Feinman, D.O., FACOS Chief Medical Officer CA WEST FLORIDA CA West Florida completed a very successful year, contributing more than $1.8 billion to the economic health of west and central Florida. As one of the largest employers, our contributions are significant, from economic to clinical and far exceeding other health systems in a variety of publicly reported metrics. With 25,000 employees, physicians, and volunteers who serve the area s unique and diverse communities, we are committed to providing the highest quality medical care, while supporting the region where we live, work, and raise our families. At a time when many businesses are cutting back, CA West Florida has significantly expanded our operations with the opening of the $186 million Medical Center of Trinity in early 2012 and our first freestanding ER in Plant City. We are expediting construction for a new hospital to serve the residents of southeast Osceola County, Poinciana Regional Medical Center, operating two Level II Trauma Centers, completing Phase I of a $52 million expansion in ernando County, significantly expanding our medical education and training initiatives, recruiting and employing more physicians to meet community needs, and breaking ground for a second freestanding ER in Oviedo. All of these projects are bringing needed medical services and specialists to the area and will serve as an economic base for future growth and expansion. In a state still recovering from a recession, CA West Florida s contributions as a major tax payer and employer are far reaching. We provided $1.17 billion in salaries and benefits in west and central Florida and paid more than $108 million in taxes in This is an increase of $33 million in tax dollars paid over the previous year. With the need to provide medical care as efficiently as possible, and our ability to secure national contracts, we were still able to demonstrate our commitment to the communities we serve by purchasing $172 million in goods and services locally. Our clinical teams provided outstanding care to more than 1.2 million patients last year, as evidenced by 12 of our 15 hospitals named Top Performers, a special quality recognition by the nation s leading healthcare accrediting body, The Joint Commission. We ended the year with an unprecedented fourth quarter composite inpatient CMS Core Measures score of percent. This places our hospitals in the top decile for quality, as defined by these objective measures of quality. The Leapfrog Safe ospital score, a key industry ranking for patient safety, awarded our CA hospitals a total of 10 A ratings in Finally, CA West Florida hospitals also earned 41 disease-specific certifications from The Joint Commission. With one-third of the state s budget spent on healthcare, CA understands the importance and significance of providing high quality and cost effective healthcare. CA West Florida will continue to work with local, state, and national governments as well as with area business leaders to ensure continued access to healthcare for the residents of west and central Florida. 3 C A W e s t F l o r i d a Oviedo ER Groundbreaking Growing To Meet The Needs Of West & Central Florida Communities Poinciana Medical Center will open in summer 2013 as a 110,000 square foot acute care hospital, featuring 24 private medical-surgical beds and a six-bed intensive care unit. The facility will provide a full range of acute care services including emergency, diagnostic imaging, inpatient and outpatient surgery, cardiac catheterization, laboratory and pharmacy. The Emergency Department is approximately 11,000 Poinciana Construction square feet with 12 exam rooms and will include an EMS entrance and helicopter pad for rapid transport of critically ill patients to Osceola Regional Medical Center. Freestanding ERs CA West Florida built and opened its first freestanding ER in October, enabling Brandon Regional ospital to expand services into Plant City. Freestanding ERs provide around the clock emergency care and are staffed, equipped and function as a hospital ER, but can be located in adjacent communities to improve access. Groundbreaking for a second freestanding ER was held in December in Oviedo, which will open later this year as a department of Central Florida Regional ospital. 8 Our Commitment To Excellence Quality And Patient Safety Putting Patients First is the foundation of our culture and passion for providing quality healthcare. Many of our initiatives serve as national models today, as we lead the way in adopting best practices to eliminate clinical variations. Clinical Excellence CA West Florida hospitals achieve industry leading performance on public clinical measures, including CMS Core Measures. We completed 2012 with a preliminary fourth quarter Core Measures composite inpatient score of 99.78%, and 12 of our 15 hospitals earned national recognition as Top Performers in Key Quality Measures from The Joint Commission in the areas of eart Attack, eart Failure, Pneumonia and Surgical Care. Larry J. Feinman, DO, FACOS Chief Medical Officer 6 Chief Medical Officer CA West Florida welcomed Chief Medical Officer (CMO) Dr. Larry Feinman to provide physician leadership, support and oversight to our robust quality and patient safety agenda. Dr. Feinman is an accomplished general and vascular surgeon who most recently served as CMO at Largo Medical Center. Today, he collaborates with CMOs and medical staff leadership throughout the division to enhance the reduction of clinical variation, support the expansion of graduate medical education programs and promote our quality and clinical decision support programs. CA WEST FLORIDA We earned 10 A ospital Safety Score ratings in 2012, which uses national performance measures from the Leapfrog ospital Survey, the Agency for ealthcare Research and Quality (ARQ), the Centers for Disease Control and Prevention (CDC) and the Centers for Medicare and Medicaid (CMS) to produce a single score representing a hospital s overall performance in keeping patients safe from preventable harm and medical errors. Our hospitals are saving lives, reducing complication rates and eliminating unnecessary costs through a strong focus on quality. We are achieving and exceeding national clinical goals and benchmarks. These efforts are strongly supported by CA s Clinical Services Group, an industry leader in bringing evidenced based medicine to the bedside in community hospitals nationwide. AIM for Zero The goal is to eliminate Central Line- Associated Blood Stream Infections (CLABSI) through the consistent application of proven techniques to protect patients from preventable blood stream infections during their hospital stay. Flu Season Preparation We are making our hospitals safer for our patients and visitors through our flu vaccine initiative. This includes providing free vaccines to staff and requiring vaccinations of health workers with direct patient contact. When a vaccine is not possible, the caregiver wears a mask. Airstrip OB & Cardiology Several of our hospitals have adopted this technology that allows physicians to remotely access and view patient monitors from their smartphones. Broselow System for Acute Pediatric Administration of Drugs All of our hospital emergency departments, nurseries and pediatric programs are equipped with this sophisticated medication safety system to ensure the proper dosing and administration of drugs to infants and children.

