I am happy to share this 2010 Community Report with you. In it you ll learn more about the high quality care that has earned us 16 designations and certifications from some of the most respected names in healthcare. Our goal is to provide quality healthcare to every person we treat. Blake Medical Center has a tradition of excellence and is frequently recognized by third parties. Blake boasts an Accredited Chest Pain Center with PCI by the Society of Chest Pain Centers; Blake s Cancer Program is accredited with commendation by the American College of Surgeon s Commission on Cancer; a designated AMI (eart Attack) Center, by the Joint Commission; Certified Advanced Primary Stroke Center also by the Joint Commission. In addition, just this past year The Joint Commission awarded Blake Medical Center Disease-Specific Care Certification for Knee and ip Replacement. Blake is one of only seven hospitals in the State of Florida to earn this distinction. To earn this distinction, a disease management program undergoes an extensive, on-site evaluation by a Joint Commission reviewer every two years. These designations mean that we adhere to the highest standards of care. Blake is also proud to be named one of the 100 Top ospitals in the nation nine times. I know that without our team of dedicated professionals, who consistently go above and beyond for our patients, none of the above mentioned awards would be possible. It s because of people like our 2010 Doctor of the Year, Dr. Alessandro Golino and our 2010 Nurse of the Year Joelle Thomas, who were recognized by patients and peers for their dedication and pursuit of excellence, that we remain leaders in healthcare for our community. Their hard work, dedication and patient care lead the way and set the example for others in the healthcare field. The recent remodel of our Rehabilitation Center and our plans to be designated a Level II Trauma Center by October 2011 show our commitment to continually elevating the level of care for our community. Indigent Patient Education Blake Medical Center is committed to helping uninsured patients qualify for local and state health benefit programs. Each ospital offers onsite benefit education and Medicaid enrollment assistance. Step Up for Students Since 2005, CA has redirected corporate income tax to the Step Up for Students program. Through this gift, low income students received a K-12 scholarship enabling them to receive a quality education tailored for their specific needs. Through this program, children who qualify for the National School Lunch Program can receive scholarships. Qualifying parents can use these grants to send their child to the public or private school that best meets their child s needs. Blake is proud to have the only CARF Accredited Inpatient Rehabilitation Center in Manatee County. The Cancer Care program at Blake has been Accredited by the American College of Surgeons Commission on Cancer since Blake is Joint Commission Accredited, a Certified Chest Pain Center with Percutaneous Coronary Intervention, and an Accredited Primary Stroke Center. Blake Medical Center also earned the Joint Commission Accreditation for Acute Myocardial Infarction (eart Attack), ip Replacement and Knee Replacement and has been awarded the Solucient Top 100 ospital in America recognition nine times! Accredited Chest Pain Center w/pci urricane Preparedness CA West Florida offers one of the most comprehensive hurricane preparedness plans to ensure that our patients and our staff who care for them are safe before, during and after a storm. Our Division has arrangements and contracts in place with vendors from around the state and nation to provide support, from on-site security if needed to medical supplies, additional generators, food, fuel, emergency transportation, building supplies and construction crews who can be dispatched immediately following the storm to make needed repairs to keep our hospitals operational. Emergency communications, several days worth of food and supplies all arrive at each facility well in advance of a storm, to ensure the hospital is self-sustaining for several days. Transfer processes and plans are in place with our network of hospitals from around the country, should patients need to be relocated. The h2u program is designed to bring members the latest in health and wellness information as well as help individuals take control of their health care needs, including maintaining a healthy lifestyle, disease prevention, immunizations, self-care and injury prevention. Cardiac/Pulmonary Education: Lunch-n-Learn Sponsored by the eart Institute and the Cardiopulmonary Rehabilitation Department, this program is designed for patients currently enrolled in Phase II or Phase III of Cardiopulmonary Rehabilitation. Additionally, this program is of interest to eart Institute Alumni, their families, and those interested in heart healthy living. The group meets every Wednesday at Blake Medical Center in the Dolphin/Pelican Room from 5:30 pm - 6:30 pm. For more information, call (941) Lymphedema Support Group This group provides education, information, and support for people with lymphedema and for those who are at risk for lymphedema. The group meets quarterly at Blake Medical Center. For more information call (941) Stroke Support Group This community based Stroke Support Group provides information, education, and support to stroke survivors, families/caregivers, and