3 Letter of Introduction Cathy Kerr Director of Adult and Community Services Dear candidate, Thank you for your interest in a role in the London Borough of Richmond-upon-Thames. This is an exciting and challenging time in the public sector and in health and social care. Here in Richmond, we are proud to be a high performing Council, delivering excellent services to our public. We are a national leader in personalised support, constantly evolving and looking at new ways to enable people to live as independently as possible and be more in control of their lives. The NHS is a key partner for us and we are bringing together commissioning and delivery of services to ensure local people experience joined up care and support. We aim to continue to deliver high quality health and social care whilst transforming our system to become more efficient. We therefore have a significant change programme in place to introduce new service models, integrate fully with our health partners and to develop new choices and options for local people. Richmond is a great place to work, set in a beautiful borough, but more importantly for me, a Council where you can be part of a strong and supportive team which is responding successfully to new challenges. 4 Commissioning Care Services in Richmond Commissioning Care Services is one of four divisions which make up the Adult and Community Services Directorate. It is headed by Derek Oliver, Assistant Director, and our services are arranged across 5 elements to meet the needs of all client groups. Our services are arranged across 5 elements, which are: Access Reablement Integrated health and social care teams for Older People, people with Physical Disability, and people with sensory loss Learning Disability team Integrated Community Mental Health Teams in partnership with South West London St George's Trust The Access Team Made up of social workers, sensory specialists, social care assessors and advisors, the Access Team is the first point of contact for Adult Social Services. It ensures that people receive good quality information, advice and guidance. The Team also offers a crisis response to people with immediate needs as well providing direct services and equipment. They screen and make referrals to the Reablement and Locality based Health and Social Care Teams. 5 The Reablement Team The Reablement Team is a multi-disciplinary team comprising Occupational Therapists and social workers. It providers a universal service offering people a short term, intensive rehabilitation programme aimed at maximising independence. The team is also responsible for ensuring safe and effective discharge from hospitals and has an important partnership role with both acute hospitals and local primary and secondary care services. Key Facts 22 posts in the team Short term, intensive service (for up to 6 weeks) Aim to help people regain their independence in their own homes Integrated Health and Social Care Teams Our older people and physical disability services are delivered by two Social Care Teams covering the following localities: Teddington and Twickenham Richmond and Barnes Each well established integrated team comprises Social Workers, Occupational Therapists and Social Care Assessors co-located with our health partners. They provide a wide range of services including: assessments, clinical intervention, support planning, review and carers assessment. They also undertake safeguarding Tel Fax
6 investigations to ensure that people are kept safe. The overall aim of the teams is to support vulnerable people to live meaningful lives and to manage their health conditions as independently as possible. The teams have close working relationships with local GP practices and a range of voluntary sector organisations. The teams are high performing with an excellent record in achieving all the key requirements of the service. Key Facts Teddington & Twickenham Team- 33 posts Richmond & Barnes Team- 29 posts The Learning Disability Team The Learning Disability Team comprises social workers who work in close collaboration with a specialist leaning disability health team. They provide long term support to adults and to young people in transition from children's services. They work closely with providers and local voluntary sector organisations to support people to live as active citizens. Key Facts 18 posts The team work alongside the 'Speciality Learning Disability Health Team' to ensure a cooperative approach to peoples' support Tel Fax
7 The Brokerage Team The Brokerage Team is a small team which places packages of care and contract for placements in residential and nursing care. The Integrated Community Mental Heath Teams London Borough of Richmond-upon- Thames has a formal partnership agreement with South West London and St George's Mental Health NHS Trust and East London NHS Trust to provide a joint mental health service. This incorporates both London Borough of Richmond-upon- Thames and all Trust staff who work in integrated teams, under the Trust's operational management. All LBRuT mental health operational staff are seconded to the South West London & St George's Mental Health NHS Trust, including professional social workers and support workers 7 Civic Centre, 44 York Street, Twickenham TW1 3BZ Tel Fax
8 About the Borough The is situated 15 miles South West of central London with excellent transport links -just 15 minutes from London Waterloo by train. It is the only London Borough to spread over both sides of the River Thames, which runs for 21 miles linking Hampton Court Palace, Richmond Town Centre and Kew Gardens. The borough has three main town centres; Richmond, Teddington and Twickenham, all bursting with local shops, cafes and restaurants. These bustling centres are offset by the presence of numerous green spaces and beautiful parks, giving Richmond its reputation as one of the most attractive Boroughs in London. Visitors are attracted by the charming riverside, historic houses, theatres and galleries. Not forgetting the sporting heritage too, for example the borough is home to Twickenham stadium; home of England rugby and the riverside boasts many rowing and sailing clubs. How to get here There are excellent communication links to our main offices in Twickenham. The Borough of Richmond is easily accessible from inner and outer London. South West Trains, Overground trains and. London Buses all serve the Borough. The train stations are: Richmond, Kew Gardens, Barnes, Barnes Bridge, Fulwell, Hampton, Hampton Wick, Mortlake, North Sheen, St Margarets, Strawberry Hill, Teddington, Twickenham and Whitton.
