1 Direct Supervisor(s): RN Manager Direct Reports: none Collaboration: CMO, Director of Clinical Operations, Health Center Managers, Project Manager(s), CEO, COO JOB DESCRIPTION: LPN-Provides relevant nursing care within an innovative Team Care Medicine Model, in an ambulatory setting. Nursing care is provided under the guidance and supervision of a registered nurse and clinical leadership. Responsibilities include collection of clinical data, care coordination, administration of medications and treatments, and documentation of care provided. Team Care Implementation Specialist-This position will be responsible for ensuring Community Health Alliance s (CHA s) goals regarding the implementation of the Team Care Medicine Model into its practices are achieved through periodic review and reporting of progress. This position may be filled by one or more persons, according to System demand. EMR Specialist Back-up Trainer- Assisting the EMR Specialist with training and other duties in conjunction and in the absence of the EMR Specialist. Scheduling: Intermittent Saturday hours every weeks along with assigned medical provider. No call. Combines nursing clinical skill, care coordination, patient education, expertise in TCM and EMR duties and training of staff, in collaboration with interdisciplinary team, to deliver quality patient care. 1) Nursing skills within scope of LPN patient rooming, onsite lab completion, basic testing, nebulizer treatments, injectable medications (within scope of practice), immunizations 2) Care coordination (site specific) referral/test tracking, patient education and self-management support, and immunization admin to be completed per protocols generated by RN manager built within the scope of practice for LPN in NV 3) Administrative duties a. Potential for front office and scheduling skills with the Practice Manager software b. Community outreach events i. Community events as representative of CHA ii. Public speaking engagements with medical provider and administrator 4) Time allocations a. Clinical days i. Potentially requires front office skill in check-in and discharge ii. Minimum of 2 patients per hour iii. Communications liaison for care team iv. Immunizations v. Lab processing vi. Other nursing tasks assigned b. Non-clinical days i. Care coordination (based upon care coordination protocols referrals/test tracking, communication liaison, patient engagement, etc) ii. Immunization clinic days 1
2 iii. Patient education appointments and telephonic self-management support 5) TCM Implementation Specialist a. Support prior to Training i. Ensure upper management s goals for implementing Team Care Medicine (TCM) are clearly defined. ii. Work with provider teams and practice managers to be sure goals are properly understood. iii. Work with practice managers, providers and TCM to identify key measures required to monitor and determine success for TC in the practices. iv. Work with System Administration and TCM project manager to identify baseline metrics on key measures of success. v. Develop / approve means of capturing and storing / recording of key metrics. vi. Ensure any/all changes to CHA s clinical workflow are understood and agreed upon with the clinical teams prior to on-site training and launch coaching. vii. Collaborate with EMR Implementation/Training Specialist with ensuring the cohesiveness of the EMR and TCM to provide proper training of staff. b. Support post Training i. Meet with provider and team to communicate specific goals (if any) for that team. ii. Periodically observe wkly team mtgs to ensure they are effective & informative for the team. iii. Perform periodic in-person review of teams (inside the exam room) to evaluate adherence to the TCM Model, team communication and team interactions using TCM evaluation forms if desired. iv. Check weekly or monthly metrics and track results versus expectations and review with provider and practice manager to ensure progress is being made towards achieving goals. v. Monitor patient appointments to ensure access for acute patients is in line with goals and regular appointments are being added to accommodate more patients when teams are ready. vi. Identify areas of weakness and encourage providers to train their teams in those areas to increase their effectiveness. vii. Continue collaboration with EMR Implementation/Training Specialist with maintaining the cohesiveness of the EMR and TCM to ensure proper training of staff. PRIMARY ACCOUNTABILITIES Achieve Results 1. Improve patient outcomes, and deliver quality care within practice areas as assigned. Ensure all clinical care is delivered in a manner that meets or exceeds goals and expectations for clinical outcomes, quality assurance standards, and patient satisfaction. 2. Ensures timely and accurate collection of patient data, and makes observations on assigned patients to provide supportive data in the patient assessment. 3. Promotes attainment of specified goals through health education to patients/families, and health care personnel as delegated by the registered nurse. 4. Provides service in a manner that is appropriate for the patient s age; demonstrates knowledge and skills necessary to meet the patient s physical, psychosocial, educational, and safety needs. 5. Ensure all patient charts and related documentation are maintained current and relevant at all time. 2
