3 A & B Shuttle # REQUIREMENTS: No special requirements. RESTRICTIONS: Must be able to walk to the car without assistance. Wheelchair van available upon request. SERVICE AREA: Douglas, Sarpy, Lancaster & Pottawattamie counties COST: --$24 to $32 one way depending on distance of trip --$42 one way flat fee for anyone needing wheelchair accommodations Additional people can ride for $4 more. SCHEDULING: Must call 24 hours in advance. An advance reservation of 48 hours is recommended. Call during regular business hours. SERVICE HOURS: Available everyday anytime by reservation only. APPLICATION: No application needed.
4 American Ambulance Service # REQUIREMENTS: No special requirements. SERVICE AREA: Will transport anywhere within the metro area. For additional cost, will transfer outside of the metro area. COST: One way trip--$37 Round trip--$71 SCHEDULING: Call as soon as possible. SERVICE HOURS: Seven days a week---24hrs a day. APPLICATION: Application not needed.
5 Bellevue Transportation Program # REQUIREMENTS: Must be 60 or older or have a disability that prevents driving. Must live in Bellevue. SERVICE AREA: Will take anywhere in Bellevue. Will take to Omaha--the boundary is 84 th & Dodge COST: In Bellevue--$2 each way To Omaha--$4 each way If help is needed, may take another person along for free. SCHEDULING: Must call Mon. through Fri. to schedule. Trips must be scheduled 48 hours in advance. SERVICE HOURS: In Bellevue: Mon. through Fri. 8 a.m. to 3p.m. To Omaha: Bus runs at 8a.m-10a.m- 12 noon-2p.m. Dr. appointments have to be made for 9a.m.,11a.m. or 1p.m. final pick up is at 2p.m. APPLICATION: Application needed. Can be taken over the phone.
6 Council Bluffs Shopper # REQUIREMENTS: No special requirements. RESTRICTIONS: Minimum of 3 riders is required. SERVICE AREA: Will take anywhere within the city limits of Council Bluffs. COST: $5.00 each way. SCHEDULING: All reservations need to be called in by 5p.m. the Tuesday before. SERVICE HOURS: Every Wednesday from 10a.m. to 2 p.m. APPLICATION: Application not needed.
7 Council Bluffs Special Transit Service # REQUIREMENTS: May qualify if legally blind and also have a mobility limitation or unable to ride the regular bus for any reason. Must reside within Council Bluffs city limits. SERVICE AREA: Will take anywhere within the city limits of Council Bluffs. Service is available to a number of medical facilities in central Omaha. COST: $2.00 each way. A personal care attendant can ride for free but must be preapproved for an attendant. SCHEDULING: May call as much as 14 days in advance but no less than 24 hours in advance. SERVICE HOURS: Mon. through Fri. 8a.m. to 5p.m.; Sat. 9a.m. to 5p.m.; Sun.--10a.m. to 5p.m. APPLICATION: Must submit an application form plus a medical physical form & visual impairment verification form which have been signed by a doctor.
8 Eastern Nebraska Office on Aging (ENOA) Transportation # REQUIREMENTS: Limited to people who do not drive and must be 60 or older. SERVICE AREA: Cass, Sarpy and rural Douglas County ASSISTANCE: Vehicles with wheelchair lifts available for the non-ambulatory. COST: $5.00 up to 20 miles $6.00 for 21 to 40 miles $7.00 for 41 to 60 miles $8.00 for 61+ miles SCHEDULING: Reserve at least one week to ten days in advance. SERVICE HOURS: Monday to Friday from 8a.m. to 5p.m. APPLICATION: Must be a client of the Office on Aging. Can sign up over the phone.
9 Home Care Service Providers A care provider will accompany the client. 9 Care Tech, Inc. # Minimum Hours of Service: 3 hours Cost: $16.50 to $18/hour plus 47 cents per mile. Home Instead Senior Care # Minimum Hours of Service: 3 hours Cost: $19/hour plus 42 cents per mile Right at Home # Minimum Hours of Service: 3 hours Cost: $17.25/hour. Client would need to provide the vehicle. Traveling Angels, Inc. # Minimum Hours of Service: 2 hours Cost: $25/hour plus 39.5 cents per mile
10 Hope Transportation # REQUIREMENTS: Must be a Medicaid client. RESTRICITONS: Must be able to walk without wheelchair assistance. SERVICE AREA: Will take anywhere within the Omaha area. COST: Round trip--$32.02 SCHEDULING: Need to call 48 hours in advance. SERVICE HOURS: Monday to Friday from 7:30 a.m. to 5p.m. APPLICATION: Must work with a case worker to complete the necessary paperwork.
