6 Sustainability: Recommended Criteria for Diversity Action Plans Whatever endeavor you need to ask Can we sustain change/efforts? What will it take to do so? Consider developing a sustainability plan; determine goals for scaling Action Coalitions can serve as a Resource for health systems Offer Best Practices in mentorship, recruitment and retention of diverse practicing nurses.

7 Sustaining Diversity Across Pillars Leadership Engage new partners (including those outside of health care that serve underrepresented groups, and those that can strengthen Action Coalition s potential for growth (i.e. fundraising skills). Education School age students (specifically target middle and high schools with high minority populations) through graduate students in nursing. # of number of new school/academic partnerships formed or strengthened) Clinical Practice Use data to emphasize important health outcomes that can be improved with a diverse nursing workforce.

9 Forming a Diversity Strategy for the Louisiana AC Louisiana s Challenges Louisiana ranks 48 th in terms of the overall health of its population according to America s Health Rankings (2014). Black adults in Louisiana have the highest mortality rates due to diabetes, heart disease, and cancer among all racial ethnic groups. Black and Hispanic adults and adolescents have the highest estimated rates of HIV diagnoses per 100,000 when compared to whites and other racial ethnic minorities in Louisiana. Hispanics in Louisiana have the highest poverty rate (39%) among all racial ethnic groups, with Blacks falling close behind at (34%). America s Health Rankings, 2014; Kaiser Foundation State Health Facts, 2015