3 Operational Excellence 6. Ensure all care and nursing practices are conducted consistent within current, relevant standards, as well as state and local laws and regulations, PCMH accreditation and related professional standards. 7. Demonstrate competency in practice and knowledge of current standards of practice. Maintains credentials and performs current nursing practice expectations within established guidelines. 8. Provides care in a manner that respects the patient s rights and choices in a multi-cultural setting. Relationships 9. Develops and maintains favorable internal relationships, partnerships with co-workers, including health center managers, nursing staff, medical assistant staff, medical providers, care coordinators, and business office staff. 10. Develops and maintains favorable external relationships with vendors, contractors, specialist offices, etc. Stewardship and Professionalism 11. Ensure all actions, job performance, personal conduct and communications represent the organization in a highly professional manner at all times. 12. Uphold and ensure compliance and attention to all corporate policies and procedures as well as the overall mission and values of the organization. PRIMARY TASKS & DUTIES 1. Provide direct care to patients as assigned and qualified to perform. 2. Assess, review, and develop nursing plans to ensure maximum patient outcomes 3. Provide counsel, support, advice, and support to patients and their families 4. Complete all patient charting, and ensure all patient records are current and complete 5. Attend meetings, patient conferences, planning sessions, related to quality improvement/assurance, patient care, and other related topics within the health center and CHA 6. Attend seminars and maintain all licensure requirements for continuing education and best practices 7. Participates in quality strategies to evaluate compliance with standards and to identify opportunities to improve patient outcomes. 8. TCM Implementation Specialist Post-training Duties: After launch, the Implementation Specialist is the key individual responsible for ensuring the full implementation of the model and the achievement of the objectives of Sr. Management. Their duties will now include: a. Becoming the main person to coach the teams, solve problems, coordinate activity, and lead process/administrative change within the pilot teams. b. Being responsible for ensuring the model is strictly followed with corresponding authority to bring issues to the attention of practice and hospital management when a team is not complying with the TCM principals. 3
4 c. Visiting each team two times per month (for 1 st three months) as coach, mentor and cheer leader and to observe and evaluate teams using TCM provided evaluation sheets. d. Gathering regular metrics updates and reviewing the results against goals with the practice manager and the provider teams. e. Initiate changes as needed to ensure Sr Management goals are being achieved and hold the practice manager, provider and team responsible. f. Providing monthly progress updates to Sr Management to update in general how the implementation is proceeding and provide detailed reports of progress toward meeting the goals of the pilot. g. Coordinate interactions with TCM staff to schedule group conference calls with providers and practice management as desired, provide updates and get issues/questions resolved. h. Ongoing participation in collaboration with EMR Implementation/Training Specialist with ensuring the cohesiveness of the EMR and TCM to provide proper training of staff. 9. EMR Specialist Back up Trainer a. Assisting and leading staff EMR trainings for new hires b. Providing back-up for coverage of EMR duties when EMR specialist is on PTO c. Assisting with EMR training curriculum development as needed ESSENTIAL FUNCTIONS/ KEY COMPETENCIES Demonstrate a high level of skill at building relationships and customer service. Demonstrate interpersonal savvy and influence skills in managing difficult clients and patients. Demonstrate high degree of knowledge and competency in the practice of medicine and associated charting requirements. Requisite skills and ability to perform certain medical tasks as assigned Demonstrate a high level of problem solving skill to better serve patients and staff. Strong attention to detail and accuracy. Ability to utilize computers for data entry and information retrieval. Excellent verbal and written communication skills. Ability to implement, and evaluate operational and administrative processes. Completes tasks as otherwise assigned by supervisor and/or leadership POSITION REQUIREMENTS Education Graduation from an accredited practical or vocational nursing program. Current and Unrestricted LPN license from the Nevada Board of Nursing Current BLS - CPR certification Experience Three years of LPN practice experience; family medicine practice or other primary care setting preferred Knowledge and experience with implementation and/or support and training of team care model and EMR Demonstrated knowledge and abilities of nursing skills Clerical/administrative experience a plus 4
5 Demonstrated success in customer service or working with the general public, preferably in a medical care facility. Demonstrated success in managing difficult customer situations Significant knowledge of medical practices and procedures in a primary care environment. Demonstrated success and experience with direct patient services and the proper knowledge and use of medical devices and equipment. Spanish language skill preferred Physical/ Environmental Occasional lifting of lbs and pushing of 5-20 lbs Sitting, standing and walking approximately 80% of the time A medium to high level of manual dexterity required Bending and reaching approximately 20% of the time Education and Licensure: I. Graduate with a LPN designation. II. Current CPR certification III. Current and Unrestricted LPN license from the NV Board of Nursing Experience: I. Current work experience in a family medicine practice or other primary care setting preferred II. Demonstrated knowledge and abilities of nursing skills III. Clerical/administrative experience a plus IV. Spanish language skill preferred Job Standard 1: Confidentiality I.