9 C A W e s t F l o r i d a St. Petersburg General ospital is one of only two hospitals in Florida designated as a GYN Robotic Epicenter. Physicians from around the country come to train on the da Vinci robotic system and learn from the hospital s best industry practices. Maternal/Fetal Safety Initiatives CA took a leadership role with the March of Dimes and other national organizations by participating in initial research studies to minimize or eliminate elective deliveries before 39 weeks gestation. Study results showed a positive impact on the health of babies. Today this is a standard practice in our hospitals. We were the first US healthcare system to perform a blood test to screen newborns for kernicterus, which can cause permanent brain damage and physical impairment. If identified immediately, it is easily treatable. Today, this screening is a standard of care in hospital newborn nurseries across the county and as a result the incidence of kernicterus has been dramatically reduced. All of our obstetrics programs adhere to the latest in patient safety guidelines, with Brandon Regional ospital providing Level III Neonatal Intensive Care Unit (NICU) services, the highest level available. Osceola Regional Medical Center offers a Level II NICU program and around the clock obstetrical coverage through its OB/ GYN hospitalist program. Medical Education More than 200 physicians came to the area last year to complete their medical education in CA sponsored programs. Studies indicate that many of these physicians will make the community where they complete their graduate medical education their permanent home. Largo Medical Center, which has been conferred the status of Statutory Teaching hospital by the Agency for ealthcare Administration, is home to the largest osteopathic teaching program in the southeastern United States and features 13 medical residency/fellowship training programs on campus. St. Petersburg General ospital and Northside ospital also sponsor medical training programs including Traditional Rotating Internship and Family Practice Residency. We are finalizing plans now to open new programs at Oak ill ospital and Osceola Regional Medical Center in 2014 and programs at other facilities in Medical Research Eight West Florida CA hospitals participated in the national Randomized Evaluation of Decolonization versus 2013 COMMUNITY REPORT 7

10 Our Commitment To Excellence Universal Clearance to Eliminate (REDUCE) MRSA Trial study in conjunction with investigators at arvard and the Agency for ealthcare Research and Quality (ARQ) and the Centers for Disease Control and Prevention (CDC). Results released in October show universal treatment of all patients in the Intensive Care Unit (ICU) resulted in a 37 percent reduction of risk of methicillinresistant staphylococcus aureus (MRSA) clinical isolates and a 44 percent reduction of bloodstream infections by all pathogens. As a result of this groundbreaking study, CA is rolling out universal decolonization company-wide. Another research focus is on improving trauma care in Florida through our collaboration with USF ealth. Level II Trauma Centers at Blake Medical Center and Regional Medical Center Bayonet Point are participating in gathering information that will be used to develop best practices in trauma care. Largo Medical Center is actively engaged in clinical research. Current research studies conducted by the medical staff involve cardiac and primary care related issues. The residents/ fellows also participate in research as part of their training. The Central Florida Cardiac & Vascular Institute at Osceola Regional Medical Center and Northside ospital are cardiovascular clinical research strategic sites, affiliated with the Sarah Cannon Research Institute of Nashville (SCRI). SCRI is one of the largest clinical research programs, conducting community-based clinical trials in oncology and cardiology through affiliations with a network of more than 700 physicians. It was formed in 2004 through a joint venture between CA and Tennessee Oncology. Additionally, many of our hospitals work with physicians engaged in research and clinical trial initiatives in multiple specialty areas. Radiation Right Radiation Right applies patient safety principles and actions to medical imaging and radiation therapy with the goal of protecting patients from the harmful effects of radiation overexposure. Radiation Right focuses on providing each patient with the Right Exam, of the Right Site, in the Right Dose. Initially developed by the CA Clinical Services Group, this program has been adopted by all CA West Florida hospitals and imaging centers. Sepsis Program Severe sepsis is now one the most common causes of death in ICUs nationally. We are aggressively working to reduce this patient safety threat through hospital and community-wide focus on early recognition and management. STEMI Initiative A STEMI (ST-Elevation Myocardial Infarction) is a deadly type of heart attack, which is identified on an EKG. Early treatment to the blocked coronary artery minimizes the extent of the heart muscle damage and preserves the pumping function of the heart, thereby increasing survival and reducing or eliminating complications. CA West Florida hospitals are saving lives through an aggressive focus on treating STEMI patients quickly. Partnering with EMS has allowed medical treatment to begin immediately on the scene, before the patient arrives in our emergency departments. STEMI times are defined as the time it takes to reestablish blood flow and the American College of Cardiology, the American eart Association and the Joint Commission have established that this should occur in 90 minutes or less. Our hospitals are averaging times well below this and, in most cases, are posting STEMI times on their hospital websites. CA West Florida hospitals have made great strides, reducing STEMI times and PCI complication rates, with 14 of our hospitals Chest Pain Center accredited and 11 offering Percutaneous Coronary Intervention (PCI) services. Vitals Now We piloted this technology to enable the automatic capture of patient vitals from the bedside into our clinical system, making them immediately available for review by clinicians. 8 CA WEST FLORIDA

