3 conference overview Skill preparation and planning for health care transition for children and youth with special health care needs (CYSHCN) is inadequate. Less than 50% of families nationwide indicate that their CYSHCH have received the services necessary to make appropriate transitions to adult health care, work, and independence. One of the major problems to receiving this care is the lack of a physician workforce trained in providing the services necessary to make the health care transition. There is a strong need on the part of physicians to have the knowledge and skills to provide services to facilitate successful transition from pediatric to adult-based care and services. Due to increased awareness in transitioning to adult-based care, clinicians are required to be updated on changing strategies for integrating emerging adult-based care into practice. For all conference participants, there will be an update of the legal issues involved in health care transition, as well as time to meet and talk with faculty, to exchange ideas among participants, and share knowledge and information about how best to plan for a successful transition to adulthood. TARGET AUDIENCE Physicians in internal medicine, family practice, pediatrics psychiatry, and physiatry; other health care providers including, psychologists, social workers, nurses, dietitians, case managers, counselors, and primary care providers; youth and young adults with chronic illness and their parents/guardians. OBJECTIVES At the conclusion of the conference, participants should be able to: Identify the current state of health care transition in the U. S., including legal and financial issues. Develop a practice-based framework for implementing health care transition services. Evaluate youth and young adults with special health care needs presenting with complex medical conditions and identify appropriate transition services. Identify barriers and successful strategies when placing youth and young adults with special health care needs with behavioral health and intellectual disabilities into employment, housing, and available medical transition services. Identify the core elements and measures of a quality improvement assessment. 4 ACCREDITATION/credit DESIGNATION Physicians is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians. designates this live activity for a maximum of AMA PRA Category 1 Credits. Physicians should claim only the credit commensurate with the extent of their participation in the activity. The presentation, Legal Issues in Transition, has been designated by Baylor College of Medicine for.75 credit of education in medical ethics and/or professional responsibility. Nursing Earn up to 12.9 contact hours by attendance at this activity. Texas Children s Hospital is an approved provider of continuing nursing education by the Texas Nurses Association, an accredited approver by the American Nurses Credentialing Center s Commission on Accreditation. Social Work Texas Children s Hospital is an approved continuing education provider for Social Workers through the Texas State Board of Social Worker Examiners. Social work continuing education units provided, 11 hours of continuing education including 1 hour of ethics. Occupational Therapy and Physical Therapy contact hours or 13.5 CCUs have been applied for this course through TPTA and TOTA. Texas Children s hospital is an Approved Provider for the Texas Occupational Therapy Association. The OCME reserves the right to limit the number of participants in an activity and will not be responsible for any expenses incurred by an individual whose registration is not confirmed and for whom space is not available. Cancellation/Refund Policy Requests for registration refunds must be in writing and received by the OCME at least 10 business days before the activity begins. Cancellations are subject to a $50 administrative fee deducted from the registration fee paid to cover guarantees and other expenses. A variety of research topics of interest to researchers, clinicians, policy makers, parents, youth and emerging adults are presented. Topics include testing of HCT models of care and programs, HCT readiness, and provider issues. Two poster sessions on HCT research will enable direct dialogue with researchers. Two breakout sessions will address interdisciplinary perspectives pertaining to HCT education and curriculum development and the creation, maintenance, and evaluation of HCT programs. registration Registration is required for all participants.
