All payments are due at the time of arrangement. Cash Personal Check (Subject to electronic funds verification) Credit Card (VISA, Master Card, or Discover) Insurance Assignment (subject to a convenience fee of 1%) All financial arrangements are made with the Purchaser, and not with the estate of your loved one.

2 The goods and services shown below are those we can provide to our customers. You may choose only those items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. Basic Services and Overhead of Funeral Director and Staff $1595 Our fee is for the basic services of funeral director and staff includes, but is not limited to, staff to respond to initial request for service; arrangement conference with family and responsible parties; arrangements of all services requested; preparation and filing of the necessary authorization and permits; recording of vital statistics; preparation and placement of obituary notice; care of the deceased; and coordination with those providing other portions of the final services. Also included in this charge are the overhead expenses relative to our facilities such as professional licensing, legal and accounting fees, insurance, building, and utility expenses, maintenance, taxes, equipment, furnishings and administrative expenses. This fee is for our basic services and overhead and will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials and forwarding or receiving remains.) Transfer of remains to funeral service facility $325 Transfer of remains to local point of transportation $200 Embalming $550 Except in certain special cases, law does not require embalming. Embalming may be necessary; however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. 4 Alternative Selections Receiving Remains From Another Funeral Home $1325 Charges for this service include transfer of deceased from local point of transportation to our facility, basic services of funeral director and staff, and hearse to the cemetery. This charge does not include merchandise, use of facilities for viewing or ceremony, supervision of graveside ceremony, or shipping charges, sales tax or cash advance items. Forwarding of Remains to Another Funeral Home $1750 Charges for this service include transfer of deceased to our location, embalming, dressing and cosmetizing, basic services of funeral director and staff, utility vehicle, and transfer of remains to local point of transportation. This charge does not include merchandise, use of facility for viewing, ceremony, or shipping charge, sales tax or cash advance items. Immediate Burial $1525 Charges for this service include transfer of deceased to our location, bathing, disinfecting, and refrigeration, basic services of funeral director and staff, utility vehicle, and hearse to cemetery. This charge does not include merchandise, use of facilities for viewing, ceremony, or supervision of graveside ceremony, sales tax or cash advance items. Direct Cremation $1345 Charges for this service include transfer of deceased to our location, bathing, disinfecting, and refrigeration, basic services of funeral director and staff, utility vehicle, and vehicle to crematory. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without outside covering). The container we provide is a fiberboard container. This charge includes the minimum cremation container and temporary container for the cremated remains. This charges does not include additional merchandise, use of facilities for viewing, ceremony, or supervision of graveside ceremony, sales tax or cash advance items. Anatomical Donation $ 995 Charges for coordination with local anatomical organizations include; basic services of funeral director, staff and overhead, removal of remains, utility vehicle, and embalming. This charge does not include viewing, ceremony, additional transportation or any merchandise. If the anatomical board does the embalming, that charge will be deducted.

5 Funeral and Cremation Package Plans Traditional Funeral Service $3925 Basic services of funeral director, staff, and overhead, transfer of deceased, embalming, other preparation of the body, utility vehicle, hearse, use of facilities and staff for viewing (1 day), chapel, church or alternate location for funeral service. This charges does not include merchandise, sales tax or cash advance items. Graveside Service after visitation - NO funeral service $3,625 This charges does not include merchandise, sales tax or cash advance items. Traditional Cremation $4,825 Basic services of the funeral director, staff and overhead, embalming, other preparation, transfer of remains, visitation/viewing. Funeral service and graveside, hearse, utility vehicle, cremation fee and ceremonial casket,($3,450 Value Wood Casket). This charges does not include sales tax or cash advance items. Memorial Service/Celebration of Life $2,145 Cremation with private family viewing followed by and memorial ceremony This charges does not include merchandise, sales tax or cash advance items. Graveside services only This charges does not include merchandise, sales tax or cash advance items. $1,945 Anatomical Donation with Memorial Service $1250 Basic services of funeral director, staff and overhead, memorial service and utility vehicle. This package does not include viewing or an urn. Receiving remains, Traditional Funeral Service and Burial $1,995 Proportion of basic service of funeral director, staff and overhead, visitation/viewing, funeral service with graveside and hearse. Receiving remains and graveside service - no visitation $1,595 Proportion of basic service of funeral director, staff and overhead, visitation/viewing, graveside and hearse. Receiving Cremated remains with Memorial service/celebration of life $1,250 Proportion of basic service of funeral director, staff and overhead, Memorial services, graveside and utility vehicle. Graveside Services only $1,000

6 Cash Advance Items: We will be happy to work and advise you and your family in arranging all aspects of your services including the following charges that we will arrange and pay for you in advance. Sales Tax (8.4%), Certified copies of death certificates (First is $20/others $13) Floral arrangements, Clergy and Musician Honorariums, Paid Funeral Notices in newspapers and additional transportation charges, out of state OTR or airline.
