1 BIODIVERSITY HABITAT SURVEY FORM Introduction This form will assist you in surveying habitat areas for the Biodiversity Index. Print this form and use it for collecting data when doing the outdoor survey. Once you have completed the survey enter the data into the Biodiversity Index website in the add a Habitat Area section. ADD A HABITAT AREA Example of a Habitat Type: ROWS OF TREES Row of trees (not within a hedgerow) USE BOTH COLUMNS IF YOU HAVE TWO HABITAT AREAS OF THE SAME HABITAT TYPE ON YOUR SITE.
