License No. FD 291 Prior to drafting any contract for goods or services, the responsible party or the decedent s survivor who is handling the funeral arrangement, is entitled to receive a copy of any pre-need agreement in the possession of the funeral establishment that has been signed and paid for, in full or in part, by or on behalf of the decedent. Each funeral establishment, prior to drafting any contract for funeral goods or services, must present to the survivor of the deceased who is handling the funeral arrangements, or the responsible party, a statement disclosing whether or not the funeral establishment has any preened agreement made by or on behalf of the deceased. **Our policy for Infants and Children less than 3 years of age is: 40% of Itemized Services, and 28% of Minimum Service Packages. For Children ages 3 through 12: 60% of Itemized Services and 50% of Minimum Service Packages. The following items are not discountable, and will be charged at the normal rate: Merchandise, Cash Advances, Autopsy Restoration, Evening/Weekend/Holiday Fees, Reception Facilities, Coffee Setup, and Additional Mileage.** INDIVIDUALLY ITEMIZED PRICES: The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on your statement describing the funeral goods and services you selected. PROFESSIONAL SERVICES: BASIC SERVICES OF FUNERAL DIRECTORS AND STAFF $1,200 This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for all funeral packages, Direct Cremations, Immediate Burials and Forwarding or Receiving of Remains). Our services include: conducting the arrangement conference; planning the funeral, consulting with family and clergy; shelter of remains, preparing and filing of necessary notices; obtaining necessary authorizations and permits; coordinating with the cemetery, crematory or other third parties. In addition, this fee includes a proportionate share of our basic overhead costs. EMBALMING $350 Except in certain special cases, embalming is not required by law. Embalming may be necessary if you select certain funeral arrangements, such as a funeral with a viewing. If you do not want embalming, you usually have the right to choose services which does not require you to pay for it, such as direct cremation or immediate burial. These packages are designed to reduce funeral costs and since you may, as an alternative, select itemized items (described above), credits for deletions are not possible. These packages do not include the cost for cash advance items or additional merchandise. If you want to arrange a direct cremation, you can use an alternative container. Alternative Containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering).
