Carer s self assessment Phone: Looking after someone is an important job, and it is not always an easy one. If you look after someone aged 18 or over, who would not be able to manage their daily living without your help, a carer s self assessment can help you access the information, advice and support you need. Domiciliary Care Phone: (Adult social care helpdesk) Contact the adult social care helpdesk for information on how to access care and support services in your home. Extra Care (Housing options) Phone: Extra Care is a new kind of accommodation that is specifically designed to help older people to continue to live independently. Live in your own self-contained home but with the security of having someone on hand 24 hours a day and a package of care that is designed to meet individual needs. Finance and benefits team (FAB) Phone: fai@portsmouthcc.gov.uk For information concerning the financial assessment process and other financial matters such as welfare benefits, this year s capital thresholds, or personal allowance rates. Housing options Phone: housing.options@portsmouthcc.gov.uk Housing options staff provide advice and assistance on a wide range of housing issues including homelessness and supported accommodation for people with special needs. 4 HIV and AIDS services Phone: (adult social care helpdesk) Confidential services for people affected by HIV or AIDS include counselling and emotional support, financial advice and practical assistance or care. Learning disabilities services (Kestrel Centre) Phone: The learning disabilities team assess the needs of people with learning disabilities and help to arrange support services. Mental health services Advice and support services for adults with mental health conditions are accessed through your GP. Occupational therapy Phone: Information for anyone experiencing difficulties at home because of mobility problems. Occupational therapy can advise on adaptations and equipment that can help you live as independently as possible. Portsmouth Rehabilitation and Reablement Team Integrated health and social care team that provides short term support to help you regain independence in your daily living tasks. The service is accessed through your GP, social worker or the hospital discharge team. Residential care Phone: (Adult social care helpdesk) There are many residential and nursing homes in Portsmouth providing care for people with a variety of different conditions and disabilities. Sensory impairment service Phone: Textphone: For people with visual or hearing impairments Shared Lives (based at Campion Place) Phone: Shared Lives is there to offer adults who need support the opportunity to live independently in a supportive family environment in the community. Sheltered or supported housing (Housing options team) Phone: Our sheltered accommodation is mainly for older or vulnerable persons who need some support with their housing. Each property has a community alarm system for help in an emergency and residents can benefit from communal areas, prepared meals and domestic assistance if needed. You don t have to be a current council tenant to apply. Social Care info Portsmouth (SCiP) Website: The SCiP website contains information, advice and a directory of local services in 8 categories. These are Daily Living, Getting Around, Health & Wellbeing, Education Training & Employment, Carers, Things to Do, Money and Staying Safe. ASC Jan 2012 Page 5
6 Substance misuse Phone: Services for people affected by drug or alcohol addiction include a structured day care programme, women-only groups, support and advice. Open access sessions based at Fratton Community Centre, Portsmouth CDA or Kingsway House. Support groups Website: There are many different support and social groups in Portsmouth.