friends. Members may be at any stage of recovery to participate. Meetings are held monthly at Blake Medical Center in the Dolphin/ Pelican Room. For more information, call (941) Screenings & Lectures Blake Medical Center is pleased to offer free health information lectures on a variety of topics throughout the year. In addition, Blake provides numerous free screenings to the public including cholesterol, blood pressure, bone density, and many more. Many of the initiatives we implement serve as national models for the healthcare industry today. hcare Electronic ealth Record (ER) The West Florida Division has invested heavily in its Electronic ealth Record (ER) initiative, which kicks off in March Throughout 2009, many projects related to the development and implementation ER were rolled out in preparation. With ER, access to patient health information is simplified and patient safety and quality improved. Our hospitals have brought numerous clinical components for the ER on-line through major investments in technology. hcare ub As more and more physician practices begin using Electronic Medical Record (EMR) systems, there is an increased need to integrate data from a variety of clinical systems, including hospital systems. Linking physician practices to our hospital data bases is now being rolled out through the hcare ub interface. This allows physician offices real-time access to patient clinical data from the hospital. Effective clinical data exchange can provide necessary information to physicians to make timely decisions and reduce duplicate testing and unnecessary hospitalizations. Picture Archiving and Communications System (PACS) The PACS system immediately digitizes images such as Xrays, and other scans including CT, MRI and PET, so they can be stored and electronically viewed on screens anywhere. This enables doctors and other health care professionals to access the images, even remotely, and to compare new images with previous images at the touch of a button. Voice Recognition To improve report turnaround time and patient care, our hospitals and imaging centers use Voice Recognition which allows Radiologists to report their interpretation of a study directly from the PACS Imaging system by speaking instead of dictating. The physician then reviews the report on screen, makes any adjustments, signs the report and it posts immediately for others to view. Report turnaround time has been greatly reduced to an average of just over 30 minutes. Everything was great. Thanks to everyone doing their jobs so efficiently, I m still here to have fun. 11 B l a k e M e d i c a l C e n t e r MUSE Blake Medical Center has installed this system which digitizes EKG tracings so physicians can review, diagnose and treat heart patients within seconds. Electronic Medication Bar Coding System (emar) We led a national effort to reduce medication errors by placing this system in all of our hospitals. Bar codes on medications are scanned at the patient s bedside, providing our clinical staff with an effective tool to ensure that the right patient receives the right medication at the right time through the right route. Unprecedented Flu Season Preparation The National Patient Safety Foundation has recognized all CA employees for their efforts in supporting CA s flu immunization policy. Whatever the 2009/2010 flu season brings, CA employees have prepared well. Our hospitals stepped up efforts to encourage employees to take advantage of our free flu vaccine program and practice good hygiene. More than 97 percent of our employees have received the flu vaccine, an amazing accomplishment which helps protect our patients as well as our employees and their families. Additionally, many of our hospitals actively participate in community outreach efforts to educate the public about the importance of seasonal flu vaccine and the 1N1 vaccine. Methicillin-resistant Staphylococcus aureus (MRSA) CA continues to be a leader in the fight to stop the spread of MRSA, and was invited to participate as a partner organization in the REDUCE MRSA Clinical Trial, with the CDC Prevention Epicenters, CDC Division of ealthcare Quality Promotion and the Agency for ealthcare Quality and Research (ARQ). CA was selected due to its commitment to infection prevention, the CA A, B, Cs (MRSA) program and our standardization of specific lab data. The goal of the study is to conduct a cluster randomized trial of hospitals to compare uniform versus target MRSA decolonization as a means to reduce healthcare-associated MRSA infection. Locally, eight CA West Florida Division hospitals are participating. Locally, we continue to be very involved in educational outreach to prevent the spread of community acquired MRSA. The national Clean ands Are Cool ands campaign, featuring teen star Mitchel Musso (annah Montana show), was launched here in 2008 and participation extended throughout 2009, with our partner schools. The goal is to educate children on the prevention of MRSA, through local presentations and a tool kit for parents and teachers available on our hospital websites. 9

