7 FORM 5 Group Advisor & Chaperone Complimentary Season Pass Application PLEASE NOTE: Please include cell phone numbers below, for all group leaders and chaperones, for EMS records. 1) Group Advisor is a Chaperone - You MUST include Advisor in on the Chaperone count based on a 1:1 Chaperone to Club Member ratio.
