is coordinated to develop a plan for subsequent treatment. Coordinated multidisciplinary evaluation of patients by Urology, Radiation Oncology and Medical Oncology may take place the same day as the initial visit. All patients receive an orientation and one-on-one educational/social support session. All cases are reviewed by the multidisciplinary team to develop a consensus-based clinical action plan, including potential eligibility for participation in clinical trials. Patients leave the office with treatment recommendations, a plan, and personalized educational materials including contact numbers and appointment times with specific team members. Any other evaluations or tests are scheduled. Treatment and surveillance management is performed by each participating service. However, multidisciplinary prostate oncology team meetings ensure overall coordination of each patient s entire treatment phase. This review is designed to make sure each patient reaches treatment milestones, including: the start of treatment and baseline supportive care measures routine supportive care patient compliance with treatment (and/or clinical trial participation) supportive care interventions if necessary successful completion of active therapy and scheduling of immediate post-treatment supportive care, appointments/tests, etc. Our teams of specialists: New patients are generally seen within two weeks of referral. At their first appointment, patients will meet the genitourinary cancer specialists who will be involved in their care. These specialists may include: Urologists and surgical oncologists - experienced in the latest prostate cancer surgery approaches, including laparoscopic and robotic surgery Radiation Oncologists - who specialize in delivering the optimal therapeutic dose while protecting healthy tissue Medical Oncologists - to prescribe the most effective treatments, including hormone therapy, chemotherapy, immunotherapy and vaccines In addition, patients may also see: Pathologists - specializing in urologic cancers Radiologists - specializing in genitourinary cancer imaging Oncology nurses and nurse practitioners - who specialize in cancer care Oncology psychiatrists and social workers - experienced in helping with the cognitive side effects of cancer and treatment, as well the emotional and spiritual side of the cancer experience A nutritionist - to help with questions about diet and nutrition during treatment 3

Patients typically are classified by disease risk, based on the biology of their tumor, and how the Center for Prostate Cancer can tailor treatment based on these specifics of the patient s disease. The most advanced surgical treatment of prostate cancer The internationally renowned surgeons at the Center for Prostate Cancer have unique expertise in a number of advanced surgical techniques including use of the DiVinci Robot. A more precise approach to radiation therapy The Department of Radiation Medicine has internationally recognized radiation oncologists who have pioneered the latest techniques in radiation delivery, including seed implantation, external radiation therapy, and robotic stereotactic radiation therapy. Advanced techniques such as intensity modulated radiation therapy are used and the prostate is targeted precisely by placing markers in the prostate and by obtaining cone-beam CT scans while on the radiation treatment machine. Innovative systemic therapies and investigative approaches Prostate cancer becomes systemic when it spreads beyond the actual prostate. In such cases, the disease is usually present in the lymph nodes and bones, and symptoms can be wide-ranging. The Center for Prostate Cancer has many tools to treat the systemic disease, and each is tailored to meet the patient s individual needs. For prostate cancer limited to the prostate, The Center for Prostate Cancer is currently investigating the use of robotic stereotactic radiation therapy. This promising form of radiation therapy has the ability to deliver a very precise and high dose of radiation to the prostate in a non-invasive way and in a shorter treatment interval than traditional radiation therapy. northshorelij.com/prostate 4
6 Specialized Expertise and Technology Our prostate pathologists are amongst the world leaders in diagnosing and evaluating prostate tissue. The Center for Prostate Cancer uses advanced technologies to provide critical information that is used in treatment planning and assessing a patient s response to treatment, including: Circulating tumor cells The role of evaluating cancer cells in the blood is new and may better dictate treatment. The Center for Prostate Cancer will routinely obtain this information to help guide the best treatment approach and for the evaluation of each treatment. Advanced imaging The Center is one of a handful of centers testing and refining the use of MRI techniques to gauge patient disease and response. Our team of radiologists and physicians are pioneering the use of imaging techniques for prostate cancer treatment, including the use of computed tomography (CT), dynamic magnetic resonance imaging (MRI), 3 Tesla magnets and positron emission tomography combined with computed tomography (PET-CT) to stage and evaluate early treatment response in patients.