12 OurCommitment To Excellence O u r C o m m i t m e n t T o E x c e l l e n c e SPECIALIZED CARE Emergenc y Excellence ighly skilled emergency physicians and nurses combined with best practices and the latest technology benefitted close to 30,000 Manatee County residents and visitors last year. Protocol driven care that can begin in the field through direct communications with local EMS teams, and meeting stringent standards to earn Chest Pain Center with PCI Accreditation, Acute Myocardial Infraction (eart Attack) Certification and Advanced Primary Stroke Center Certification by the Joint Commission are all part of our commitment to improve quality in our busy emergency department. Our strong focus on enhancing care and improving ER processes has also dramatically reduced wait times to below national averages. In fact, we re so successful, current ER wait times are now posted on our hospital website, a billboard on Cortez Road West, and by text messaging: ER to e a r t & Va s c u l a r C a r e The eart Institute at Blake Medical Center provides life-saving technology and the latest minimally invasive techniques in the detection and treatment of cardiac and vascular conditions. Blake s eart Institute, in partnership with our Emergency Department, has been recognized as an Accredited Chest Pain Center with Percutaneous Coronary Intervention, Accredited Acute Myocardial Infarction Center, a 100 Top Cardiovascular ospital by Solucient (now Thompson Reuters), and a Blue Distinction for Cardiac Care award winner by Blue Cross Blue Shield. By partnering with our local EMS, Blake routinely beats the national STEMI benchmark of 90 minutes, dramatically improving the survival and resulting quality of life of our patients experiencing a heart attack. The latest surgical techniques for minimally invasive mitral valve repair and atrial fibrillation treatments provide safer alternatives for patients suffering from these conditions, improving their quality of life and reducing their risk for other serious complications. Our Cardiac Cath and EP labs offer state-of-the-art technology and treatments to invasive and noninvasive cardiac procedures. My life s fulfillment is being with my children and grandchildren. 13 B l a k e M e d i c a l C e n t e r Neurosciences & Stroke Care Our hospital has earned Joint Commission Primary Stroke Center designation, the gold standard for ensuring the highest level of care is immediately available. In 2009, Blake was also awarded the Gold Plus Performance Award from the American Stroke Association. Cancer Care Cancer is the second leading cause of death in this country and our hospital is committed to meet and exceed the many standards required for the American College of Surgeon s Commission on Cancer (ACoS CoC). To become accredited, our hospital has committed to regular meetings of the Cancer team, which includes surgeons, medical oncologists, radiation oncologists, pathologists, radiologist and hospital clinical and support staff to proactively review patient cases. Each day new lives are touched by cancer. My hope is to improve the journey for each patient and their family by providing compassion, support, education and individualized attention. I strive to be a comfort to them and to connect them with resources that can help every step of the way. Our hospital has earned the prestigious cancer program designation from the ACoS CoC. We continue to ensure that patients can receive the care they need, right in our own community, close to family and friends. Our Cancer Resource Room, located in suite 3400 of our Medical Arts Building, offers free lectures, exercise classes, and a place for research and quiet reflection for cancer patients and their families. Advanced systems from scheduling to pre-op and discharge planning ensure that patients receive the care they need. Minimally invasive procedures include Single Incision Laparoscopic Surgery (SILS) for a variety of general surgery procedures, like gall bladder removal, and incision-less surgery for the treatment of Gastroesophageal Reflux Disease (GERD). Orthopedic and Spine Surgery Patients needing advanced treatments for orthopedic and spine issues can find high quality care in a beautiful hotel-like setting at The Joint Care Center and the Spine Care Center at Blake Medical Center. Advanced procedures including minimally invasive knee and hip resurfacing, as well as partial knee resurfacing, are available to relieve pain and help patients return to normal activities. Our surgeons are committed to performing the latest innovations in their field, and patients benefit from the experience of a panel of orthopedic experts, individualized treatment plans and a high caliber of care. The Spine Care Center at Blake Medical Center provides advanced treatment protocols with a warm and caring environment to improve outcomes and speed recovery. The team includes pain management professionals, diagnostic-imaging specialists, neurosurgeons, physical medicine and rehabilitation specialists. I tried injections, pills, and physical therapy none of which relieved the pain of walking long distances or up and down stairs. I finally decided to go the next step. I had both knees replaced at the Joint Care Center at Blake Medical Center. After the surgery and rehabilitation, I realized that I should have done it years ago. The 28-bed Inpatient Rehabilitation Center is the only Acute Inpatient Rehab in Manatee County and is accredited by the Joint Commission and the Commission on Accreditation of Rehabilitation Facilities (CARF). Our Rehabilitation Center is committed to providing comprehensive rehabilitative care in a supportive atmosphere so that each individual can achieve the highest possible level of functional independence.