11 MEDIVAC # REQUIREMENTS: Service designed for the elderly that use a wheelchair & those with disabilities that use a wheelchair. SERVICE AREA: Will take anywhere. ASSISTANCE: Escort service available. COST: Base fare--$30 plus $1.50 per mile. There is an additional cost of $10 per hour for escort service and an additional $25 for use of services on the weekend. SCHEDULING: Recommend a minimum notice of 24 to 48 hours. A 24 hour notice is strongly encouraged for weekend, evening, or holiday trips. SERVICE HOURS: Mon. through Sun. anytime. APPLICATION: No application needed.
12 Midwest Medical Transport # REQUIREMENTS: No special requirements. SERVICE AREA: Bellevue, Omaha, Fremont. ASSISTANCE: Vehicles with wheelchair lifts available for the non-ambulatory. COST: $36.00 one way for up to 14miles--$2.15/mile for each mile over 14miles. $70.00 round trip for up to 28 miles-- $2.15/mile for each mile over 28 miles. Additional $18 charge for oxygen. SCHEDULING: Can reserve a ride a day or two in advance. Must call at least two hours before pick-up. SERVICE HOURS: 24hours--7 days a week APPLICATION: Application not needed.
13 Moby Van Service # , ext (Website: 13 REQUIREMENTS: May qualify if legally blind and also have a mobility limitation or unable to ride the regular bus for any reason. RESTRICTIONS: Drivers will only help with getting on & off of the van at the curb. SERVICE AREA: Will take anywhere as long as departure and destination points are within 3/4 miles of a bus route in Omaha. COST: $2.00 each way. A personal care attendant can ride for free or a companion can ride for $2.00. SCHEDULING: Must schedule a minimum of 24 hours in advance. To be assured of a ride, can call 2 weeks in advance. Must give a 2 hour notice to cancel. (Note: To use services on Sat., Sun., or Mon., must call to schedule by 4:30p.m. on Friday.) SERVICE HOURS: Mon. through Fri. 5a.m. to 10p.m., depending upon the bus route; Sat. 6a.m. to 9:30p.m.; Sun.--7a.m. to 7p.m. APPLICATION: Must submit an application form plus a medical physical form & visual impairment verification form which have been signed by a doctor.
14 OMALiNK, Inc # RESTRICTIONS: No special restrictions. SERVICE AREA: A shuttle service between Lincoln and Omaha. OMALiNK picks up in Omaha at these locations: --Eppley Airfield Ground Transportation area --Omaha Downtown --Any exit along the route on I-80 OMALiNK picks up in Lincoln at these locations: --UNL Student Union and East Campus --Union College & Wesleyan University --Any hotel --Lincoln airport --Will pick up at home address for extra charge COST: $49.95 one way for one person $79.95 round trip for one person A $5 discount for persons over 65 or military SCHEDULING: Must make a reservation. SERVICE HOURS: Departs from both Omaha and Lincoln seven days a week, nine times a day between 5a.m. and 11p.m.
15 Papillion Special Services # REQUIREMENTS: Must be 60 or older or have a disability. A non senior without a disability can use the service for full price. Must live in Papillion. SERVICE AREA: Will take anywhere in Papillion. Will take to Omaha the boundary is Dodge to the north, 120 th St. to the west and 40 th St. to the east. Will also take to Bellevue if the destination is within 10 miles from the center of Papillion. COST: In Papillion--$2 each way To Omaha & Bellevue--$2 each way A helper may ride for free. SCHEDULING: Must call 24 to 48 hours in advance. SERVICE HOURS: In Papillion: Mon. through Fri. 7:30 a.m. to 4p.m. Last pick up is 3:00p.m. No doctor appointments can be made for after 1:30p.m. To Omaha & Bellevue: Mon., Wed. & Fri 7:30a.m. to 4p.m. Last pick up is at 3p.m. No doctor appointments can be made for after 1p.m. APPLICATION: No application needed.
