3 Welcome At Henry Ford Macomb Hospitals, our goal is to help you become as independent as possible while achieving your physical, emotional, psychological, social and work-related potential. This may require learning or relearning skills. Your abilities, not disabilities, are emphasized. Our program is designed to focus on three areas: increasing your ability to perform everyday skills educating you and your family enhancing the quality of your future We also encourage participation from family and friends in the treatment process. As a member of the Henry Ford Health System, our mission is to improve human life through excellence in the science and art of health care and healing. Helping you heal in body, mind and soul is at the core of our mission. 1

4 About Your Rehabilitation Team Upon your arrival at Henry Ford Macomb, the rehabilitation team evaluates your physical, medical, psychological, vocational and social needs. Working with you and your family, they develop an individualized goal plan. The team then meets weekly to review your progress, revise goals and plan for your successful discharge from the program. Your involvement and the involvement of your family are critical to your progress. In light of this, a team member will always involve you in plan recommendations and changes. If you or a family member has questions about your care, please direct them to a team member or the program manager. The following are descriptions of team members and their roles. You and Your Family You and your family are the most important members of the team and are encouraged to participate in the program by communicating with team members and the physician, and by attending patient/ family conferences, as requested. Your family or caregiver will be scheduled to attend therapy and nursing instruction sessions to aid in understanding your individual needs and prepare for discharge. 2

5 Physiatrist The physiatrist is a physician who specializes in Physical Medicine and Rehabilitation and is generally responsible for the medical management of patients involved in the rehabilitation program. The physiatrist evaluates, advises and directs the total care of the patient and utilizes the expertise of a variety of professionals. Rehabilitation Nurse Our specially trained nurses provide support and care on a 24-hour basis. They also provide you and your family with education. Physical Therapist The physical therapist addresses areas such as strengthening, balance and coordination, wheelchair activities, walking and stair climbing. 3

6 Occupational Therapist The occupational therapist helps you use your current physical capabilities to become as independent as possible. Therapists focus on self-care skills, daily living activities, work activities, and perceptual and cognitive tasks. Speech/Language Pathologist The speech/language pathologist evaluates and treats problems with understanding and judgment, gesturing, speaking, reading, writing, hearing and swallowing. Recreational Therapist At our Clinton Township program, a recreational therapist is available to provide therapy in the form of leisure activities, focusing on memory skills, understanding and judgment, leisure interests, socialization and coordination functions. 4

7 Social Worker The social worker provides counseling and emotional support to you and your family. The social worker is also responsible for coordinating the discharge planning process, which may include outpatient and/or home care services, equipment, transportation and utilization of community resources. About Your Stay What to Bring Our goal is to help you achieve independence as soon as possible, and part of that work includes getting dressed every day. In addition to personal hygiene products, you should bring the following clothes: comfortable clothing, such as jogging suits or loose-fitting pants (5 sets) robe, pajamas undergarments (5 sets) regular or athletic shoes jacket or sweater Family members should make arrangements to do your personal laundry in a timely manner. 5

8 Daily Schedule A routine for your daily activities will be developed. Physical, speech and occupational therapy sessions are available seven days a week, and the minimum expectation for program participation is that you participate in therapy three hours per day, five days a week, or a minimum of 15 hours per week. Therapy schedules are established by the individual therapists and are posted on the units. Services available We have a variety of services available to you during your stay on the rehabilitation unit. Some services are provided on-site while others are provided on a referral basis. Examples 6

9 of services that are provided on-site include nutrition services, foreign language interpreter services, laboratory, pharmacy, radiology, neuro-psychological services, durable medical equipment (walkers, wheelchairs, hospital bed, non-custom orthotics, etc.) and assistive technology to modify your feeding utensils. If you feel you need a service that is not being provided, please ask your nurse, social worker, or therapist. Although we provide comprehensive rehabilitation services, some services may be provided by outside partners upon discharge. Examples of these services include, but are not limited to, custom prosthetics and orthotics, vocational rehabilitation, driver s assessment, sexual counseling and smoking cessation support. 7

