1 2011 COMMUNITY REPORT I want to thank EMS, Blake and Dr. Rivera for everything they did for me. They ve given me more time with my wife and family and that s so important to me. 2 Our Commitment To Excellence A Message From Our CEO: Blake Medical Center is proud to be a leading provider of high quality, affordable healthcare. We are committed to sustaining the community service values that have been at the core of our existence for over 35 years. I am happy to share this 2011 Community Report with you. In it you ll learn more about the award winning care that has earned us 17 designations and certifications from some of the most respected names in healthcare. Our goal is to provide quality healthcare to every person we treat. Blake Medical Center has a tradition of excellence and is frequently recognized by third parties. Blake boasts an Accredited Chest Pain Center with PCI, by the Society of Chest Pain Centers as well as a Certified AMI (eart Attack) Center and Primary Stroke Center, by The Joint Commission. Blake also has an Accredited Cancer Program, from the American College of Surgeon s Commission on Cancer and a Certified Inpatient Rehabilitation Center, from the Commission on Accreditation of Rehabilitation Facilities. We are a designated Blue Distinction Center for Knee and ip Replacement from BlueCross BlueShield and obtained a Disease-Specific Certification for Knee and ip Replacement from The Joint Commission. Blake Medical Center has recently achieved recognition as an Accredited Vascular Laboratory. This makes Blake Medical Center the only hospital in Manatee County with this accreditation by the Intersocietal Commission for the Accreditation of Vascular Laboratories (ICAVL). I know that without our team of dedicated professionals, who consistently go above and beyond for our patients, none of the above mentioned awards would be possible. It s because of people like our 2011 Doctor of the Year, Dr. Raj Rajan, and our 2011 Nurse of the Year, Billie Kyser, who were recognized by patients and peers for their dedication and pursuit of excellence, that we remain leaders in healthcare for our community. Their hard work and dedication to our patients lead the way and set the example for others in the healthcare field. We are pleased to welcome ealthcare America Medical Group, Pinnacle Medical Group, and Doctors of Manatee to the Blake family. This network of care will further stabilize access to primary care physicians in our community. Our long-term strategy is to create a seamless healthcare system leading to optimal care for our patients. We strive to maintain our reputation of excellence throughout the hospital by offering top-notch care in a comfortable environment. Blake is moving forward with efforts to open the first trauma program in Sarasota, Manatee, and DeSoto Counties. We plan to obtain provisional trauma status by this fall. We are currently undergoing renovations and expansions to accommodate these plans as well as show our continued commitment of elevating the level of care for our community. Indigent Patient Education The hospital is committed to helping uninsured patients qualify for local and state health benefit programs. Blake Medical Center offers onsite benefit education and Medicaid enrollment assistance. Step Up for Students Since 2005, CA has redirected $55,475,000 in corporate income tax to the Step Up for Students program. Through this gift, 14,351 low income students received a K-12 scholarship enabling them to receive a quality education tailored for their specific learning needs. Through this program, children who qualify for the National School Lunch Program can receive scholarships. Qualifying parents can use these grants to send their child to the public or private school that best meets their child s needs. Volunteering In 2010, Blake employees helped more than 100 local, state, and national organizations through gifts of time and money. From providing medical care on mission trips to helping give hope to children through programs such as Special Olympics and Riding Therapy, our staff gives back to our community, both in and outside of our facility. Going Green Many CA hospitals are involved with local green initiatives, like Blake Medical Center s Green Team which focuses on implementing environmentally friendly practices throughout the hospital. 4 Blake Medical Center Giving Back to our Community

Blake is proud to have the only CARF Accredited Inpatient Rehabilitation Center in Manatee County as well as the only hospital in Manatee County to be an Accredited Vascular Laboratory. The Cancer Care program at Blake has been Accredited by the American College of Surgeons Commission on Cancer since Blake is Joint Commission Accredited, a Certified Chest Pain Center with Percutaneous Coronary Intervention, and an Accredited Primary Stroke Center. Our Division has arrangements and contracts in place with vendors from around the state and nation to provide support, from on-site security to medical supplies, additional generators, food, fuel, emergency transportation, building supplies and construction crews who can be dispatched immediately following the storm to make needed repairs to keep our hospitals operational. Emergency communications, food and supplies all arrive at each facility well in advance of a storm, to ensure the hospital is selfsustaining for several days. Transfer processes and plans are in place with our network of hospitals from around the country, should patients need to be relocated. Social & Mobile Media We are actively engaged in social and mobile media through blogs and micro-blogs such as Facebook and Twitter, and Blake Medical Center s website is updated routinely to keep our community informed on the latest information, new physicians in the community and health offerings. The h2u program is designed to bring members the latest in health and wellness information as well as help individuals take control of their health care needs, including maintaining a healthy lifestyle, disease prevention, immunizations, self-care and injury prevention. Cardiac/Pulmonary Education: Lunch-n-Learn Sponsored by the eart Institute and the Cardiopulmonary Rehabilitation Department, this program is designed for patients currently enrolled in Phase II or Phase III of Cardiopulmonary Rehabilitation. Additionally, this program is of interest to eart Institute Alumni, their families, and those interested in heart healthy living. Meetings are held in the Dolphin/Pelican room at Blake Medical Center. Light lunch is provided. It teaches female cancer patients beauty tips to look better and feel good about how they look during chemotherapy and radiation treatments. 