16 Papio Transport Services, Inc. # REQUIREMENTS: Must be able to walk without wheelchair assistance. SERVICE AREA: Will take anywhere within Douglas and Sarpy County. Will also take to another state. COST: One Way--$20 Round Trip--$40 To travel to another state--$1.50/mile May take another person along for $7.95. If a medical escort is needed, there is no charge for the escort. SCHEDULING: Need to schedule at least 24 hours in advance. SERVICE HOURS: Monday to Friday from 6 a.m. to 8p.m. Saturday from 6a.m. to 4:30p.m. No service on Sundays or holidays. APPLICATION: No application needed.
17 Prince of the Road # REQUIREMENTS: No special requirements. Also a service for Medicaid clients. SERVICE AREA: Omaha Metro area and surrounding area. COST: One way trip--$19.69 Round trip--$39.80 Outside of Omaha---extra $1.32 per mile Extra $5 charge each way for wheelchair accommodations. A medical escort may ride for free. SCHEDULING: Must call 72 hours in advance. SERVICE HOURS: Monday to Saturday 6 a.m. to 7p.m. APPLICATION: No application needed.
18 Ralston/La Vista Special Services # REQUIREMENTS: Must be 60 or older or have a disability. Must live in Ralston or La Vista. SERVICE AREA: Will take anywhere in Ralston & La Vista. Will also take to Omaha & Bellevue. COST: Within Ralston & La Vista--$1 each way Out of Ralston or La Vista --$3 each way --$6 each way if trip is North of Dodge or West of 120 th St. Multiple stops additional $1 $30 fare tickets are available from driver A helper may ride for free. SCHEDULING: Must call 24 hours in advance. SERVICE HOURS: Mon. through Fri. 7:00 a.m. to 4:30p.m APPLICATION: Application needed.
19 Rural Metro Area Ambulance # REQUIREMENTS: No special requirements. SERVICE AREA: Will transport anywhere within the metro area, including Council Bluffs. COST: $34 each way. No additional cost for one companion. SCHEDULING: Best to schedule in advance. May schedule up to two weeks in advance. SERVICE HOURS: Mon. Fri.: 7a.m. to 6p.m.; Sat: 8a.m. to 6p.m. No Sunday service. APPLICATION: Application not needed.
20 Share A Fare (Half Price Taxi Fare) # Web Site: 20 REQUIREMENTS: Must be legally blind or visually impaired with a visual acuity of 20/80 or less in the better eye with correction and/or a visual field no greater than 100 degrees or blind in one eye and visual acuity of 20/60 or less in other eye with correction. COST: This is a cab subsidy program for the blind and visually impaired of the Greater Omaha Area which allows participants to purchase cab coupon books at a fifty percent discount. The participating cab companies are Happy Cab and Safeway Cab. APPLICATION: An application must be completed and signed by an authorized person: Dr., nurse, optometrist, public welfare agent or librarian.
21 Shared Mobility Coach # REQUIREMENTS: No special requirements. SERVICE AREA: Will take anywhere in the Omaha metropolitan area, including Bellevue. COST: Ambulatory passengers--$16.25 each way. Wheelchair van--$39.50 each way. Escort allowed for special needs no charge. SCHEDULING: May schedule as much as 5 weeks in advance or as little as 48 hours in advance. Must call Mon. through Fri. between 8a.m. to 5p.m. SERVICE HOURS: Mon. through Fri. 4:30a.m. to 5:30p.m.; Sat. from 4:30a.m. to 4:30p.m. No Sunday service. APPLICATION: No application needed.
22 Veterans Transportation # REQUIREMENTS: Must be a veteran. RESTRICTIONS: Will only transport to the VA Medical Center for medical appointments or to another clinic for service if a VA referral has been made for the service. SERVICE AREA: Will transport within a 50 mile radius of Omaha. COST: No cost. SCHEDULING: Call one week in advance. SERVICE HOURS: Monday to Friday starting at 7a.m. Morning hours preferred. APPLICATION: Must be enrolled in the VA system.
23 Additonal Transportation Information 23 Metro Area Transit # Ext Handicapped person may use the city bus system at half-price fare after obtaining an identification card. Every bus has either a lift or a ramp. ($1.25 Regular Adult Fare; $0.60 Half-Fare) Air Travel Services Personal service assistants are available to escort handicapped passengers on and off planes, manage transfers, locate accommodations within airports, collect luggage and access local transportation. Ask travel agent to mark ticket visually handicapped. (NOTE: A Web site on travel and tourism for people with disabilities can be found at Amtrak # A 15% discount fare is available to passengers with disabilities.
