, Chronic Disease Prevention POSITION SUMMARY Position open for the Nurse Program Coordinator of the Peak Wellness and Chronic Disease Community Health Team programs addressing cardiovascular disease, diabetes and cancer prevention. Coordinates, in partnership with the nutrition coordinator, the work of a multidisciplinary team of 8-10 and directly supervises three nurses and one administrative assistant. Responsible for planning, development and implementation of the Chronic Disease Community Health Team program in close partnership with the nutrition coordinator and nursing manager. Responsible for supervision and staffing of all Chronic Disease Community Health Team activities, as well as orientation and training. Responsible for development, implementation and evaluation of programmatic policies and procedures. Requires establishing partnerships and maintaining collaboration with multiple community partners. Will work with primary care clinics and community based organizations. Serves on TCHD work groups and committees and community meetings and activities. GENERAL STATEMENT OF DUTIES Performs technical and advanced practice nursing leadership and work in the coordination and administration of an assigned public health nursing program(s). Coordinates agency and community activities and assesses community needs relative to the services provided by the Tri-County Health Department. REPORTING RELATIONSHIPS Supervision Received: General supervision is provided by a Nursing Manager, Associate Director of Nursing or Director of Nursing. Supervision Exercised: Provides direct and indirect supervision through second-level supervisors, employees, and functions within selected nursing programs. Oversees policy and procedure standards and evaluates performance of direct reports and program operations. Recommends and administers actions impacting employee pay and status, hiring and discipline. May act as a resource or expert in program areas providing technical guidance and education to less experienced staff. Oversees staff development of all nursing program personnel, including orientation, inservice training. Coordinates and conducts staff trainings, instruct staff in the methods, techniques, procedures, and protocols utilized in assigned programs. Ensures compliance with program guidelines and objectives. Assesses community needs, assists with development of program objectives, and evaluates outcomes. Responsible for monitoring compliance with program contracts, monitors budgets, expenditures and billing processes of assigned program. Assists in program evaluations and makes recommendations to nursing administration concerning planning activities; recommends program and policy changes. Participates in local, state and community boards and other community outreach activities to promote understanding and support of public health nursing programs and to promote local, state and national standards. Collaborates with professional and community groups to develop goals and increase access to public health care programs. Coordinates and integrates program activities with other nursing programs, TCHD divisions, allied agencies, service providers, and community groups. Provides consultation, leadership and management in specialized program areas. Provides technical expertise to TCHD staff in the interpretation of program policy, procedures, and protocols. Participates in nursing, emergency preparedness and other program staff team conferences in order to provide expertise and coordination of activities. ADDITIONAL DUTIES Participates in various committees and boards related to program activities and responsibilities. EMERGENCY PREPAREDNESS AND RESPONSE Employee responds, as required, to support public health emergencies, incidents and events. Employee participates in all exercises and drills on emergency preparedness, as required. Completes trainings identified as appropriate for this level employee. KNOWLEDGE, SKILLS, OR ABILITIES Education and Experience: Requires the minimum of a Bachelor s Degree in nursing from a school accredited by the National League for Nursing Accrediting Commission or the American Association of Colleges of Nursing plus advanced education and/or experience in specialty area. Requires a minimum of two years progressively responsible public health nursing experience including supervisory experience. Master s degree preferred with nursing background or experience. Special Qualifications: Licensed to practice as a registered nurse (RN) in the State of Colorado. Possession of a valid driver s license. Knowledge of: Principles, practices, techniques, and protocols related to community health nursing and its application in a public health program. Practical implementation of educational and presentation methods for diverse population groups. Comprehensive knowledge of research methods and curriculum development. Inter-related professional services and resources in the Tri-County Health Department and community for the implementation of programs. Supervisory techniques and methods. Program planning and program development. Basic computer applications and health data analysis. Page 3
Ability to: Supervise and mentor professional and support staff in a productive and positive manner. Establish and maintain effective working relationships with other Tri-County Health Department employees, representatives of other agencies and organizations, and members of the community. Communicate effectively both verbally and in writing to provide instruction and educational information to individuals and groups. Act as an official representative of TCHD in a professional manner with local and national agencies and organizations. Maintain sensitive and confidential client information according to the HIPAA policy. EQUIPMENT USED Standard office equipment including, but not limited to, personal computer and associated hardware and software, telephone, copier, calculator, and fax. Standard medical equipment such as blood pressure cuff, weight scales, stethoscope, thermometer and syringes, vaccines and blood borne pathogens kit, assessment and vehicles. WORKING CONDITIONS AND PHYSICAL REQUIREMENTS Work is primarily conducted indoors with occasional travel to other TCHD locations or outside agencies to provide program services or attend meetings. Incumbent experiences routine exposures and hazards of a typical office environment. May require frequent travel between office sites or to locations in the surrounding community for conducting educational presentations and attending meetings. 1. Requires sufficient hearing and speech ability to communicate verbally in response to inquiries, complaints, and to speak to groups of people in an educational presentation. 2. Requires occasional light lifting/carrying under 15 pounds. 3. Occasional use of automobile, usually no more than one hour at a time. 4.