9 Blake Medical Center Quality & Patient Safety Putting Patients First is the foundation of our culture and passion for quality health care. Many of the initiatives we implement serve as national models for the industry today. Blake Medical Center has achieved industry leading performance on public Clinical Measures, such as the CMS Core Measures for Congestive eart Failure, Pneumonia, Acute Myocardial Infarction, and Surgical Care Improvement Project. Our hospital is thoroughly engaged in programs and initiatives which promote and optimize Patient Satisfaction. hcare Electronic ealth Record (ER) Blake Medical Center continues to move forward with rolling out the ER initiative, which allows for electronic access to patient health information. Our hospital has brought numerous clinical components for the ER on-line through major investments in technology, including: n hcare ub Linking physician practices to our hospital database is accomplished through this interface. Realtime access to patient clinical data from the hospital can provide necessary information to physicians to make timely decisions and reduce duplicate testing and unnecessary hospitalizations. n Picture Archiving and Communication System (PACS) The PACS system immediately digitizes images from radiology and cardiac studies so they can be stored and electronically viewed on screens remotely. This enables doctors and other health care professionals to access the images and, when available, compare new images with previous images at the touch of a button. n Voice Recognition To improve report turnaround time and patient care, our hospitals and imaging centers use Voice Recognition, which allows physicians to update patient records and report their interpretation of a study by speaking instead of dictating or using a keyboard. The physician then reviews the report on screen, makes any adjustments, signs the report and it posts immediately for others to view. Report turnaround time has been greatly reduced. n MUSE Blake Medical Center has installed this system which digitizes EKG tracings so physicians can review, diagnose and treat heart patients within seconds. Electronic Bar Coding Medication System (BCMA) Blake Medical Center, and its parent company CA, led a national effort to reduce medication errors by placing this system in all of our hospitals several years ago. Bar codes on medications are scanned at the patient s bedside, providing our clinical staff with an effective tool to ensure that the right patient receives the right medication at the right time through the right route. Electronic Bar Coding Transfusion System (BCTA) Bar coding was expanded at Blake Medical Center this past year to include transfusion products. Prior to administering any blood product, the unit is scanned to ensure the patient is receiving the right transfusion product at the right time through the right route COMMUNITY REPORT 7

10 Our Commitment To Excellence Broselow System for Acute Pediatric Administration of Drugs in the ER The hospital s emergency department has installed the Broselow System to ensure the proper dosing and administration of drugs to children. Pediatric dosing can be difficult, especially in emergency situations, and the Broselow System allows medical professionals to instantly access proper dosing, improve the speed and accuracy in acute preparation and administration of drugs, reduce medical errors and standardize pediatric dosing so that we can provide our young patients with high quality emergency care, safely. Responsible Imaging The Imaging Department at Blake Medical Center has implemented an innovative radiation safety program called Responsible Imaging. The program is a divisionwide commitment to providing the highest quality medical images with the lowest reasonably achievable radiation dose. VTE Protocol Blake Medical Center, in conjunction with sanofi-aventis, implemented an aggressive venous thromboembolism (VTE) Rule-Out initiative and has actively participated in deep vein thrombosis awareness efforts to help educate the public about these health risks. The Rule-Out protocol helps ensure that patients are screened and, when appropriate, receive preventive treatment for VTE which poses a significant health risk and consists of two potentially life threatening conditions: Deep Vein Thrombosis and Pulmonary Embolism. The Staff at Blake Medical Center and the EMS was top notch all the way. They knew what they were doing and I took great comfort in that. I feel normal again. I didn t realize just how bad I felt prior to having my heart attack. 11 Blake Medical Center Unprecedented Flu Season Preparation The National Patient Safety Foundation has recognized all CA employees for their efforts in supporting CA s flu immunization policy. Whatever the flu season brings, CA employees have prepared well. Our hospitals stepped up efforts to encourage employees to take advantage of our free flu vaccine program and practice good hygiene. More than 97 percent of our employees have received the flu vaccine, an amazing accomplishment which helps protect our patients as well as our employees and their families. Additionally, many of our hospitals actively participate in community outreach efforts to educate the public about the importance of seasonal flu vaccine and the 1N1 vaccine. Methicillin-resistant Staphylococcus aureus (MRSA) CA continues to be a leader in the fight to stop the spread of MRSA, and was invited to participate as a partner organization in the REDUCE MRSA Clinical Trial, with the CDC Prevention Epicenters, CDC Division of ealthcare Quality Promotion and the Agency for ealthcare Quality and Research (ARQ). CA was selected due to its commitment to infection prevention, the CA A,B,Cs (MRSA) program and our standardization of specific lab data. The goal of the study is to conduct a cluster randomized trial of hospitals to compare uniform versus target MRSA decolonization as a means to reduce healthcareassociated MRSA infection. Locally, eight CA West Florida Division hospitals are participating. Locally, we continue to be very involved in educational outreach to prevent the spread of community acquired MRSA. The national Clean ands Are Cool ands campaign, featuring teen star Mitchel Musso (annah Montana show), was launched here in 2008 and participation extended throughout 2009, with our partner schools. The goal is to educate children on the prevention of MRSA, through local presentations and a tool kit for parents and teachers available on our hospital websites. Nation s First Infant Screening Program CA, Blake s parent company, was the first US healthcare system to voluntarily perform a simple blood test to screen newborns for elevated levels of bilirubin that triggers jaundice. By identifying newborns at risk for Kernicterus, which causes permanent brain damage and physical impairment, we can treat them immediately and prevent this serious health threat COMMUNITY REPORT 9