11 C A W e s t F l o r i d a Specialized Care Emergency Excellence & Trauma Care We are a national leader in improving ER processes to enhance patient care and reduce wait times. From stroke care to ST-Elevation Myocardial Infarction (STEMI) times, we are saving lives and improving the quality of lives through advanced technology delivered quickly. Last year, 618,511 patients received care in our ERs. All 15 hospitals are Primary Stroke Centers and 14 are Accredited Chest Pain Centers, most with PCI services to treat STEMI patients immediately. Our STEMI times are well below the national goal of 90 minutes and we post these on hospital websites along with the ER wait time. 12 Our Commitment To Excellence Regional ospital, Osceola Regional Medical Center and Oak ill ospital posting Pediatric ER wait times on their websites and by texting ER to eart & Vascular Network Several exciting new cardiovascular services opened this past year, including Blake Medical Center successfully performing the first transcatheter aortic valve replacement (TAVR) in Manatee County. Valve Clinics opened at Brandon, Osceola and Blake, while Regional Medical Center Bayonet Point opened its comprehensive Arrhythmia Center. Words cannot express the gratitude I have for the doctors and nurses at Osceola Regional Medical Center for skillfully saving my life. To say that the nursing staff was excellent doesn t do them justice. They were the bloodline of care I needed to pull through and recover. Alexis Lex Cooper Emergency Thoracic Surgery We are also extending our ER care beyond the immediate community through freestanding ERs. Freestanding ERs provide around-the-clock emergency care and are staffed, equipped and function as a hospital ER, but can be located outside of a hospital s immediate service area, often in communities needing better access to services. Brandon Regional ospital opened a Plant City satellite freestanding ER this past year and Central Florida Regional ospital broke ground on a freestanding ER in Oviedo, which will open in In younger communities, several of our hospitals now provide pediatric emergency care in dedicated units, with Brandon Our cardiovascular network ensures that life-saving technology and minimally invasive techniques are available. The latest surgical techniques for vascular, minimally invasive mitral valve and atrial fibrillation repair provide a safer alternative for patients suffering from these conditions. Our Valve Clinics can identify patients early in the disease process, when more treatment options are available to improve outcomes and quality of life. The TAVR procedure offers patients who are unable to undergo traditional open-heart surgery, or chose not to, a minimally invasive option to surgical aortic valve replacement therapy. Fawcett Memorial ospital received state approval for an Open eart Surgery program, which will open in spring Several of our hospitals have earned certifications and recognition for the outstanding cardiac and cardiovascular care they provide. Comprehensive Cancer Care Cancer is the second leading cause of death in this country, exceeded only by heart disease. Our hospitals have committed to providing the highest quality of care by striving to meet or exceed the many standards for accreditation. Nine CA West Florida hospitals have already earned the Accredited Cancer Center distinction by the American College of Surgeons Commission on Cancer (ACoS CoC). Tumor Board reviews, screenings and education programs, and access to the latest diagnostic and treatment technologies are just a few of the ways we accomplish this. From digital mammography to robotic surgical technology, our hospitals are providing physicians and surgeons the latest techniques and services to treat patients. 2U ealth to You (2U) provides wellness solutions through its consumer and employer based programs. Orthopedics National recognition for clinical excellence has been awarded to many of our orthopedic programs, including Disease Certifications from The Joint Commission, and Blue Cross Blue Distinction and Unitedealth Premium designations. Our surgeons are committed to performing the latest innovations in their field, including minimally invasive procedures. Many of our hospitals offer classes in advance of surgery to assist patients with planning for their hospital stay and recovery after discharge. Several hospitals offer luxury accommodations and amenities to encourage quick return to daily activities and family involvement, often in conjunction with spine programs. Spine Care Spine injuries can be the result of sudden trauma or unrelentingly degenerative. Either way, they can greatly impact an individual s quality of life. Our network offers a full spectrum of resources to provide an accurate diagnosis and treatment plan, and many programs have earned Joint Commission Certifications and Blue Distinction and Unitedealth Premium designations. Our spine surgeons perform the most minimally invasive procedures available today, with outstanding outcomes and greatly reduced hospital stays.
