SECTION I: JOB IDENTIFICATION Job Title: Clinical Program Manager FLSA: Exempt Position: Outpatient Clinical Manager Salary Grade: 21 Reports to: Department: CLINICAL SERVICES DIRECTOR Outpatient Therapy SECTION II: SUMMARY OF JOB FUNCTION The general purpose of this position is to provide clinical and administrative supervision to Therapists along with ensuring that all regulatory guidelines are being met within all assigned programs. SECTION III: JOB SPECIFIC DUTIES 1. Carries out supervisory functions of training, scheduling, monitoring and evaluating performance of all assigned staff. 2. Administers disciplinary action when necessary and in a timely and professional manner, consulting with Human Resources in accordance with policy. 3. Works in collaboration with Human Resources to establish departmental/program staffing needs, participating fully in the recruitment and selection process. 4. Quality Assurance: Handles consumer concerns and complaints. Performs chart audits and assist in compliance issues. 5. Reviews treatment plans, discharge summaries and transfer summaries SCNDs and Crisis plans. 6. Assists with crisis calls during the daytime hours and is on call when assigned. 7. Approves therapy notes for assigned staff. 8. Assigns members to appropriate therapists.
2 9. Provides clinical supervision to Therapists and assists with oversight and supervision with other clinical staff. 10. Ensures professional staff are working toward licensure. *Performs all other duties as assigned SECTION IV: JOB QUALIFICATIONS Requires the following EDUCATION/TRAINING/CERTIFICATION/LICENSURE/EXPERIENCE: Requires a Master's Degree and at least two (2) years experience; License with the Board of Behavioral Health Examiners preferred; Must be at least 21 years of age. SPECIAL EQUIPMENT/SKILLS: Proficient with MS Office products, , internet, basic database navigation. POPULATION SERVED: Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the consumers served in the department. Infant & Early Childhood (birth 5 years) Child (6 11 years) Adolescent (12 17 years) Young Adult (18 21 years) SECTION V: JOB SPECIFIC EXPECTATIONS PROFESSIONAL DEVELOPMENT: EDUCATION/TRAINING/CERTIFICATION/LICENSURE Must maintain current Arizona Fingerprint Clearance card. SECTION I: JOB IDENTIFICATION Job Title: Therapist / Clinician FLSA: Exempt Position: Family Centered Autism Program Therapist Salary Grade: 16 Reports to: Department: FCAP PROGRAM SUPERVISOR Family Centered Autism Program SECTION II: SUMMARY OF JOB FUNCTION The general purpose of this position is to provide clinical behavioral health care to consumers and their families using the assigned Evidence Based Program (EBP) model of treatment. SECTION III: JOB SPECIFIC DUTIES 11. Provides outpatient individual, group, and family therapy for Touchstone consumers utilizing Best Practice. 12. Facilitates and participates in Child and Family Teams (CFT) according to the AZ Practice Model. 13. Conducts Intakes as needed and assigned. 14. Completes discharges in a timely and efficient manner. 15. Completes all required clinical documentation & reports and develops & updates consumer documentation timely and accurately. 16. Responds to Crisis in a timely manner according to determined procedure. 17. Acts as liaison between Touchstone and collaborative agencies. *Performs all other duties as assigned SECTION IV: JOB QUALIFICATIONS
4 Requires the following EDUCATION/TRAINING/CERTIFICATION/LICENSURE/EXPERIENCE: Requires a Master's degree in a behavioral health related field; Strong preference for licensure with the Board of Behavioral Health Examiners or Board Certified as Behavioral Analyst under the Behavioral Analyst Certification Board. Must be at least 21 years of age. SPECIAL EQUIPMENT/SKILLS: Intermediate computer skills. Working knowledge of MS Office products or similar software, internet, and . POPULATION SERVED: Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the consumers served in the department. Infant & Early Childhood (birth 5 years) Child (6 11 years) Adolescent (12 17 years) Young Adult (18 21 years) SECTION V: JOB SPECIFIC EXPECTATIONS PROFESSIONAL DEVELOPMENT: EDUCATION/TRAINING/CERTIFICATION/LICENSURE Must be actively seeking licensure. Must complete all Agency prerequisites for conducting assessments within 6 months of hire. Must maintain current CPR/First Aid and CPI Certification. Must maintain current Arizona Fingerprint Clearance Card. Must maintain a valid Arizona driver s license and current personal auto liability insurance. Must be able to provide non-violent physical intervention. Must be willing and able to videotape and/or audiotape. Includes non-traditional work hours including some evenings and occasional weekends Work is conducted primarily in families homes resulting in possible exposure to allergens including pets and cigarette smoke. SECTION III: JOB SPECIFIC DUTIES 18. Provides outpatient family, individual, and group therapy for Touchstone consumers utilizing best practices. 19. Facilitates and participates in Child and Family Teams (CFT) according to the AZ Practice Model. 20. Conducts intakes, assessments, reports and disenrollments as needed and assigned. 21. Completes discharges, disenrollments, transfers in a timely and efficient manner. 22. Completes all required clinical documentation and reports within 72 hours of the service date; updates consumer documentation as appropriate to maintain an accurate Electronic Medical Record; maintains record compliance in accordance with regulatory and agency expectations including, but not limited to assessments, demographics, consents, and treatment plans. 23. Responds to Crisis in a timely manner according to determined procedure. 24. Acts as liaison between Touchstone and collaborative agencies. 25. Maintains case load of at least 50 clients.
6 26. Bills an average of at least 100 units (25 hours) weekly, of which at least 60 units (15 hours) are family, individual, or group therapy, CFTs, treatment planning, intakes, or assessments. 27. Bills a minimum of 100 out-of-office units monthly unless providing specialized services requiring office visits that have been approved by Supervisor. *Performs all other duties as assigned SECTION IV: JOB QUALIFICATIONS Requires the following EDUCATION/TRAINING/CERTIFICATION/LICENSURE/EXPERIENCE: Requires a Master's degree in a behavioral health related field; Must be at least 21 years of age.