12 Our Commitment To Excellence Specialized care Emergency Excellence ighly skilled emergency physicians and nurses combined with best practices and the latest technology benefited close to 30,000 Manatee County residents and visitors last year. Protocol driven care that can begin in the field through direct communications with local EMS teams, and meeting stringent standards to earn Chest Pain Center with PCI Accreditation, Acute Myocardial Infraction (eart Attack) Certification and Advanced Primary Stroke Center Certification by the Joint Commission are all part of our commitment to improve quality in our busy emergency department. Our strong focus on enhancing care and improving ER processes has also dramatically reduced wait times to below national averages. In fact, we re so successful, current ER wait times are now on our hospital website, billboard, and by text messaging: ER to eart & Vascular Care The eart Institute at Blake Medical Center provides life-saving technology and the latest minimally invasive techniques in the detection and treatment of cardiac and vascular conditions. Blake s eart Institute, in partnership with our Emergency Department, has been recognized as an Accredited Chest Pain Center with Percutaneous Coronary Intervention, Accredited Acute Myocardial Infarction Center, a 100 Top Cardiovascular ospital by Solucient (now Thomson Reuters), and a Blue Distinction for Cardiac Care award winner by Blue Cross Blue Shield. Blake s Vascular Program recently became the only accredited Vascular Laboratory by ICAVL. By partnering with our local EMS, Blake routinely beats the national STEMI benchmark of 90 minutes, dramatically improving the survival and resulting quality of life of our patients experiencing a heart attack. The latest surgical techniques for minimally invasive mitral valve repair and atrial fibrillation treatments provide safer alternatives for patients suffering from these conditions, improving their quality of life and reducing their risk for other serious complications. Our Cardiac Cath and EP labs offer stateof-the-art technology and treatments to invasive and noninvasive cardiac procedures. 10 Blake Medical Center Specialized Care

13 Blake Medical Center Expanding Trauma Care CA and USF ealth have entered into an affiliation agreement to improve access to quality trauma care and services in Florida. The agreement includes two West Florida hospitals: Blake Medical Center and Regional Medical Center Bayonet Point. Both facilities have received state approval on their extension request to move forward with the development of trauma programs which will provide this highly specialized level of emergency care to communities identified by the state as underserved. Both hospitals plan to submit their completed application to the state in 2011 with the plans of being granted provisional status to operate trauma programs in fall Neurosciences & Stroke Care Our hospital has earned Joint Commission Primary Stroke Center designation, the gold standard for ensuring the highest level of care is immediately available. In 2009, Blake was also awarded the Gold Plus Performance Award from the American Stroke Association and in 2010, the Target Stroke Award. Cancer Care Cancer is the second leading cause of death in this country and our hospital is committed to meet and exceed the many standards required for the American College of Surgeon s Commission on Cancer (ACoS CoC). Our Cancer team, which includes surgeons, medical oncologists, radiation oncologists, pathologists, radiologist and hospital clinical and support staff meet regularly to proactively review patient cases. I ve driven a big rig my whole life and never had an accident until I slipped and fell off my truck last October. My wife, a Blake Medical Center employee for almost 30 years urged me to go into the ER and in turn, she saved my life. That s when they found the mass on my lung, had I not went into Blake that day, I would not be here today. After my surgery, Dr. Ganey presented my case to the Tumor Board and told me I needed to follow up with chemotherapy and possibly radiation. I thought, if all those docs think this is what I need I better get it done. I ve been so pleased with the Cancer care Team at Blake. Glen Williams Cancer Patient Women s Services The Breast Care Center at Blake Medical Center offers a unique experience, dedicated to treating women in a warm and relaxing atmosphere. We are committed to helping women receive life-saving treatment quickly. Our Patient Navigators work with our cancer team and newly diagnosed patients to reduce the time between diagnosis and treatment. They also provide information and support giving our patients peace of mind. Each day new lives are touched by cancer. My hope is to improve the journey for each patient and their family by providing compassion, support, education and individualized attention. 14 Our Commitment To Excellence Advanced Surgical Services Our 45,000 sq. ft. surgical center offers some of the most efficient and well equipped operating rooms in the area. Advanced systems from scheduling to pre-op and discharge planning ensure that patients receive the care they need. Minimally invasive procedures include Single Incision Laparoscopic Surgery (SILS) for a variety of general surgery procedures, like gall bladder removal, and incision-less surgery for the treatment of Gastroesophageal Reflux Disease (GERD).