10 Visiting Hours The Clinton Township hospital s weekday visiting hours are from 4 to 8:30 p.m. and weekend and holiday visiting hours are from 9 a.m. to 8:30 p.m. The Warren location s visiting hours are 10 a.m. to 8 p.m. daily. Exceptions to these hours are made for immediate family members; however, we ask that therapy sessions not be interrupted by visitors. Children visiting patients in the program must be supervised at all times. Smoking Restrictions Smoking is not allowed anywhere in the hospital or on hospital property. If you are having difficulty complying with the no smoking rule, please talk with your physician about receiving assistance during your stay. Safety Promoting a safe environment is a priority. Belts called transfer or gait belts are used with all patients to assist in safe walking or movements from sitting or standing positions. Your family can help with your safety by maintaining the use of safety devices put in place by staff; by requesting assistance 8

11 in moving you; and by reminding you not to grab anything that is not secure. Of course, we also want to make sure that patients and family members use the call light in the rooms to seek assistance whenever needed. Television, DVD/VCR and Stereo System A television, DVD/VCR and stereo system are available in the activity room to promote socialization with others. Individual TV and phone service are available for all hospitalized patients. We provide a wireless environment for our patients and visitors. Preparing for Your Discharge Family Outings As you approach discharge, we encourage you to take practice trips outside the hospital. These outings provide both you and your family an opportunity to discover some of the challenges that may occur after discharge, when staff members 9

12 are not available to help. The outings range from a short car ride to a trip home for the day. Following each outing, the staff will meet with you and your family to assess the outing and work to resolve any issues. Prior approval by your rehabilitation physician is required for these outings. Discharge Plans Plans for your discharge are actually put into place as soon as you are admitted to the Rehabilitation Units in Clinton Township or Warren. We encourage family support in preparing for your discharge. If further therapy, home care or special adaptive equipment is needed, arrangements will be made. You will be encouraged to make follow-up appointments with your personal physician. 10

13 Home Evaluation Depending on your needs, a home evaluation may be scheduled. A home evaluation is an assessment of your living environment. This evaluation will be scheduled during your stay in the Rehabilitation Program. The therapist meets you and your family at your home. While you are at home, the therapist will evaluate your mobility skills, address safety issues, and recommend proper equipment. Any questions or concerns will be addressed prior to discharge. Graduation Day Celebration While in our rehab program, you will re-learn the things necessary for your successful return home. You will demonstrate your success on Graduation Day. This will occur the day before you leave the hospital. On this day, you will perform the tasks necessary to show that you are ready to 11

14 return home, and have achieved your goals. Graduation Day is truly your day to show off your achievements! Follow-up Services Henry Ford Macomb Hospitals and Henry Ford Health System offer comprehensive outpatient services to help you continue your recovery. Recommendations and arrangements for many of these services will be made prior to your discharge. Outpatient Therapy Henry Ford Macomb Hospitals offers an extensive array of physical, occupational and speech therapy programs on an outpatient basis. Therapy is offered on the campuses of our Warren physical rehab center and Clinton Township hospital as well as several conveniently located outpatient sites throughout Macomb County. Home Care Henry Ford Home Health Care offers an in-home team of certified health care professionals providing skilled nursing; physical, occupational and speech therapies; home health aide services; medical social work services; and nutritional counseling. Medical Equipment Henry Ford Health Products provides medical equipment, including ambulatory aids and wheelchairs; hospital beds, bedside commodes or shower benches; and traditional products such as blood pressure and diabetes supplies. Stroke Support Group This group meets monthly at the Clinton Township hospital. It offers support and socialization to you and your family as you adjust to lifestyle changes brought on by a stroke. 12

15 Insurance & Payment Information Medicare (with a secondary insurance) pays for the cost of inpatient rehabilitation provided you meet criteria at admission and during your stay. This criteria includes a medical condition that necessitates acute care in an inpatient rehabilitation program, along with the ability to participate in at least 15 hours of therapy per week. The Medical Directors at each program will make the decision whether you meet criteria, but it is always subject to review from Medicare. Private insurance, Medicaid, and managed care benefits will be verified prior to admission and pre-authorization for admission will be obtained by hospital staff. This is not a guarantee of payment. Throughout your stay, the rehabilitation staff will work with your insurance provider to obtain continued authorization and we will provide information regarding co-pays and deductibles. Private payment and reduced cost arrangements for patients who qualify for financial assistance can be arranged through the billing department of Henry Ford Macomb Hospitals. Third party funding sources such as Worker s Compensation can be negotiated as needed. You are entitled to and will receive a written disclosure of your specific insurance information and coverage that is available to us at the time of your admission to the inpatient rehabilitation unit. Our admission referral coordinators will help you with this information.
